Following Measles Outbreaks, Officials Grow Wary of Renewed Threat

Cases this year have already topped the total in 2023. Unvaccinated travelers account for most infections.

Measles, a highly contagious but preventable disease, is resurging in pockets of the United States, a warning of the dangers of the strengthening anti-vaccine movement.

The Centers for Disease Control and Prevention has recorded more cases this year than the 58 tallied in all of 2023, although the agency is not expected to release exact numbers until Friday. On Monday, the agency advised health care providers to ensure that unvaccinated patients, especially those traveling internationally, stay updated on their immunizations.

The number of cases is likely to keep rising because of a sharp spike in measles worldwide, along with spring travel to some regions with outbreaks, including Britain, said Dr. Manisha Patel, chief medical officer at the C.D.C.’s respiratory disease division.

Nearly all the cases in the United States so far are related to unvaccinated travelers. “We’re not going to see widespread measles cases going throughout the country,” Dr. Patel said. “But we do expect additional cases and outbreaks to happen.”

Measles is among the most contagious of diseases; each infected person can spread the virus to as many as 18 others. The virus is airborne and can stay aloft up to two hours after an infected person has left the room, spreading rapidly through homes, schools and child care facilities.

In Chicago, one case of measles at a migrant shelter has grown to 13 , prompting the C.D.C. to send a team to help contain the outbreak. (Two additional cases in the city appear to be unrelated.)

In Florida, seven students at an elementary school contracted measles even as the state’s surgeon general, Dr. Joseph Ladapo, left it to parents to decide whether unvaccinated children should attend school.

In southwest Washington, officials identified measles in six unvaccinated adult members of a family living in two counties. And in Arizona, an international traveler infected with measles dined at a restaurant and transmitted the virus to at least two others .

Measles was eliminated in the United States in 2000, and American children generally must be immunized to attend school. Yet sporadic cases lead to larger outbreaks every few years. But now a drop in vaccination rates, exacerbated by the coronavirus pandemic, has experts worried about a resurgence.

When vaccinations lag, “the first disease to appear is measles, because it’s highly infectious,” said Dr. Saad Omer, dean of the O’Donnell School of Public Health at U.T. Southwestern in Dallas.

Nine of 10 unvaccinated people in close contact with a measles patient will become infected, according to the C.D.C.

Measles is far less deadly in countries with high immunization rates and good medical care. Fewer than three of every 1,000 American children with measles will die as a result of severe complications like pneumonia or encephalitis, the swelling of the brain.

Still, about one in five people with measles may end up in a hospital.

Because widespread measles outbreaks have been rare, most Americans, including doctors, may not recognize the vibrant red rash that accompanies respiratory symptoms in a measles infection. They may have forgotten the impact of the disease on individuals and communities.

“Most of our local health department folks have never seen a measles outbreak,” said Dr. Christine Hahn, state epidemiologist of Idaho, which contained a small cluster of cases last year.

“It’s going to be a big challenge to us to respond if and when we get our next outbreak,” she said.

Before the first measles vaccine was introduced in the 1960s, the disease killed an estimated 2.6 million people worldwide each year. But its full impact may have been much greater.

Measles cripples the immune system, allowing other pathogens easier entry into the body. A 2015 study estimated that measles may have accounted for as many as half of all infectious disease deaths in children.

For about a month after the acute illness, measles can stun the body’s first response to other bacteria and viruses, said Dr. Michael Mina, chief science officer of the digital health company eMed and formerly an epidemiologist at the Harvard T.H. Chan School of Public Health.

That leaves patients “massively susceptible to bacterial pneumonias and other things,” said Dr. Mina, who was the lead author on the 2015 study.

“It’s very risky for people in those first few weeks post measles,” he added.

The virus also induces a sort of immune-system amnesia. Normally the body “remembers” the bacteria and viruses it has fought before. Dr. Mina and his colleagues showed in 2019 that people who have measles lose between 11 and 73 percent of their hard-won immune repertoire, a loss that can last for years.

That does not mean the body no longer recognizes those pathogens at all, but it does shrink the arsenal of weapons available to fight them.

“People should be aware that if they’re choosing not to vaccinate, that’s the position they’re putting themselves and their family in,” Dr. Mina said.

The C.D.C. recommends receiving the first dose of the measles vaccine after 12 months of age, and a second between ages 4 and 6. Even a single dose of the vaccine is 93 percent effective. Measles vaccination averted 56 million deaths between 2000 and 2021, according to the World Health Organization.

