Chances are if you’ve seen a healthcare specialist within the last two years either for dental work, mental health care, or a regular doctor’s visit, you’ve utilized telehealth. It’s become a commonplace method for healthcare professionals to schedule, pre-screen, visit with, and monitor patients, but it’s uses go far beyond just these elements. 

Telehealth vs telemedicine?

Telehealth encapsulates all types of phone and technological communication for healthcare including the health apps on your smartphone and digital watch. It includes telemedicine, which more specifically involves a regular doctor to patient interaction via a virtual method like a video or phone call. When thinking about the difference between telehealth versus telemedicine, you might think of telemedicine like a regular doctor’s visit but online and telehealth as everything involved in getting you to that visit including the visit itself.

Through telehealth, doctor’s can collaborate with other specialists, receive further education and training in their fields, and collect data to learn more about the human body and our needs. You can utilize telehealth through many applications on your smartphones and digital watches. You may self-monitor your heart rate and activity levels, learn more about your sleep patterns, and more easily access your own health records.

How does a virtual doctor’s visit work?

Virtual doctor’s visits work in a few different ways. One of the most common works like a regular doctor’s visit, but much faster. A basic way you might virtually see your doctor is to schedule an appointment via a physician’s website, an app on your phone, or a simple phone call to your doctor’s office. Then, a real doctor, physician, or nurse contacts you via a phone call or a face-to-face virtual video call, and you talk to them about your health care needs or symptoms just as you would in an in-person environment.

Although the use of value-based payment models is growing, 1 the most common payment method in physicians' employment contracts is still a base salary plus a production bonus based largely on relative value units (RVUs). 2 RVUs reflect a physician's volume of work and level of effort in treating patients. The more RVUs a physician generates, the more income the practice (and the physician) should receive.

Knowing the RVUs assigned to different types of services allows physicians to understand how their productivity is measured. And knowing their RVUs per visit and per year allows physicians to understand how they compare to their peers. They can then assess whether they need to see more patients, work more efficiently, or change key processes such as billing and coding to better reflect the amount of work they are doing.

The four questions outlined in this article can help physicians get started in understanding and improving their RVUs.

Knowing the work RVUs assigned to different types of services can help you understand how your productivity is being measured.

Work RVUs vary depending on the work and effort required; for example, a 99212 has 0.70 work RVUs while a 99214 has 1.92 work RVUs.

To improve your total work RVUs per year, you may need to work more efficiently so you can see more patients, improve your coding practices to capture the full value of your work, or offer more high-value services, such as procedures or transitional care management.

1. WHAT ARE RVUS?

RVUs are part of the Resource-Based Relative Value Scale (RBRVS), which Medicare and other payers have used for decades to determine payment for physician services. RVUs are just one of three components that factor into the payment calculation. 3 The other two components are geographic practice cost indices (GPCIs), which adjust RVUs to reflect regional differences in the cost of physician services, and a conversion factor (CF), which is the dollar amount by which total, geographically adjusted RVUs are multiplied to arrive at the payment amount for a given service. (See “ How payment for a service is calculated. ”)

Total RVUs are the sum of three types:

Work RVUs , which reflect the physician's work, including mental effort, decision making, technical skills, physical effort, stress related to patient risk, and amount of time spent,

Practice expense RVUs , which reflect the clinical and nonclinical labor costs and practice expenses,

Malpractice RVUs , which reflect the liability insurance costs.

The remainder of this article will focus on work RVUs because they are a key measure of physician productivity. Work RVUs for common services in primary care are listed at the end of this article. For example, a 99214 established patient evaluation and management (E/M) office visit has 1.92 work RVUs, a “Welcome to Medicare” visit has 2.6 work RVUs, and a 99223 initial hospital visit (admission) has 3.5 work RVUs.

HOW PAYMENT FOR A SERVICE IS CALCULATED

Relative value units (RVUs) are just one of three components that factor into physician payment under the Resource-Based Relative Value Scale:

Total RVUs , which is the sum of work RVUs, practice expense (PE) RVUs, and malpractice RVUs,

Geographic practice cost indices (GPCIs), which adjust RVUs to reflect regional differences in the cost of physician services,

A conversion factor (CF), which is the annually updated dollar amount by which total, geographically adjusted RVUs are multiplied to arrive at the payment amount for a given service.

