Telehealth is the use of telecommunications and information technology to provide access to health assessment, diagnosis, intervention, consultation, supervision and information across distance. At one time, telehealth in Medicaid had been referred to as telemedicine.

Telehealth seeks to improve a patient's health by permitting two-way, real-time interactive communication between the patient and the physician or practitioner at the distant site. This  communication often requires  the use of interactive telecommunications equipment that can include both audio and video components, but can also be conducted via audio-only, as states deem appropriate.

Telehealth includes such technologies as telephones, electronic mail systems, and remote patient monitoring devices, which are used to collect and transmit patient data for monitoring and interpretation.

State Telehealth Flexibilities

For most Medicaid benefits, federal Medicaid law and regulations do not specifically address telehealth delivery methods or the criteria for implementation of telehealth.  As a result, states have broad flexibility in designing the parameters of telehealth delivery methods to furnish services. However, underlying services must continue to meet the requirements of the overarching provisions in Title XIX of the Social Security Act (the Act), regulations, the federal policy framework of the covered Medicaid benefit, and the parameters of the state’s CMS-approved Medicaid state plan or a subsequent state plan amendment (SPA). Where Medicaid law or regulations set forth telehealth delivery requirements for specific benefits, those requirements must be observed. For example, the Community First Choice (CFC) Option at 1915(k) has general requirements for using telehealth for performing the assessment of need (§441.535), but does not have requirements for other CFC activities that could be performed using telehealth.

CMS Approach to Reviewing Telehealth SPAs

Unless required by regulation or policy, states are not required to submit a (separate) SPA for coverage or reimbursement of Medicaid coverable services delivered through telehealth if they decide to reimburse for services delivered through telehealth in the same way/amount that they pay for face-to-face services.

States must submit a (separate) reimbursement (attachment 4.19B) SPA if they want to provide reimbursement for services or components of services delivered through telehealth differently than is currently being reimbursed for face-to-face services.

States may submit a coverage SPA to better describe the services they choose to cover through telehealth, such as which providers/practitioners are identified by the state to use telehealth to deliver services; where it is provided; how it is provided, etc. In this case, and in order to avoid unnecessary SPA submissions, it is recommended that a brief description of the framework of telehealth may be placed in an introductory section of the state plan, e.g., Section 3 – Services: General Provisions 3.1 Amount, Duration and Scope of Services, and then a reference made to coverage through telehealth in the applicable benefit sections of the state plan. For example, in the physician section it might say that dermatology services can be delivered via telehealth provided all state requirements related to telehealth as described in the state plan are otherwise met.

What is a virtual visit? It may be a term you’re hearing more often. That’s because the use of virtual visits, also known as telehealth, has expanded recently, offering people more ways to connect with a health care provider from home or work. During a virtual visit, you use digital technologies, like your smartphone, tablet or computer, to talk with a provider. You can get treatment options and even prescriptions for medications, if needed.

UnitedHealthcare offers members options for virtual visits with local providers or by using UnitedHealthcare’s preferred national providers. For benefits coverage information, please  sign in to your health plan account . Let’s go over some of the basics about virtual visits and how they work.

Are virtual visits covered?

Virtual visits may be covered by your health insurance in a similar way to an office visit with your doctor. Depending on your benefit coverage, you may be able schedule a virtual visit with a local provider or have a virtual visit for primary care, therapy, specialty care or 24/7 with a national provider for urgent care or when your provider is not available. Sign in to your health plan account  or call the number on your member ID card to check your benefits for virtual visit coverage. Below are some options that may be available, based on your health plan.

Virtual care may include in-network virtual visits for medical, mental health, physical therapy, occupational therapy, speech therapy, chiropractic, home health, vision, hearing and dental services.

Virtual visits for mental health

If you need care for mental health issues, local health care providers may be able to provide virtual visits. Coverage may depend on your health plan benefits, so  sign in to your health plan account  or call the number on your member ID card first to learn what benefits may be available to you.

Emotional support by phone or mobile app

Some emotional support programs may be included with certain health plans. Sign in to your health plan account to learn if the following benefits may be available with your health plan.

  • With Talkspace online therapy, you can regularly communicate with a therapist, safely and securely from your phone or desktop. No office visit required. Talkspace is convenient, safe and secure. Telehealth Services Let You Visit with a Provider by Phone or Video.

You will get the care you need without driving a long distance. When you see a provider through Telehealth, it will be a lot like a regular office visit. Services may include everyday care for non-emergency conditions, behavioral health, dermatology, and more!

Services not covered include:

  • Standard phone calls, faxes or email – combined or separate.
  • Getting your medication based on an online survey.

Any in-person care that needs prior approval will need the same prior approval through Telehealth.

How do I get Telehealth Services?

We’ve made it easy. There are now two ways to get Telehealth Services: 1.  Through Teladoc  We put access to quality care at your fingertips through Teladoc! Talk to a doctor anytime, anywhere by phone or video.