Vaccination rates in the United States have shown a distinct, if small, dip to 93 percent in the 2022-23 school year from 95 percent in 2019-20 — the level required to protect everyone in the community. Rates of vaccination exemptions increased in 40 states and the District of Columbia.

In a survey last year , just over half of Republicans said that public schools should require measles vaccinations, compared with about 80 percent before the pandemic. (Support for vaccines among Democrats held steady.)

While national or state-level vaccination rates may be high, there may be pockets of low immunization that provide tinder for the measles virus, Dr. Omer said.

If there are enough unvaccinated cases to sustain an outbreak, even those who are vaccinated but whose immunity may have waned are vulnerable, he said.

In Idaho, 12 percent of kindergarten-age children do not have a record of vaccination. Some of the gap results from parents unable or unwilling to share records with the schools, and not because their children are not immunized, Dr. Hahn said.

Still, online schools, which proliferated through the pandemic and remain popular in the state, have some of the highest rates of vaccine exemptions, she said.

In September, a young Idaho man brought measles back after international travel and became ill enough to be hospitalized . Along the way, he exposed fellow passengers on two flights, dozens of health care workers and patients, and nine unvaccinated family members. All nine developed measles.

Idaho got “very lucky” with the outbreak because the family lived in a remote area, Dr. Hahn said. But there are most likely many other areas in the state where an outbreak would be difficult to contain.

“We’ve got plenty of tinder, if you will,” she added.

Some large outbreaks in recent years exploded among huge clusters of unvaccinated people, including the Amish in Ohio and the Orthodox Jewish community in New York City.

In September 2018, one unvaccinated child returned to New York City from Israel, ferrying measles virus picked up during an outbreak in that country.

Even though the city maintains high vaccination rates, that single case set off an outbreak that raged for nearly 10 months, the largest in the country in decades. The city declared a public health emergency for the first time in more than 100 years.

“We had more than 100 chains of transmission,” said Dr. Oxiris Barbot, the city’s health commissioner at the time, and now the president and chief executive of United Hospital Fund.

“Keeping all of that straight was a challenge,” she recalled. “And to have to investigate over 20,000 exposures like that, that was huge.”

Working with community leaders, city officials hurriedly administered about 200,000 doses of vaccine. More than 550 city staff members were involved in the response, and the final cost to the city’s health department topped $8 million.

The C.D.C. is working with state and local health departments to identify pockets of low vaccination and prepare them for outbreaks, Dr. Patel said. The agency is also training health care providers to recognize measles symptoms, particularly in patients with a history of international travel.

Measles is a slippery adversary, but public health is intimately familiar with the tools needed to contain it: screening, tracing contacts and vaccinating the susceptible.

“We’re not helpless bystanders,” Dr. Omer said. “The focus needs to be on meat-and-potatoes public health.”

Donald Trump won a legal victory in the Georgia election fraud case against him Wednesday, when a judge granted him the right to appeal his ruling allowing District Attorney Fani Willis to stay on the case.

Fulton County Judge Scott McAfee granted the former president’s motion seeking an immediate review by Georgia’s Court of Appeals of his bombshell ruling on Friday allowing the embattled DA to continue prosecuting the case, despite her affair with lead prosecutor Nathan Wade.

Trump and seven other co-defendants seeking to boot Willis did not have an automatic right to appeal before the start of a trial.

However, McAfee granted the rare measure, citing the importance of the case. The appeal also delays a trial from being scheduled, making it highly unlikely it would take place before the election in November.

The defendants must now wait to see if the appeals court agrees to hear the case.

Trump lawyer Steve Sadow called McAfee’s Wednesday decision “highly significant” and added he’s hopeful the appeal will win.

“The defense is optimistic that appellate review will lead to the case being dismissed and the DA being disqualified,” he added.

The trial against Trump and 14 remaining co-defendants has effectively screeched to a halt since co-defendant Mike Roman’s lawyer filed explosive papers in January revealing Willis and Wade’s affair and calling for her to be disqualified for alleged improper conduct.

After hearing two days of testimony last month — including from Willis and Wade — McAfee found the pair didn’t have a conflict of interest in the case, but the mere appearance of wrongdoing meant that one of them had to step down.

Wade resigned from the case the same day.

What to know about District Attorney Fani Willis' trial

• Fulton County, Ga., District Attorney Fani Willis is being accused of misusing state and federal funds, and also engaging in an “improper” relationship with special prosecutor Nathan Wade .
• Willis admitted to having a “personal relationship” with Wade, but said it didn’t become romantic until after 2022 due to the case against former President Donald Trump.
• Willis hired Wade to work on the Trump case and paid him $654,000 in 2022, according to Trump co-defendant Michael Roman.
• Trump and his co-defendants are looking to disqualify Willis from the case and to have all charges, centered around the state’s expansive anti-racketeering RICO law, dismissed.
• The defense has presented dozens of pings from Wade’s cellphone that placed it at Willis’ rented condo prior to 2022. A former friend of Willis, who owned the condo, has testified that she saw the two of them “hugging” and “kissing” in 2019.
• On March 15, a judge ruled Willis can stay on and prosecute the Georgia election interference case against Trump and his co-defendants for allegedly trying to overturn the results of the 2020 election — if Wade steps aside.

But lawyers for Trump and the others slammed McAfee’s ruling — which was highly critical of Willis — claiming it didn’t go far enough and the whole prosecutorial team needed to go.

In his motion asking to appeal, Sadow and the attorneys criticized Willis’ January  speech at Atlanta’s Big Bethel AME Church , where she claimed the backlash against her and Wade was racially motivated because they are both black.

They said this would mean the defendants would not be able to get a fair trial, writing: “Whether District Attorney Willis and her office are permitted to continue representing the State of Georgia in prosecuting the defendants in this action is of the utmost importance to this case, and ensuring the appellate courts have the opportunity to weigh in on these matters pre-trial is paramount.”

In January, Sadow had filed  another motion to have Willis taken off the case , saying her comments at the church were “falsely and intentionally injecting race into this case.”

Keith Johnson, a Georgia-based defense attorney and former prosecutor, told The Post Wednesday that McAfee’s decision allowing the appeal pre-trial is rare and said that until the matter is decided, the case will effectively “be at a standstill.”

“Practically speaking, the trial will not proceed until the Georgia Court of Appeals makes a ruling on Judge McAfee’s order to allow District Attorney Willis to remain as a prosecutor on the case,” Johnson said.

Willis’ office didn’t immediately return a request for comment Wednesday morning.

President Vladimir Putin  has arrived for his first-ever presidential visit to Chukotka in Russia 's Far East - just 55 miles from the US state of Alaska . 

Putin arrived in Anadyr, the local capital of the Chukotka region this morning after flying from Moscow some nine time zones away. 

Chukotka is the easternmost region of Russia, with a maritime border on the Bering Strait with Alaska.

The Russian president was met in Anadyr by a motorcade and was whisked away in a limousine amid frigid temperatures of -28C. 

It's the closest he has come to US soil since he met with President  Barack Obama in New York City in 2015.

Chukotka is so close to Alaska that Roman Abramovich - the ex-Chelsea FC owner - was reported to fly to Anchorage in Alaska for lunch when he was the governor of the region from 2001 - 2008.

Putin's visit comes at a time when US-Russian relations are at their lowest ebb in decades amid the war in Ukraine and a growing East-West divide. 

Meanwhile, Ukrainian President Volodymyr Zelensky today landed in Lithuania as part of an unannounced trip to the Baltic states to drum up more support for the conflict.

The three Baltic states - all former Soviet republics which are now EU and NATO members - are among Ukraine's staunchest allies.

'Estonia, Latvia, and Lithuania are our reliable friends and principled partners. Today, I arrived in Vilnius before going to Tallinn and Riga,' Zelensky said on social media platform X, formerly Twitter.

'Security, EU and NATO integration, cooperation on electronic warfare and drones, and further coordination of European support are all on the agenda,' he said.

The Baltic tour marks Zelensky's first official trip abroad this year.

In Lithuania, a key donor to Ukraine, Zelensky said he will hold talks with the president, prime minister and the speaker of parliament, and meet with the Ukrainian community.

The visit comes as other Kyiv allies waver on fresh aid, nearly two years into Russia's invasion.

Ukraine has come under intense Russian shelling in recent weeks, retaliating with strikes on Russia's border city of Belgorod.

Zelensky has urged allies to keep military support flowing and held in-person talks with officials from the United States, Germany and Norway last month.

But an EU aid package worth 50 billion euros ($55 billion) has been stuck in Brussels following a veto by Hungary, while the US Congress remains divided on sending additional aid to Ukraine.

Following his trip to Chukotka, Putin is expected to visit several regions in the Russian Far East to boost his re-election campaign amid the war with Ukraine, which has seen more than 300,000 Russians killed or maimed.

He is due to stand in March, seeking another six years in the Kremlin.

The only Kremlin leader ever to travel to Chukotka previously was Dmitry Medvedev in 2008.

