SG Arrival Card (SGAC) with Electronic Health Declaration
You can submit the SGAC via:
- SGAC e-Service (submit online here) ; or
- MyICA Mobile application - Download the official app for free from the App Store (iOS) or Google Play (Android) and enjoy seamless submissions with the MyICA Mobile app. Travellers are encouraged to download the MyICA Mobile application for your own convenience. The profile-saving function in the app can auto-populate relevant profile information for your subsequent trips to Singapore. Find out how the MyICA Mobile app can bring convenience to you on-the-go.
SGAC submission is free of charge. ICA does not support or endorse services offered by commercial entities assisting to submit the SG Arrival Card at a fee. They are not affiliated with ICA.
All travellers are required to submit the SG Arrival Card before arriving in Singapore, except:
- Those transiting/transferring through Singapore without seeking immigration clearance; and
- Residents (Singapore citizens, Permanent Residents, Long-Term Pass holders*) travelling through Woodlands and Tuas Checkpoints.
*Long-Term Pass holders include holders of Student’s Pass, Dependant’s Pass, Long-Term Visit Pass and Work Pass
You are required to submit the SGAC within three (3) days (including the day of arrival) before arrival in Singapore .* This is to avoid unnecessary delays during immigration clearance upon your arrival.
*For example, if you are arriving in Singapore on 30 Jun 2023, you can submit your declaration from 28 Jun 2023 onwards.
- The SG Arrival Card is not a visa. For more information on general entry requirements, please visit the Entering Singapore page. It contains information such as Visa requirements .
- At the SG Arrival Card e-Service , you may begin the submission by selecting one of the two options:
- You will receive an acknowledgement email sent to the email address declared in your SGAC upon successful submission.

CBP Traveler Entry Forms
Whether you are a visitor to the United States or a U.S. citizen arriving in the United States, you must complete one or more entry forms.
You must complete the CBP Declaration Form 6059B . CBP Declaration Form 6059B provides us with basic information about who you are and what you are bringing into the United States, such as agricultural and wildlife products and whether or not you have visited a farm prior to traveling to the United States. If you are traveling with other immediate family members, then you can complete one form for your entire family.
Some travelers (such as refugees, asylees or individuals with parole documents) will need to complete CBP Form I-94 Arrival/Departure Record .
If you have questions about your form, please don't hesitate to ask a CBP officer for help.

Introducing the Nigeria International Travel Portal (NITP), for Intending Travellers to Nigeria
Are you intending to travel to Nigeria?
If yes, before your trip, visit https://nitp.ncdc.gov.ng to carry out the following:
– Fill out the Pre-boarding Health Declaration and Screening form.
– Upload on the portal, COVID-19 Negative PCR test result of a test taken not more than 96 hours before the date of travel
– Select an appointment date and laboratory for a repeat COVID-19 test to be done 7 days after arrival in Nigeria. (Online payment will be required, on the site).
– Receive a payment confirmation and a bar-coded print out which you will present at airport check-in in your country of departure and to the Health officials at your port of entry in Nigeria.

Your patients are going places: help them have a healthy trip abroad..
Many travelers don’t seek health advice when they’re planning international trips, even though their travel may put them at risk for preventable illnesses. Include travel questions in the routine history you gather from patients to assess possible risks and identify opportunities for patient vaccination, medication, and education.
- Ask about underlying conditions, allergies, and medications.
- Ask about special conditions, such as pregnancy, immunocompromising conditions, history of heart disease or stroke, and recent surgery.
- Ask about immunization history.
- Ask about prior international travel.
- Get trip details, such as destination, trip length, and reason for travel.
- Ask about travel style, such as traveling alone or with a group, staying at a hotel or with a host family, and modes of transportation.
- Ask about special activities, such as providing medical care or doing disaster relief work, doing adventure travel/extreme sports traveling (skydiving), being on a cruise ship, traveling to high altitudes, or being exposed to open water (diving, rafting). Also ask if they expect to be in contact with animals.

