A Day Trip to the Ramat Gan Safari
If you’re looking for a day trip where you’ll have just as much fun as your kids will, make your way to the Ramat Gan Safari.
Only 15 minutes from the center of Tel Aviv , you can have as close to an authentic African safari experience as it gets. It’s no wonder why the zoo is nicknamed “Africa in the heart of Israel”
Just like in their natural habitats, the animals roam freely in large herds. As a visitor, you will be able to observe the natural behavior of the animals such as their feeding habits, social behavior, activities and resting patterns. The Zoological Center Tel Aviv - Ramat Gan The Ramat Gan Safari is located on 250 acres of nature in the city of Ramat Gan , ironically in the middle of a densely populated urban area in Israel.
It is home to the largest animal collection in the entire Middle East and boasts unique large herds of mixed species of African animals. This includes 1,600 animals of different species, comprised of:
● 68 species of mammals ● 130 species of fowl ● 25 species of reptile
Most notable of the safari’s breeding herds are the African and Asian elephants, the gorilla, chimpanzee and orangutan families, the hippopotamus herd, and of course, the pride of lions.
If you’re lucky, you may even be able to witness mating rituals and power struggles, and see how females raise their offspring. The safari is known for its success rate in breeding its animals, particularly the endangered species.
The Ramat Gan Safari Attractions: The Ramat Gan Safari is divided into three sections:
The Safari Park At the Safari Park, you can enjoy a 30-45 minute drive along a winding road that passes through the territory of a variety of wild animals such as rhinos, gnus, zebras, oryx, pink flamingos and more. Some of the animals may even come straight up to your car to check you out. In this case, remember to follow the important safety rule of never exiting your vehicle.
The Lion Safari The Lion Safari provides you with a glimpse of the more dangerous species of animals—lions and tigers, oh my! The car ride takes approximately 20-30 minutes where you can enjoy observing these majestic animals in their natural habitat. But remember, they’re not kidding when they tell you to keep your windows closed.
The Zoo The Zoo portion of the safari features elephants, gorillas, polar bears, and more animals, divided by continent with detailed explanations along the way. The entire walk can take between 1-3 hours, with plenty to see.
If you’re visiting in the summer you may want to consider renting a small buggy to get around quicker. There are also many places to stop and rest along the way, including benches, picnic areas and food stands.
What’s more, the entire park is very kid friendly with a variety of fun activities such as musical plays, animal face painting, and games. You can also find a petting zoo and farm pony rides open all year round.
The Night Safari: In addition to the fun activities during the day, the Ramat Gan Safari offers guided nighttime tours of the park as well!
The Safari Layla or Night Safari begins when the stars go up and is approximately two hours long. The fun night time tour is geared towards families and visitors of all ages (5 and up)
As you ride the safari train at night, you will get a rare opportunity to discover what the animals do at night. What do the deer do at night? What animals only leave the lake when it's dark? And, why do zebras wear pajamas?

Avraham Krinitzi, Ramat Gan’s mayor, wanted to establish a park around the city. And in 1951, he established Ramat Gan National Park. Two years later, a small petting corner was created in the park.
Today the Safari extends over an area of around 1000 dunams. It has the most extensive collection of animals in the Middle East and is unique worldwide because of the vast herds that graze in the African park.
The word “Safari” originates from the Swahili language and means “a journey.” The idea is to invite visitors to join the journey following animals in African Savannah.
The Safari is located at Hatzvi Avenue 1, Ramat Gan.

A short explanation of what you can find inside the Ramat Gan Geological Center. The center has two areas:
- In the open area , also called the African park , you can find different animals walking freely. You drive around on paved roads. Also, part of the African area has a closed space. That is the lion’s habitat.
- Inside the open area, there is also a zoo .
There is also an animal hospital, but you can visit it only on a dedicated tour. These tours are free but do not take place every day. Check the events schedule on the official site .
On a typical visit to the Safari, you drive through the open space. Then leave the car near the zoo and walk inside. And after a visit to the zoo, you drive back through the open area towards the exit. You can visit the lions before or after your visit to the zoo.
If you do not have a car , you can purchase an open train ticket to take you through the open area to the zoo and back. But, since it is an open train, it will not enter the lion’s territory.
Opening Hours
- Cash registers at the entrance are closed two hours before the closure time.
- In case of bad weather, the opening hours may change.
- The lion area is open from 10:00.