Teladoc is available for:

  • Everyday Care : Talk to a licensed doctor 24/7 who can treat non-emergency conditions such as the flu, sinus infections, sore throats, and more.
  • Mental Health Care : Talk to a therapist 7 days a week (from 7 a.m. to 9 p.m. in your local time zone). Teledoc’s experts provide support for anxiety, stress, depression, medication management, and more.
  • Dermatology (included with some plans) : upload images of skin issues online and get a custom treatment plan within two days for common skin conditions such as eczema, acne, rashes, and more.

2. Through Your Provider Ask your provider if he/she offers Telehealth Services. Many providers offer this service directly. If so, your provider will give you all the information you need to get started.

With our telemedicine program, you can:

From your smartphone, you can speak with a board-certified doctor for health concerns like sore throats, earaches, the flu, allergies, sinus infections, rashes, and more. Support for anxiety, depression, post-traumatic stress, and family issues is also available.

With your phone or online visit, you can save time and money by avoiding ER visits for non-emergencies. (Remember, in an emergency, always call 911 or go to the nearest ER).

  • Talk to a health care provider, anytime
  • Receive a prescription if you need one, and pick up your medication from the pharmacy of your choice ( costs for prescription drugs depend on your pharmacy benefit, and are not covered as part of the telemedicine program)

Medicare Part B (Medical Insurance)  covers E-visits with your doctors and certain other health care providers.

Your costs in Original Medicare

After you meet the Part B deductible , you pay 20% of the  Medicare-approved amount  for your doctor's or other provider's services.

E-visits allow you to talk to your doctor or other health care provider using an online patient portal to answer quick questions or decide if you need to schedule a visit.

You can get an E-visit with:

  • Nurse practitioners
  • Clinical nurse specialists
  • Physician assistants
  • Licensed clinical social workers (in specific circumstances)
  • Clinical psychologists (in specific circumstances)

E-visits can be used for the treatment for the Coronavirus (COVID-19) from anywhere, including places of residence (like homes, nursing homes, and assisted living facilities).

  • To get an e-visit, you must request one with your doctor or other provider.

Can I access mental health services?

Doctor On Demand offers both medical and mental health appointments; however, we are still in the process of enrolling our mental health providers in Medicare Part B. In the meantime, we encourage you to book a visit with our medical providers, who are cross-trained in mental health services. Your medical provider may recommend that you continue treatment with a mental health provider.

If I’m already registered with Doctor On Demand, how do I add my Medicare insurance to my account?

Emergency medical care is provided free to all foreign nationals in case of life-threatening conditions that require immediate medical treatment. You will be given first aid and emergency surgery when necessary in all public health care facilities. Any further treatment will be free only to people with a Compulsory Medical Insurance, or you will need to pay for medical services. Public health care is provided in federal and local care facilities. These include 1. Urban polyclinics with specialists in different areas that offer general medical care. 2. Ambulatory and hospitals that provide a full range of services, including emergency care. 3. Emergency stations opened 24 hours a day, can be visited in a case of a non-life-threatening injury. It is often hard to find English-speaking staff in state facilities, except the largest city hospitals, so you will need a Russian-speaking interpreter to accompany your visit to a free doctor or hospital. If medical assistance is required, the insurance company should be contacted before visiting a medical facility for treatment, except emergency cases. Make sure that you have enough money to pay any necessary fees that may be charged.

Travelers need to arrange private travel insurance before the journey. You would need the insurance when applying for the Russian visa. If you arrange the insurance outside Russia, it is important to make sure the insurer is licensed in Russia. Only licensed companies may be accepted under Russian law. Holders of a temporary residence permit or permanent residence permit (valid for three and five years respectively) should apply for «Compulsory Medical Policy». It covers state healthcare only. An employer usually deals with this. The issued health card is shown whenever medical attention is required. Compulsory Medical Policyholders can get basic health care, such as emergencies, consultations with doctors, necessary scans and tests free. For more complex healthcare every person (both Russian and foreign nationals) must pay extra, or take out additional medical insurance. Clearly, you will have to be prepared to wait in a queue to see a specialist in a public health care facility (Compulsory Medical Policyholders can set an appointment using EMIAS site or ATM). In case you are a UK citizen, free, limited medical treatment in state hospitals will be provided as a part of a reciprocal agreement between Russia and UK.

Pharmacies can be found in many places around the city, many of them work 24 hours a day. Pharmaceutical kiosks operate in almost every big supermarket. However, only few have English-speaking staff, so it is advised that you know the generic (chemical) name of the medicines you think you are going to need. It is standard practice for expats to visit private clinics and hospitals for check-ups, routine health care, and dental care, and only use public services in case of an emergency. Insurance companies can usually provide details of clinics and hospitals in the area speak English (or the language required) and would be the best to use. Investigate whether there are any emergency services or numbers, or any requirements to register with them. Providing copies of medical records is also advised.

Moscow hosts some Western medical clinics that can look after all of your family’s health needs. While most Russian state hospitals are not up to Western standards, Russian doctors are very good.

In a case of a medical emergency, dial 112 and ask for the ambulance service (skoraya pomoshch). Staff on these lines most certainly will speak English, still it is always better to ask a Russian speaker to explain the problem and the exact location.

Ambulances come with a doctor and, depending on the case, immediate first aid treatment may be provided. If necessary, the patient is taken to the nearest emergency room or hospital, or to a private hospital if the holder’s insurance policy requires it.