How to comply and fly: Pre-travel requirements for Delta’s Covid-tested flights
Delta is offering COVID-tested flights between the U.S. and Italy through 30 June 2021.
With the latest travel requirements in mind, this program allows business travellers who have tested negative for coronavirus in the USA and on arrival in Italy to enter Italy without quarantining.
Delta’s COVID-tested flights are operated between Atlanta and New York-JFK and Rome-Fiumicino airports, and between New York-JFK and Milan Malpensa Airport.
Our trusted partner, Alitalia, also offers COVID-tested flights between New York-JFK and Rome-Fiumicino International Airport.
To fly on Delta’s COVID-tested flights, customers will have to provide a negative Covid-19 test and need to comply with the following requirements:
TRAVELLING FROM ITALY TO U.S.
Before departure:
All customers ages 2 and above traveling to or connecting in the United States are required to
- present proof of a negative COVID-19 (NAAT, PCR, LAMP, or antigen) test taken within 3 days of their scheduled departure,
- sign an attestation confirming a negative test result,
- adhere to Delta’s Commitment to Safer Travel and
- must provide the following information to Delta for contact tracing purposes : full name, a primary and secondary phone number, an email address and a destination address. 1
On arrival:
Customers should be prepared to show their negative COVID-19 test result before entering the U.S. Additional testing will not be required, though customers may be subject to a health screening. Quarantine requirements upon reaching the destination in the U.S. vary by state and local governments.
TRAVELLING FROM THE U.S. TO ITALY
Customers traveling to or connecting through Italy on a Delta COVID-tested flight are required to
- present proof of a negative COVID-19 (PCR) test taken within 3 days of their scheduled departure and
- take a rapid test at no additional cost prior to their departure and
- fill in a digital Passenger Locator Form and additional self-declaration forms prior to boarding. 2
Proof of COVID-19 vaccination does not exempt customers from testing requirements or from Delta’s face mask policy .
Customers will be directed to a rapid testing area for
- another temperature screening and
- a COVID-19 rapid antigen test.
Upon receiving a negative test result, customers will be free to enter Italy without the need to quarantine.
- Customers must arrive at the airport 4.5 hours before departure from ATL, JFK and MXP and 4 hours from FCO.
- Travellers need to arrive at gate no less than one hour before boarding.
- Boarding will begin 50 minutes prior to the scheduled departure time.

The COVID-19 pandemic has drastically changed the way we fly. As governments implement stricter travel policies, airport processes have become a lot more complicated. Here in the Philippines, one of the most recent measures is the pre-registration procedure before the flight.
Each passenger must accomplish the Passenger Profile and Health Declaration (PPHD) form a few days prior to the date of departure:
- Domestic – as early as 3 DAYS before the flight
- International – as early as 5 DAYS before the flight
But where do you get this form? In this post, we’ll share with you how to get one if you’re flying with Philippine Airlines.
WHAT'S COVERED IN THIS GUIDE?
1. Make sure you have the flight details and an email address.
Before registering, you must have the following:
- A valid email address. You will be sent the form and other details via email so make sure you have one that is working. If you don’t have one yet, you can easily create an account.
- Flight details. You need to provide the passenger’s full name, flight number, and destination.
2. Pre-register.
To get the PPHD form, you need to pre-register first. Domestic and international passengers have separate pre-registration forms:
✅ DOMESTIC PRE-REGISTRATION
✅ INTERNATIONAL PRE-REGISTRATION
Note that this is not yet the actual PPHD form. Once you’ve submitted this pre-registration form, you will be sent via email the following:
- a PPHD form
- a unique reference code
- further instructions
Take note of the reference code and follow the next instructions.
3. Fill out the PPHD form.
As mentioned, one of the items that you will receive via email after pre-registration is the actual Passenger Profile and Health Declaration Form (PPHD form) . Accomplish that.
Note that each passenger should fill out the form, even if they’re traveling as part of a group.
4. Secure the other requirements.
On top of the PPHD Form, there may be additional documents that are required at the airport. For more information about the departure process, we have a step-by-step guides here:
