This past year saw a surge of travel throughout destinations in Africa , with air travel across the continent approaching prepandemic levels . Safari camps from South Africa to Kenya were fully booked, and travelers lingered in popular destinations like Cape Town and Zanzibar well past peak season. What does this mean for the year ahead?

A wave of hotly anticipated luxury resorts —many of which were in the works long before grinding to a halt in the pandemic—are finally ready to throw open their doors, new air routes are making parts of the continent more accessible than ever, and popular events and festivals are enticing visitors to explore regions they may not have contemplated before. Yes, there are plenty of thrilling safari camps in the pipeline that we’re excited about, from iconic corners of Botswana to a less-visited park in Mozambique , but there’s also a compelling food movement in Accra , a milestone anniversary in Rwanda , a monumental new cultural landmark in Morocco ’s capital city of Rabat, and so much more to look forward to. Could this be the year Johannesburg pries visitors away from Cape Town with a cool new city hotel and a dynamic dining scene, or when sunseekers take another look at Mauritius instead of the Maldives ? Accra continues to draw travelers with a surge of cool new pop-up restaurants, museums, and the pilgrimage-worthy music festival, AfroFuture (previously Afrochella).

Accra has steadily been cementing its reputation as the continent’s capital of cool, and a reimagining of pan-African heritage has ignited the city’s foodies. Leading the way is the Ghana Food Movement , a network of food change makers intent on surfacing the sexy in sustainable dining. With the May 2024 opening of its Link-up Kitchen in the buzzing Osu neighborhood, this meeting place–cum–test kitchen will house the Movement’s popular Dine and Dance series, where young chefs highlight local ingredients in multicourse dinners before getting down to beats by local DJs. The hub will also host pop-ups featuring up-and-coming chefs and Indigenous menus, such as those offered by Abena Offeh-Gyimah, whose food tours spotlight heirloom ingredients and a “reconnection to ancestral ways of eating.” This year also sees chef Selassie Atadika of Midunu resuming her Nomadic Dinners celebrating African ingredients , as well as quarterly chocolate tastings of her gorgeous bonbons inspired by flavors like beriberi chili and Cape Malay spice. After dinner, you’ll find the balmy West African capital’s electric club scene spoils you for choice. And while the Grammys have just come to the party (the awards will feature a Best African Music Performance category for the first time, in 2024), locals have been lighting it up at festivals like Chale Wote Street Art Festival (August) and AfroFuture (previously Afrochella ; December) for years. Should you find time to sleep, the December 2023 opening of the 145-room Hilton Accra Cantonments marks the brand’s debut in Ghana.

Looking ahead: A state-of-the-art cultural institution is coming just outside the city as well. Enriched by voices from the diaspora, an overdue zeitgeist to reclaim African narratives finds a physical home in the Pan African Heritage Museum , which has begun construction and will open in 2025 or 2026. Located on 10 lush acres in Winneba (40 miles from Accra), the space’s first offerings will include a music amphitheater, a youth innovation center, and a food court. And though the city battles infrastructure challenges and social inequality, it’s impossible to ignore the positive energy infusing South Africa’s economic capital. That’s evident in a resurgent restaurant scene rapidly giving Cape Town a run for its money. Cape stalwart the Pot Luck Club expanded with a new location here in 2023, as did Insta-worthy British café EL&N London , marking its only African outpost. For a more continental culinary journey, book a table at Gigi , the new outing by celebrated local chef Besele Moses Moloi . Make your base in the artsy suburb of Rosebank, where the upscale Park Hyatt Johannesburg will open in late 2024 with decor by Yabu Pushelberg. Until then, Voco Johannesburg Rosebank —the first of the Voco subbrand from IHG in Africa—offers Art Deco glamour and an enviable in-house art collection (ask for a guided tour). It’s just a short taxi ride to artist Roger Ballen’s engaging new Inside Out Centre for the Arts , a striking exhibition space and educational center opened in March 2023.

Rosebank is less than 30 minutes from the international airport on the high-speed Gautrain. This year brings with it multiple new reasons to bypass South Africa ’s iconic Kruger National Park and head instead into the scenic Zululand hills of northern KwaZulu-Natal (KZN), the country’s easternmost province. If you’ve traveled here before, you may have been to &Beyond Phinda Private Game Reserve , known for species such as cheetahs, black rhinos, and pangolins. Phinda Forest Lodge , one of six properties on the reserve, reopens after an extensive refurbishment, with a new family suite and a reimagined pool, in December 2023. It comes on the heels of the Madwaleni River Lodge ’s opening in September 2023, in Babanango Game Reserve —there, from a private deck overlooking the White Umfolozi River, you might spot elephants cooling off in the water. (Their recent reintroduction completed the reserve’s Big Five population.) The reserve has been rewilded over the past five years, a project driven by community trusts who own parts of the reserve. Next up, the long-awaited Homestead will welcome guests to the Nambiti Private Game Reserve in early 2024. The 12-suite lodge is set to have electric vehicles and private butler service. With suites starting at $7,500 a night, though, its rates are double those of comparable properties—time will tell what kind of bang it offers for the buck. Looking to combine these camps with a luxe seaside escape for the ultimate bush-and-beach combo? Because if you’ve always dreamed of discovering Botswana’s Okavango Delta, where seasonal floodwaters transform parched sands into a filigree of shimmering channels, right now is when you’ll be spoiled for choice. A surge in demand, coupled with a proactive approach to developing community-owned concessions, has led to a flush of new lodges across the world’s largest inland delta.