Putin's trip sees him escape a wave of ugly protests in western Russia over hundreds of thousands of people scraping by in freezing conditions due to breakdowns in communal heating supplies.

In Elektrostal, Moscow region, desperate residents say they have had no communal heating - which Russians routinely expect the state to supply usually through piped hot water - for the entire winter so far.

'We have been without heating since [9 October],' one resident said in a video circulating on Telegram.

'It is impossible to be in our homes… We are freezing! We are freezing! We are freezing!' they said. 

CDC updates Covid isolation guidelines for people who test positive

People who test positive for Covid no longer need to isolate for five days , the Centers for Disease Control and Prevention said Friday.

The CDC’s new guidance now matches public health advice for flu and other respiratory illnesses: Stay home when you’re sick, but return to school or work once you’re feeling better and you’ve been without a fever for 24 hours.

The shift reflects sustained decreases in the most severe outcomes of Covid since the beginning of the pandemic, as well as a recognition that many people aren’t testing themselves for Covid anyway.

“Folks often don’t know what virus they have when they first get sick, so this will help them know what to do, regardless,” CDC director Dr. Mandy Cohen said during a media briefing Friday.

Over the past couple of years, weekly hospital admissions for Covid have fallen by more than 75%, and deaths have decreased by more than 90%, Cohen said.

“To put that differently, in 2021, Covid was the third leading cause of death in the United States. Last year, it was the 10th,” Dr. Brendan Jackson, head of respiratory virus response within the CDC’s National Center for Immunization and Respiratory Diseases, said during the briefing.

Many doctors have been urging the CDC to lift isolation guidance for months, saying it did little to stop the spread of Covid.

The experiences of California and Oregon , which previously lifted their Covid isolation guidelines, proved that to be true.

“Recent data indicate that California and Oregon, where isolation guidance looks more like CDC’s updated recommendations, are not experiencing higher Covid-19 emergency department visits or hospitalizations,” Jackson said.

Changing the Covid isolation to mirror what’s recommended for flu and other respiratory illnesses makes sense to Dr. David Margolius, the public health director for the city of Cleveland.

“We’ve gotten to the point where we are suffering from flu at a higher rate than Covid,” he said. “What this guidance will do is help to reinforce that— regardless of what contagious respiratory viral infection you have — stay home when you’re sick, come back when you’re better.”

Dr. Kristin Englund, an infectious diseases expert at the Cleveland Clinic, said the new guidance would be beneficial in curbing the spread of all respiratory viruses.

“I think this is going to help us in the coming years to make sure that our numbers of influenza and RSV cases can also be cut down, not just Covid,” she said.

Still, the decision was likely to draw criticism from some clinicians who point to the fact that the U.S. logged 17,310 new Covid hospitalizations in the past week alone.

“It’s something that is likely to draw a wide array of opinions and perhaps even conflicting opinions,” said Dr. Faisal Khan, Seattle’s director of public health. “But [the CDC’s] rationale is sound in that the pandemic is now in a very different phase from where it was in 2021 or 2022 or 2023.”

Though the isolation guidelines have been wiped away, the CDC still encourages people to play it safe for five days after they are feeling better. That includes masking around vulnerable people and opening windows to improve the flow of fresh air indoors.

The majority of viral spread happens when people are the sickest. “As the days go on, less virus spreads,” Cohen said.

People at higher risk for severe Covid complications, such as the elderly, people with weak immune systems and pregnant women, may need to take additional precautions.

Dr. Katie Passaretti, chief epidemiologist at Atrium Health in Charlotte, said it was a “move in the positive direction.”

“We are continuing to edge into what the world looks like after Covid, with Covid being one of many respiratory viruses that are certain that circulate,” she said.

The new guidance is for the general public only, and does not include isolation guidelines in hospital settings, which is generally 10 days.

On Wednesday, the agency said that adults 65 and older should get a booster shot of the Covid vaccine this spring. It’s anticipated that the nation will experience an uptick in the illness later this summer.

Winter and summer waves of Covid have emerged over the past four years, with cases peaking in January and August, respectively, according to the  CDC .

Another, reformulated, shot is expected to be available and recommended this fall.

CDC’s main tips for reducing Covid spread:

• Get the Covid vaccine whenever it is available. Cohen said that 95% of people who were hospitalized with Covid this past winter had not received the latest vaccine.
• Cover coughs and sneezes, and wash hands frequently.
• Increase ventilation by opening windows, using air purifiers and gathering outside when possible.