Payments are calculated as follows:

[(Work RVUs x Work GPCI) + (PE RVUs x PE GPCI) + (Malpractice RVUs x Malpractice GPCI)] x CF = Payment amount

2. WHAT ARE YOUR WORK RVUS?

If you are an employed physician, you likely receive productivity reports from your employer, but you may not be in the habit of looking at them closely or you may not know what to do with the information. A helpful report should provide you with the following individualized data:

Annualized work RVUs (projected total if you continue to generate the same amount of work for the rest of the fiscal year),

Number of visits,

Annualized number of visits,

Types of visits you are seeing (such as preventive care visits, E/M visits, hospital visits, and Medicare wellness visits),

A breakdown of your E/M visits (i.e., what percentage are coded as level 1, 2, 3, 4, or 5).

If you are expected to meet a goal for work RVUs, you should receive your data on a regular and predictable basis — e.g., monthly, bimonthly, or quarterly — allowing you to analyze the data and make adjustments in scheduling, coding, or practice efficiency to meet the goal. Waiting until the end of the fiscal year to distribute a finalized report does not allow physicians enough time to make effective changes.

Data should be accurate, trustworthy, and presented to physicians in a user-friendly way. Practices should have a contact person who understands the information well and can answer physicians' questions or investigate discrepancies should they arise.

If your employer does not provide you with a productivity report, or does not provide it in a timely manner, ask for it. Most EHRs and practice management systems capture the required data and can generate customizable reports.

You can estimate your work RVUs on your own using the process described in the physician example .

HOW TO CALCULATE WORK RVUS PER VISIT AND PER YEAR: AN EXAMPLE

Dr. Smith is a family physician in a physician-owned outpatient practice. He sees an average of 10 patients per session, or 20 visits per day, and his average work RVUs are 1.69 per visit. Out of 260 total workdays per year, Dr. Smith has 40 days for vacation, continuing medical education, holidays, etc., leaving 220 workdays per year. That means he averages 4,400 visits and 7,436 work RVUs per year, which puts him well above the 50th percentile in national benchmarks but below the 90th percentile.

Here's the method used to estimate his work RVUs:

First, list the types of visits in an average session, assign work RVUs to each visit, and add the values to get the total work RVUs per session.

Next, complete the following calculations:

Work RVUs per session ÷ Visits per session = Work RVUs per visit

16.88 ÷ 10 = 1.69

Workdays per year x Visits per day = Visits per year

220 x 20 = 4,400

Visits per year x Work RVUs per visit = Work RVUs per year

4,400 x 1.69 = 7,436

3. HOW DO YOUR WORK RVUS COMPARE TO BENCHMARKS

Practices should establish work RVU benchmarks at the beginning of the fiscal year and include them in productivity reports so physicians can see whether they are meeting work RVU goals. Practices may use internal data to set benchmarks, comparing you to your peers, or they may rely on national benchmarks from external sources. Benchmarks should be specialty-specific, but they may differ for inpatient versus outpatient practice, new versus established physicians, and physician-owned practices versus practices owned by hospital or health systems.

For example, according to data from the Medical Group Management Association, for family medicine physicians in physician-owned outpatient practices, the 50th percentile is 5,945 annual work RVUs and the 90th percentile is 9,063. 4 Meanwhile, for those in hospital-owned or health-system-owned outpatient practice, the 50th percentile is 4,715 annual work RVUs and the 90th percentile is 7,451. One possible reason for the difference is that physician-owned practices may be more likely to reward productivity, while practices owned by a hospital or health system may be more likely to use straight salary compensation.

4. HOW CAN YOU INCREASE YOUR WORK RVUS?

If your work RVUs are below goal, there are several ways you may be able to increase them. Start by examining the following factors.

Visit volume . To assess whether low patient volume is contributing to low work RVUs, identify how many patients you see per day. A 2018 survey by the Physicians Foundation found an overall average of 20.2 patients per day across all specialties. 5 According to data from the American Academy of Family Physicians, family physicians average 84 patient encounters per week — 63 in the office, 12 e-visits, seven in the hospital, and two in nursing homes or house calls. 6 What ultimately matters is the benchmark your group is using. If your visit volume is too low, that does not necessarily mean that you need to work longer each day, but you may need to work more efficiently in order to see more patients. For example, you may need to delegate more tasks to your team, or you may need to address a high no-show rate.

Visit levels . Identify what percentage of your E/M visits are level 1 through 5, and compare this distribution to that of your peers. If your practice cannot provide this data, you can use national benchmarks derived from Centers for Medicare & Medicaid Services data (see this FPM worksheet , which has been updated with 2021 data). If you find that you are billing a higher percentage of lower-level visits than your peers, you will need to either see more patients to achieve the same overall work RVUs or investigate other issues. For example, it could be that your billing, coding, and documentation practices are inefficient, leading to under-coding or missed coding.

High-value visits . Identify which high-value visits you frequently provide, and which ones you should be providing. (See the list of work RVUs for different visit types on page 7 .) For example, new patient visits have higher work RVUs than established patient visits, so if you've closed your panel to new patients, this could be affecting your work RVUs. Many procedures are also valued highly, so you may want to consider expanding your scope of practice by adding skin procedures, joint aspirations and injections, treadmill stress tests, etc. Transitional care management visits and Medicare annual wellness visits are also higher-value services. Consider whether you can provide these to patients who need them, and how your care team can assist with the workload.

Visit lengths . Track your visit lengths for various visit types to see whether you need to adjust your scheduling practices. Adjusting visit duration in your scheduling template can allow you to see more patients per day or per session to meet your work RVU goals. For example, a physician who has a goal of 30 work RVUs per day and averages 1.3 work RVUs per visit (the equivalent of an established-patient, level 3 office visit) would need to see 23 patients per day. In an eight-hour workday, that would allow for an average of 20-minute visits. Note that time-based billing became easier following the 2021 E/M coding changes and now includes the physician's total time spent on the day of the visit (not only face-to-face time but also pre-visit planning, coordination of care after the visit, and other physician activities).

WORK RVUS FOR COMMON VISIT TYPES

FOCUSING ON WHAT YOU CAN CONTROL

Some aspects of physician productivity are beyond your control. For example, you can't really control whether your schedule is full every day or what level of care your patients need. However, it's still beneficial to understand the factors that are affecting your work RVUs so you can assess your performance and make the case for what you need, such as more clinical support staff or a second exam room. Improving productivity isn't just about working harder, but working smarter and identifying changes in practice operations and efficiency that will actually make a difference.

How do virtual visits work?

Consumers expect convenience with just about everything they do these days – from streaming TV shows on-the-go to ordering groceries without ever walking into a store. But many people don’t realize that seeing a doctor may be just as simple. 

Telehealth, or virtual visits is helping make access to health care more convenient. People can see and talk to a doctor through their smartphone, tablet or computer, meaning they don’t have to go to a doctor’s office or sit in a waiting room. Instead, they can visit with a doctor from the comfort of their own home.

Beyond the convenience, virtual visits may also save consumers time and money. According to a  study , a virtual visit may save consumers more than an hour of their time. These visits may cost much less than a trip to urgent care or an emergency room. 

Some examples of health issues that can be addressed with a virtual visit include:

• Bladder or urinary tract infections
• Migraines or headaches
• Seasonal flu
• Sinus problems
• Sore throat
• Stomachache

There are times when an in-person visit may be more appropriate. Virtual visits is not intended to address chronic or complicated conditions, a health issue that may need a hands-on exam, lab test or X-ray or an emergency medical need.

Telehealth services may be covered by your insurance, so check with your health plan to see if it’s covered for you.

Today, the Social Security Administration published a final rule, “Omitting Food from In-Kind Support and Maintenance (ISM) Calculations.” The final rule announces the first of several updates to the agency’s Supplemental Security Income (SSI) regulations that will help people receiving and applying for SSI.

“A vital part of our mission is helping people access crucial benefits, including SSI,” said Martin O’Malley, Commissioner of Social Security. “Simplifying our policies is a common-sense solution that reduces the burden on the public and agency staff and helps promote equity by removing barriers to accessing payments.”

SSI provides monthly payments to adults and children with a disability or blindness, and to adults aged 65 and older, who have limited income and resources. SSI benefits help pay for basic needs like rent, food, clothing, and medicine. People applying for and receiving SSI must meet eligibility requirements, including income and resource limits. Under our old rules, ISM includes food, shelter, or both a person receives – the agency counts ISM as unearned income, which may affect a person’s eligibility or reduce their payment amount.

Under the final rule, beginning September 30, 2024, the agency will no longer include food in ISM calculations. The new policy removes a critical barrier for SSI eligibility due to an applicant’s or recipient’s receipt of informal food assistance from friends, family, and community networks of support. The new policy further helps in several important ways: the change is easier to understand and use by applicants, recipients, and agency employees; applicants and recipients have less information to report about food assistance received from family and friends, removing a significant source of burden; reducing month-to-month variability in payment amounts will improve payment accuracy; and the agency will see administrative savings because less time will be spent administering food ISM.

The agency continuously examines programmatic policy and makes regulatory and sub-regulatory changes as appropriate. Look for more SSI announcements in the coming weeks.

I applied for my Social Security retirement benefits on March 9, 2024. I also applied for my Medicare retirement benefits about a month prior to March 9. There is some confusion as to additional documentation when I went to the local Social Security office. I retired from the government and Social Security office wants me to provide a request for employment information and this is part of the Medicare and Medicaid services. However, I want wanting to find out what my retirement benefit, monthly retirement benefit is going to be coming in from Social Security. It seems to be there’s some confusion between the two programs around the same time of filing. Please help.

What Does It Mean When a ‘State of Emergency’ Is Declared?

States of emergency are often in effect for catastrophic weather, but they are also used in many other situations.

Only hours after a colossal cargo ship struck a major bridge in Baltimore and caused it to collapse on Tuesday, Maryland’s governor and Baltimore’s mayor each declared a state of emergency.

The order unlocks additional powers that can be used to deal with natural disasters or man-made crises, often by creating or suspending laws and by redirecting public funds to the task at hand.

In Maryland, Gov. Wes Moore’s declaration of a state of emergency was intended to provide additional public safety resources and to help meet the needs of residents or visitors affected by the collapse.

Here’s what you need to know:

Who can declare a state of emergency?

Every state in the country allows its governor to declare a state of emergency during extraordinary conditions.

Governors are “at the zenith of their power when they make an emergency order like this,” said Meryl Chertoff, executive director of the State and Local Government Policy and Law Project at Georgetown Law. “They can suspend laws, order evacuations, commandeer private property and impose curfews, among other powers.”

In about a half-dozen states, the legislature can also call an emergency.

Most states also allow leaders of cities, towns, villages and counties to declare a state of emergency in their local district.

While those are the best known and most often used instances, the president also has such powers.

What happens after an emergency is declared?

It depends on the situation. For example, by declaring a state of emergency, a governor can create new laws or suspend ones already in place to temporarily help manage dire conditions.

The crisis proclamation can also give a governor flexibility to redistribute state funds, without having to go through the legislature. It is also a requirement in order to receive federal emergency aid.

“They’re kind of expected, if something happens quickly, to declare an emergency because then you have a different set of powers, and you may not even need them,” Raymond Scheppach, who worked with more than 300 governors during his three-decade tenure as executive director of the National Governors Association. “But a lot of times they feel that they may need them if the disaster gets worse.”

Any changes by executive order must expire at the end of the emergency, usually from 30 to 90 days. They cannot be written permanently into law when the declaration is in place.

What are some of the limits?

The temporary laws set by state, county and city leaders cannot eclipse the Constitution or overstep federal authority.

Some states also have a system of checks and balances that allows the legislature to reject a governor’s emergency decree. Usually, all it takes is a majority vote. In other occasions, a legislature has the final say on whether an emergency declaration can be extended.

“It’s really important for the governor to have those powers in the acute phase,” Ms. Chertoff said. “But then at a certain point, it would be good as a policy matter, to get additional input from the state legislature.”

In fewer instances, states can require a governor to call a special session of the legislature. If the state cannot manage the response on its own, it can ask the president to declare a major disaster.

Under the National Emergencies Act, the president has almost 150 distinct powers , only some of which have meaningful restrictions and congressional oversight, according to an analysis by the Brennan Center for Justice at New York University School of Law.

What are some of the concerns?

The coronavirus pandemic brought criticisms of emergency powers to the fore. All 50 states, the District of Columbia and United States territories declared public health emergencies in response to the pandemic in March 2020.

Those declarations allowed state officials to lift limits on hospital capacity, expand access to telehealth services and even allow highway weight limits to be exceeded, in case the National Guard needed to quickly move in. But they also included face mask mandates, business and school closures, eviction moratoriums and the extension of tax filing deadlines.

The governors’ expansive rule-making attracted scrutiny, and some pandemic-related executive orders were successfully challenged in court.

Critics have often been concerned by the emergency powers extending for years. Hundreds of bills have been introduced in nearly every state and territory since 2020, with at least 30 states paring back some powers before this spring.

Pennsylvania was among the first to pass restrictions on a governor’s power, in May 2021. The Republican-controlled Indiana State Legislature is advancing a similar bill this spring. And voters in Arizona will decide on Election Day whether to give the Legislature similar powers through a constitutional amendment .

The attitude of most legislators toward a governor’s emergency declaration, Mr. Scheppach said, once was, “I don’t want to get in the middle of it, basically.” But now nearly every state has considered amending the process for a state of emergency, shortening the maximum length of each declaration and cutting the number of times that a governor can renew an emergency declaration.

What about the politics of using them?

A leader can come under criticism for using these powers, but there are clear benefits to looking like a strong executive who acts decisively.

Mr. Scheppach said that his organization gave governors lessons in emergency preparedness. “You’ll often see governors on TV, right? Because we train them to go to the site because they need to appear in control,” he said. “And if they don’t do it, there’s often a lot of criticism.”

In extreme cases, he said, a poor performance in a crisis leads to a failure to be re-elected. “You’ve got to do it right,” Mr. Scheppach said.

All this has been taken care of without a single visit to my doctor’s office. 

Not only are virtual visits safe, easy, and effective, they give your doctor important ongoing information about your health and wellness. This means that when you see your doctor in person, the visit will go smoothly because he or she has up-to-date information about your health. Because of the virtual visit, your doctor is already aware of any non-emergency health problems you have had, so you do not need to spend time reviewing that information again unless it is necessary for your care.

Corey Landstrom is a self-described night owl originally from northern Minnesota who likes to run and can make a mean chili. A lover of all things music-related, he has seen Elvis twice (his mother was a huge fan), met Bob Dylan backstage at a show, and looks forward to seeing all kinds of live music at the Englert. A first-generation college graduate, Landstrom has built his career in higher education, working at six small colleges across the country before joining the College of Nursing as the director of the Felton Student Success Center. 

When asked what brought him to the University of Iowa in October, Landstrom says that he was looking for  a new opportunity that fit his experience but was also challenging and close to him home in Grinnell. Additionally, he had some experience with nursing at Luther College, where he was the vice president and dean for student life. 

“I have always appreciated the nurses in my family, the nurses who served in student health services, a department I supervised at Luther College, and finally as a patient,” Landstrom says. “I deeply respect the professionalism that nurses bring to their work.”

Landstrom was also attracted to the unique culture of the college. “It was pretty evident when I came for my interview that this is a community that really cares about their students and who also care about each other.” 

Spending time learning and understanding College of Nursing policies and processes has taken up much of his first 100 days on the job. He has also spent time meeting faculty and staff in the college and getting to know his team in order to prepare for the departmental reorganization they will be implementing in the coming months.

An aspect on Landstrom’s mind is how the Felton Student Success Center can maximize its impact for prospective and current students. “A key priority for me is to identify the populations and communities that we should be connecting with regarding our opportunities and meeting prospective students where they are in their decision-making process,” he says. 

Outside of university-related work, he loves listening to music, so you may find him looking at vinyl records at local record stores – his favorite is Wax Xtatic in Marshalltown – or attending a live show. 

As time and body allow, Landstrom also enjoys running and he and his partner like to visit their daughter in Decorah where she works as an AmeriCorps VISTA with a local non-profit.