Train ride (for those who do not have a car) through the open area costs an extra 7 NIS.
- Children under the age of two do not need tickets.
- Opening hours, entrance fees, and coupons were updated in December 2022, and in any case, before visiting, recheck the official site .

Usually, there are few coupons for Safari (though the number of offerings started to grow in recent years). But on multiple occasions, I was able to find discounted tickets. And the coupons reach up to 50%.

If you purchase discounted tickets, you must buy them beforehand (usually online) and then book your visit at the official site.
In recent years, Safari started to offer various tours (in addition to regular visits). The number of tours continually grows, and currently, there are:
- Safari morning tour – starts before the Safari opens to the public (usually at 7:30). During this three-hour tour, you get to learn and feed the animals. I have joined several of those tours and will show one later. The total ticket price is 77 NIS. Age restriction: above three years old.
- Memories from Africa – offers a fascinating journey following disappearing animals and forgotten tribes. And you will reveal stories about the connection between them. This 2.25-hour tour costs 72 NIS. Age restriction: above three years old.
- Night Safari – when the stars go out, you will embark on a guided night tour through the open African terrain and enjoy a rare opportunity to discover what the animals are doing during the night. The 2.25-hour tour costs 86 NIS and come with an age restriction of above five years old.
- Midnight Safari is similar to Night Safari, but it takes place later (around 22:00). It is a 2.25-hour tour, and the entrance fee is 96 NIS. Age restriction: above 18 years old.
Tickets for tours can be purchased on the official site . And I would suggest buying them in advance since they might run out.
During a regular visit, you can watch an animal feeding and get a short explanation regarding those animals. Here is the timetable:

And now, let’s start by joining Safari Morning Tour.
Safari Morning Tour
One of the benefits of this tour is that you start early (around 7:30). The tour starts before the Safari is open to the public while it is still not too hot. And the animals are more active. But before buying tickets, if you do not know Hebrew, check out what language the guided tour will be in (our tours were in Hebrew).
Also, note that if you plan to come without children, then you will probably be the only one 🙂
In the Open Area
Last week we attended such a tour. We started by riding the train into the open area. That is the time when they are feeding the animals.
And, of course, the rhino, the symbol of Ramat Gan Safari. But we will get to them later.
After riding the train in the open area and receiving explanations from our guide, the train entered the zoo. We received explanations about different animals and had a chance to feed them. For example, we threw fruits and vegetables to bear territory (before their door to the open area was opened).
For me, the highlight of this tour was feeding giraffes with cabbage.
There are two ways to feed a giraffe. The “natural” one: giving food as if it is taken from a tree. Like in the following photo:
And the more courageous method is placing the food on your palm. But in that case, your hand will be wet with the giraffe’s saliva. The instructor told us that some believe this (being licked by a giraffe) is a good sign and brings luck for the next seven years.

Since small elephants can not cover themselves with dust, their mother does it for them.
Though I did not see the crocodile move, we were told that this was the real thing 🙂
The duration of the Safari morning tour was about three hours. After the tour, we were allowed to enter the Safari in our car (included in the morning tour price). So we made another round since we wanted to visit the lions.
Note: the train is open (it has no windows). Thus it does not enter the lions ` area.
After the Tour
We entered the open space once again in our car. And these are the photos I took on our way to the lions.

When we reached the lions, it was already around 11:00 and boiling. So all lions lay in the shade:
Overall, we liked the Safari morning tour a lot. There are many advantages. The main ones are: you get a guide, you come early while it is not too hot, and you get to feed the animals. As long as kids are happy, their parents will be delighted. The only downside is the ticket price. But, since the difference in ticket price between the morning tour and the regular visit is not significant, I am leaning towards the morning tour. If you are planning to visit Safari and buy full-price tickets, consider the Safari morning tour. In my case, I purchased the tickets through a coupon, and the ticket price was 35 NIS per person (this is even cheaper than the regular entrance). So, it was worth it.
Safari Morning Tour – Three Years Later
We participated in the first Safari Morning Tour in the summer of 2014. In 2017, I had another chance to buy discounted tickets, so I did. In this part, I want to tell you about updates/changes in the tour from our February 2017 visit.
Overall the trip was quite similar. We started our train ride in the open area and headed to the zoo. But, since we had a different guide and saw slightly different animals, we were told different things. For example, do you know how baby zebra differentiates his mother from other zebras?
The form of stripes is unique, like a fingerprint. And each baby zebra knows the back stripes of his mother.
While we were next to the pond, this rhino ran straight toward our train. In such cases, a jeep is protecting the train, and the Safari keeper entered with his vehicle between the train and the rhino.
As it turns out, rhinos do not have good vision. They mostly use the smell sense. And he probably did not see us.

Since we mentioned rhinos, do you know what rhino horn is made of? I thought the horn was some bone, but actually, it is made of keratin, the same type of protein that makes up hair and fingernails.
The Parrot Story
One of the highlights (same as in the previous tour) was giraffe feeding. And after giraffe feeding, our program was different. For example, we were introduced to this parrot. As it turns out, parents bought this bird for a young girl. As that girl aged and reached 18, she went into the army. The parrot became very sad, and the girl’s parents finally brought it to Safari. Until this day, the parrot holds grudges, and if a woman tries to pet him, he bites (the biting is aimed only at women and not men). Parrots may live 40-50 years. So before taking a pet, think about the pet’s whole life and not only the near future.
Our tour lasted a little more than three hours. We were brought by train back to the entrance and then entered Safari’s back gate with our car (this entrance is included in the tour price).
Safari Morning Tour – 2018 update
The morning tour was pretty much the same. But each time, some of the activities are different. And this time, we got to see the pelican and penguin feeding.
Also, Safari workers started to make feeding in the afternoon. Thus if you are near the birds’ area, you can see the pelican and penguin feeding at 13:45 as part of a regular visit.
After the morning tour, we returned mainly to the lions. But they were lying far away and without a clear line of sight.
Rhinos Reproduction Program
Our guide told us that many people ask her how she works in a zoo as an animal lover. But Safari is not a zoo. It is a zoological center. They have many different programs for helping animals.
As it turns out, Safari is one of the only zoological centers in the world where rhinos reproduce. In September 2015, Safari’s 27 baby rhino was born. That increases the rhino population in zoos and zoological centers significantly. And who knows, they might release them one day into the wild.
So I guess this is why they selected the rhino as their logo and symbol.
The bottom line has not changed. If you love animals (and since you read this post, you probably do), then go for it (especially if you can get cheap tickets). We loved it!
Regular Visit at Ramat Gan Safari
I have visited Ramat Gan’s Safari many times before, but when I saw a deal offering a ticket for less than 30 NIS (while the usual ticket price was 67 NIS), I could not resist.
Here are several photos from the open area.
An interesting fact about Wildebeests is that they often graze in mixed herds with zebra. It gives heightened awareness of potential predators. Gnus are also alert to the warning signals emitted by other animals, such as baboons.
While driving through the African area, Safari workers ask people to close their windows. They are especially strict about it in the lion’s territory. Also, please do not feed the animals !
And as I always suggest, start early. Early in the morning, it is cooler, and the animals are more active during this time. We entered the Safari at around 9:30, and it was already quite warm. Thus I changed the usual visiting order. First, we visited the Safari animals, then we drove to the lions and only then went to the safari zoo.
But though I was hoping to see some actions, the lions were already in standard position, lying:
We exited the lion’s area and drove to the zoo.
In the open area, we saw a group of Arabian oryx. Looking at the Arabian oryx from the side, you see only one horn. I read theories that suggested that Arabian oryx were the base for unicorns (since when looking from a distance, you can see only one horn).
The Arabian oryx was extinct in the wild by the early 1970s, but was saved in zoos and private preserves, and was reintroduced into the wild starting in 1980. In 1986, the Arabian oryx was classified as endangered on the IUCN Red List, and in 2011, it was the first animal to revert to vulnerable status after previously being listed as extinct in the wild.

The first animal at the zoo that we saw was the kangaroo. And I thought it was hotter in Australia 😉

We arrived just for the feeding time of the bats. Interestingly, I noticed that bats do everything upside down. They rest upside down and eat upside down. But there is one thing they do not do upside down. Do you know what it is? Take a look at the following photo:
One of the differences that occurred in Safari was the reorganization of the monkey area. They are changing older fences and water barriers to walls with windows. The Baboon area is one example of this change:
Somali wild ass is a subspecies of the African wild ass, and this species is believed to be the ancestor of the domestic donkey.
When the Giraffes finished eating the leaves they were given, one went to the zookeeper and asked for more. Here they are together:
Another change inside the safari zoo is construction of a new playground for children. The downside is that after several minutes there, your shoes will be full of sand.
We finished our visit to the zoo and drove through the safari area (towards the exit).
When to Visit Ramat Gan Safari?
It is always lovely to visit Ramat Gan Safari. Especially on colder days. But on one occasion, it was not one of such days. And it seems that the turtle does not mind the heat.

But it is hot for the elephant. Thus, this zoo worker decided to cool them down (it is hard to see, but the baby elephant is hiding behind his mother):

And most of the other animals were hiding in the shade, and there was no clear line of sight in many cases. Thus, the bottom line is, though you can visit Safari all year round, I would advise considering the weather and going during the colder days and less hot hours of the day .
Life is tough, especially on a hot day 😉
So the bottom line, come either early or late (not during midday), and prefer cooler days.

Ramat Gan Safari is an excellent attraction for the whole family. And it is probably a must attraction for animal lovers. Moreover, if you want to upgrade your visit, then check out available tours. I would recommend Safari Morning Tour. I attended this tour three times and enjoyed each one. Moreover, my family, including my wife, kids, and parents, also liked it.
The only downside is the price. Coupons to Safari are rare, but you can find them occasionally (see the coupons section above).

Ramat Gan Safari - All You Need to Know BEFORE You Go (2024)
Safari Ramat Gan
Offering a fascinating glimpse of fantastic beasts, this is close as you'll get to the Serengeti plains in Israel. It's both a drive-through safari and a large zoo – the safari section includes rhinos, hippos, zebras and flamingos; the zoo has elephants, giraffes, monkeys, kangaroos and more, plus a petting farm, workshops for children, cafes and picnic areas. On the drive out you can pass the lion enclosure. It runs morning feed tours and two-hour night tours throughout the year.
By far the best way to get to the safari is by car, but it can be reached by buses 67 or 55 from Tel Aviv. Bus 67 stops on Raziel/Aluf David Sts, just eight minutes' walk from the entrance. At the entrance to the safari, you'll need to take the park's minibus to get to the zoo section.

About Ramat Gan Safari Park

- Most of the visitors prefer driving in using their own private car, for such visitors, they have to ensure that the windows of their car remain closed most of the time as some animals like the ostriches may pop their head in search of food.

About the Safari
The Safari is located on 100 hectares of wild nature in an urban center in central Israel: Africa in the heart of Gush Dan.
The word “Safari” comes from Swahili, meaning journey. We invite you, our visitors, to go on a fascinating journey following the animals of the African savanna here, in Ramat Gan. Just like in their native habitats in Africa, the animals here move freely in large herds. And just like travelers in Africa, you could also be the guests of nature and watch the typical behavior of animals while eating and socializing, being active or resting. You would be able to distinguish between males and females, watch supremacy displays, courtship demonstrations, mating and maybe even births, and you would definitely be thrilled by seeing parents taking care of their babies.
The Ramat Gan Safari has the largest collection of animals in the Middle East, and it is unique thanks to the large herds that roam the African grounds. The Safari contains about 1,600 animals from all over the world. Among them 68 mammal species, 130 bird species and 25 reptile species. The Safari is home to African and Asian elephants, giraffes, orangutans, Sumatran tigers, gorillas and more.

Entrance to the zoo is according to the cashier working hours. No entry after working hours for members or for people that booked in advance.
Starting Sunday November 28th 2021, the entrance to the petting zoo will be closed to visitors.
These days the Safari is in a renewal boom which includes the renovation of the livestock exhibit and we are eagerly looking forward to its future opening . We apologize for the inconvenience that this may cause.

The Ramat Gan Safari is an African animal Safari found next to Tel Aviv in its neighbouring city. Lions, Rhinosaurises, Zebras and Elephants can all be seen here. Visitors are invited to drive in their cars to see a variety of animals. After they finish, the car can be parked and many more animals can be seen on foot. The Safari houses 83 species of mammals, 92 species of birds and 23 species of reptiles. Inside the safari is also a petting zoo.

The Ramat Gan Safari Park has the largest number of exhibited animals in the Middle East and consists of two sections. There is a 200-acre African Safari park that was opened in 1974. You view the safari park from your car and drive slowly on the route indicated. You can identify the animals from the brochure provided, which also provides a short description of each of them. There is also a secure lion section. You are asked to keep your windows closed when driving through. The total drive should take about 35 to 45 minutes.
Then go to the parking lot to visit the zoo. All the animals are in a natural environment and not in cages. Many are viewed through plexiglass and this works out fine for photos. There are 4 sections. The map in the brochure tells you by number the animals you are seeing. At the entrance to the zoo are listed their feeding times. Most of the explanations about the exhibits are also in English.

Admission: Hours are Sunday to Thursday 9.00 a.m. to 6.00 p.m. (the lion section closes at 5.00 p.m.). The ticket office closes at 4.00 p.m. On Friday and holiday eves the ticket office is open from 9.00 a.m. to 2.00 p.m. and the park closes at 4.00 p.m. The lion section is open from 10.00 a.m. to 3.00 p.m. On Saturday and holidays the park opens at 8.30 a.m. and closes at 6.00 p.m. and the ticket office closes at 4.00 p.m. The lion section is open from 9.30 a.m. to 5.00 p.m. Admission is 99 NIS for everyone 2 years and older. For seniors and students the price is 77 NIS. You can save 10 NIS by booking on line. For those without a car, there is a shuttle through the park for 7 NIS. There are WCs by the entrance to the park and in the zoo section. There is a restaurant in the zoo section with air conditioning in the summer and shaded tables outside. There is a shaded picnic area with picnic benches on fake grass. There is a gift shop and kiosk for drinks and snacks. There is a small children’s play area. Their phone number is 03 632 0222. Public transport: Enter “Safari” into Moovit and click on “Sderot Hatsvi, Ramat Gan.” The closest bus stop is either a 13-minute walk and a distance of 1.1 Km or a 20-minute walk and a distance of 1.7 Km.

Hippopotami and zebras. The hippopotami are often submerged in the lake during the hot part of the day.
An addax. Their original habitat was the deserts of North Africa where they are now almost extinct.
A herd of Arabian oryx. They were hunted almost to extinction in the 1970s.
Ramat Gan National Park
It is also worth driving or walking to the Ramat Gan National Park adjacent to the zoo. A gate at the parking lot in the zoo leads directly to the National Park.
The park has very expansive green lawns and a large lake, around which is a paved path. There are boats for hire. As you walk around the lake you will notice deck chair in areas with sand and deck chair in areas without sand overlooking the lake. There are plenty of ducks in the lake. There is a covered exercise area with machines and a short track. There is a kids play area. Part of the lawns can be used for barbecues and there are picnic benches here.
For driving to the park enter “Ramat Gan National Park.” This will lead you to a paid parking lot. You can also park on the street leading to the park on blue and white marked parking areas. There are WCs by the entrance to the park with piped music and WCs within the park.
Ramat Gan: Diamonds, Football, Malls & Safari
Ramat Gan (Literally: Garden height ) is a city in the center of Israel, located east of Tel Aviv and part of the Tel Aviv metropolitan area. It is home to one of the world’s major diamond exchanges, and many high-tech industries.
Ramat Gan is an eclectic town famous for many things that have no relation to each other – including diamonds, football, malls, Iraqi restaurants, the Bar-Ilan university (which is known as the “religious” university), and even a Safari. Living up to its name, small green public gardens are plentiful and all streets are tree lined.
History and Economics
Ramat Gan was founded in 1921 as a moshav, a communal farming community. According to the 1931 census, The young community had 975 inhabitants, in 253 houses – but over time the population grew (as it did the entire country of Israel and Tel Aviv) and today its population exceeds 160,000.
The city is located in the Gush Dan metropolitan area, east of Tel Aviv. It is bounded in the north by the Yarkon River and in the east by Bnei Brak. Givatayim lies to the southwest.
North Ramat Gan features a cluster of sparkling sky scrapers, which, at night, impresses you with a magnificent Manhattan-style skyline. Ramat Gan’s economy is dominated by the Diamond Exchange District in the northwest of the city, home to a large concentration of skyscrapers, including Moshe Aviv Tower (City Gate), Israel’s second tallest at over 240 metres (790 ft), the Israel Diamond Exchange (a world leader in diamonds) and many high-tech businesses.
The rest of the city, just like its name, is residential with green gardens here and there. It is the greenest city in Israel, with its 25% green area.
Some foriegn embassies are located here (including embassies of Ghana, Kenya, Ivory Coast, Jordan, Eritrea, Norway, Belgium, the Netherlands, and the European Economic Community). Ramat Gan Sheraton City Towers, which is a preferred hang out for Likud Party primaries conferences.
In the North East, you will find the first successful shopping mall in Israel (Ayalon Mall) surrounded by huge outlets. Next to it, the biggest soccer stadium in the country and the Ramat Gan National Park. On the other side of town, Bar Ilan University, a world famous institution combining modern studies with Rabbinical curriculum.
The city is also an important center for industry and manufacturing hub, mainly for food and beverages, textile and furniture. The Elite Tower, set to be the tallest one in the city, is being built on the site of the historic Elite Candy factory. As a tribute to the history of the site, the lower floors of the tower will house a chocolate museum.
If you are feeling sick, two major hospitals are available – the Sheba Medical Center located in southeastern Ramat-Gan and Tel HaShomer, which is Israel’s largest hospital.
Last but not least, Ramat-Gan is known for its Iraqi heritage. Many Jewish immigrants from Iraq settled here in the 1950’s and as a result, many good Iraqi restaurants can be found here.
What To Do in Ramat Gan
For higher culture in Ramat Gan, one might go to the Ramat Gan Theater, the Diamond Theater and the Russell Cultural Center. The Beit Zvi School of Performing Arts is based in Ramat Gan as well.
For more popular mass market fun, Ramat Gan operates two cinemas complexes: the Lev-Elram Cinema and the “Yes Planet” megaplex.

Ramat Gan Stadium
Sport fans will like it here, as Ramat Gan is a football city. Ramat-Gan Stadium was Israel’s national football stadium until 2014, and holds some major events like the Maccabiah Games (The Jewish olympics) that are held in it every four years and many music concerts.
Hakoah Amidar and Hapoel Ramat-Gan are the two main football clubs of the city, both play at the Winter Stadium.
Ramat Gan Safari
Ramat-Gan also has a safari park. The 250-acre site consists of both a drive-through African safari area and a modern outdoor zoo. It houses 1,600 animals, and has the largest animal collection in the Middle East.
The African animal park opened to the general public in 1974. In 1981, the zoo was established in the middle of the park to replace the Tel Aviv Zoo, which had closed down. It has more than 700,000 visitors annually from all social and ethnic groups of Israel as well as tourists from abroad.
The safari houses 83 species of mammals, 92 species of birds and 23 species of reptiles. Among other outstanding groups of animals, it has white rhinos, hippos, lions, African and Asian elephants, gorillas, orangutans, and a Komodo dragon. All animals are seen in open air enclosures amid subtropical gardens.
Along with the safari, the place is home for the Israeli Wildlife Hospital (IWH) that was founded in 2005 as a cooperative enterprise between Ramat Gan Safari and the government Israel Nature and Parks Authority (INPA). The IWH treats over 2,000 wild animals annually. As Israel is located on a central migration route (over half a billion birds migrate over Israel every migratory season) the IWH receives more than 100 avian species every year.
How Far Is It From Tel Aviv?
As we said, the two cities are bordering each other, so it depends what neighborhoods in both cities you are referring to. from the city center of Ramat Gan to the city center of Tel Aviv is about a 15 minutes drive.

Yes, as the water in Ramat-Gan (and all over Israel) is very safe to drink.
There are regulations for guaranteeing the quality of drinking water, and the Ministry of Health makes sure to update these based on scientific development, accumulated knowledge, analytical developments and international legislation.
Water sources are tested in laboratory tests (for the detection of over 130 different components) before they are approved for supply as drinking water, and all the water supplied to the public is decontaminated with Chlorine, which prevents the development of harmful bacteria.

- Have a clinical diagnosis of SLE at least 24 weeks prior to screening.
- Have documentation of having met at least 4 of 11 Revised Criteria for Classification of Systemic Lupus Erythematosus according to the 1997 Update of the 1982 American College of Rheumatology (ACR) criteria for classification of SLE prior to randomization.
- Have a positive antinuclear antibody (ANA) (titer ≥1:80) and/or a positive anti-double-stranded deoxyribonucleic acid (dsDNA), and/or a positive anti-Smith (anti-Sm) as assessed by a central laboratory during screening.
- Have a total Systemic Lupus Erythematosus Disease Activity Index 2000 (SLEDAI-2K) score ≥6 during screening.
- Have a clinical SLEDAI-2K score ≥4 at randomization.
- Have active arthritis and/or active rash.
Exclusion Criteria:
- Have severe active lupus nephritis.
- Have active central nervous system (CNS) lupus.
- Have a history or presence of cardiovascular, respiratory, hepatic, gastrointestinal, endocrine, hematological, neurological, or neuropsychiatric disorders or any other serious and/or unstable illness that, in the opinion of the investigator, could constitute an unacceptable risk when taking investigational product or interfere with the interpretation of data.

Background: In the COMBI-v trial, patients with previously untreated BRAF Val600Glu or Val600Lys mutant unresectable or metastatic melanoma who were treated with the combination of dabrafenib and trametinib had significantly longer overall and progression-free survival than those treated with vemurafenib alone. Here, we present the effects of treatments on health-related quality of life (HRQoL), an exploratory endpoint in the COMBI-v study.
Methods: COMBI-v was an open-label, randomised phase 3 study in which 704 patients with metastatic melanoma with a BRAF Val600 mutation were randomly assigned (1:1) by an interactive voice response system to receive either a combination of dabrafenib (150 mg twice-daily) and trametinib (2 mg once-daily) or vemurafenib monotherapy (960 mg twice-daily) orally as first-line therapy. The primary endpoint was overall survival. In this pre-specified exploratory analysis, we prospectively assessed HRQoL in the intention-to-treat population with the European Organisation for Research and Treatment of Cancer quality of life (EORTC QLQ-C30), EuroQoL-5D (EQ-5D), and Melanoma Subscale of the Functional Assessment of Cancer Therapy-Melanoma (FACT-M), completed at baseline, during study treatment, at disease progression, and after progression. We used a mixed-model, repeated measures ANCOVA to assess differences in mean scores between groups with baseline score as covariate; all p-values are descriptive. The COMBI-v trial is registered with ClinicalTrials.gov, number NCT01597908 , and is ongoing for the primary endpoint, but is not recruiting patients.
Findings: From June 4, 2012, to Oct 7, 2013, 1645 patients at 193 centres worldwide were screened for eligibility, and 704 patients were randomly assigned to dabrafenib plus trametinib (n=352) or vemurafenib (n=352). Questionnaire completion rates for both groups were high (>95% at baseline, >80% at follow-up assessments, and >70% at disease progression) with similar HRQoL and symptom scores reported at baseline in both treatment groups for all questionnaires. Differences in mean scores between treatment groups were significant and clinically meaningful in favour of the combination compared with vemurafenib monotherapy for most domains across all three questionnaires during study treatment and at disease progression, including EORTC QLQ-C30 global health (7·92, 7·62, 6·86, 7·47, 5·16, 7·56, and 7·57 at weeks 8, 16, 24, 32, 40, 48, and disease progression, respectively; p<0·001 for all assessments except p=0·005 at week 40), EORTC QLQ-C30 pain (-13·20, -8·05, -8·82, -12·69, -12·46, -11·41, and -10·57 at weeks 8, 16, 24, 32, 40, 48, and disease progression, respectively; all p<0·001), EQ-5D thermometer scores (7·96, 8·05, 6·83, 11·53, 7·41, 9·08, and 10·51 at weeks 8, 16, 24, 32, 40, 48, and disease progression, respectively; p<0·001 for all assessments except p=0·006 at week 32), and FACT-M Melanoma Subscale score (3·62, 2·93, 2·45, 3·39, 2·85, 3·00, and 3·68 at weeks 8, 16, 24, 32, 40, 48, and disease progression, respectively; all p<0·001).
Interpretation: From the patient's perspective, which integrates not only survival advantage but also disease-associated and adverse-event-associated symptoms, treatment with the combination of a BRAF inhibitor plus a MEK inhibitor (dabrafenib plus trametinib) adds a clear benefit over monotherapy with the BRAF inhibitor vemurafenib and supports the combination therapy as standard of care in this population.