Take Sitatunga Private Island , or Natural Selection’s North Island Okavango , where just three tented suites rest on an island beneath wild ebony trees. Natural Selection will add to their portfolio in May 2024 with a new build: Tawana . Set in the wildlife-rich Moremi Game Reserve, this partnership with the chief of the local Batawana tribe will offer eight thatched suites amid the grasslands lining the perennial Gomoti River. Each stay is at least 1,000 square feet in size and comes with a private plunge pool and terrace.

Water is gold in the Delta, and this past year, African Bush Camps (ABC) opened the riverside Khwai Lediba . But our eye is really on Atzaro Okavango , slated for a March 2024 debut: It’s the third Icon camp in the Delta for ABC, with 10 lagoon-side suites promising impressive levels of Delta luxe and a strong sense of place through design (both markers of the Icon portfolio).

If you’re feeling adventurous, look east to the untapped Mababe marshlands and the brand-new Wilderness Mokete (opening mid-2024) for a fresh look at under-canvas exploration—out with plush campaign-style furnishings and in with geometric lines and contemporary decor. Cool music festivals? Big-name architecture? Luxury hotels? Morocco's capital of Rabat has all that and more, and 2024 brings a wave of newness.

The Moroccan capital of Rabat may be one of the country’s four imperial cities, but it’s long been bypassed by travelers drawn to the timeworn beauty of places like Marrakech , Fez , and Tangier . That may change this year, as the city is in the midst of a cultural and artistic reinvention. One of the most talked about attractions: the forthcoming Mohammed VI Tower, designed to be LEED Gold and HQE-certified and projected to be the tallest and only spire of its kind in Africa. But perhaps even more exciting is the new Grand Theatre of Rabat opening nearby. One of legendary architect Zaha Hadid’s unfinished masterpieces, it will incorporate an 1,800-seat theater, an experimental performance space, and a 7,000-person amphitheater, along with green spaces and a restaurant. It is set to be the largest venue of its kind in both Africa and the Arab world and will host symphony, ballet, opera, and philharmonic performances (all of the above are anticipated before 2023 comes to a close). Another hotly anticipated cultural foray is the 2024 return of the Mawazine festival , coming back this summer after a COVID-19-induced hiatus—there’s no word on a lineup yet, but in the past Africa’s largest festival has drawn A-listers like Rihanna and Mariah Carey.

A flurry of high-profile hotel openings will make for a soft landing in the city while you’re there: The newly opened Fairmont La Marina Rabat Salé and Conrad Rabat Arzana both herald a new era of modern luxury in the historic city; now, the buzz is centered around the hotly anticipated Four Seasons Hotel Rabat at Kasr Al Bahr , opening in late 2023, as well as a the planned Ritz-Carlton Rabat Dar Es Salam . — Harriet Akinyi

Beyond famed gorilla treks, 2024 is the year Rwanda unveils lesser-known adventures and plenty of new reasons to hire a guide, hop in a Land Cruiser, and traverse the four national parks. Nyungwe National Park , freshly anointed with UNESCO World Heritage status, is one of Africa’s oldest rainforests, renowned for its chimpanzee trekking, cycling, and hiking. The new Cyinzobe trail —a thrilling three-day, 15.5-mile hike, reveals breathtaking views of three cascading waterfalls, and the Kamiranzovu river. With lodging limited to six wooden A-frame cabins, you’ll have the rainforest almost to yourself. Spend your days learning about wild orchids, spotting silver monkeys and gray-cheeked mangabey, and marveling at some of the 345 bird species inside the park—30 of which are Albertine Rift endemics. Kingfisher Journeys invites adventurers to explore Rwanda’s uncharted white water rivers for the first time—there are 30 intermediate-grade rivers in the country for pioneering descents—led by expert rafting guides. In November 2023, the Mantis Kivu Queen uBuranga introduces luxury houseboat cruising with a three-day voyage on the emerald waters of Lake Kivu—one of the African Great Lakes. Kayaking, swimming, cycling, hiking, and cultural immersion in local villages on small islands await.

And at Meza Malonga in Kigali, Congolese-born chef Dieuveil Malonga boasts Afro-fusion cuisine inspired by his travels to 48 African countries. The culinary marvel—voted a Champion of Change winner by World’s 50 Best —will open an expansive innovation lab in Musanze in August, where he’ll train young chefs and offer unique accommodations for food enthusiasts. The newly opened Kweza Craft Brewery , Rwanda’s first craft brewery, is fully female-owned and operated using 85% locally sourced ingredients, with an aim to once again empower women in what was historically a female-led craft.

As Rwanda commemorates the 30th anniversary of the devastating Tutsi genocide in 2024—marked by the poignant lighting of the flame of remembrance on April 7 at the Kigali Genocide Memorial —these new openings and experiences showcase just how far the country has come. — Alicia-Rae Light

Sierra Leone is a country on the rise, with new flights, a first ever ecotourism circuit, and national park lodges coming in 2024—visit before everyone else you know catches on.

The small West African nation of Sierra Leone has been working on a new tourism-centered vision. A recent visa on arrival program has already simplified the process of getting there. And, in 2023, a solar-powered airport near the capital, Freetown, opened a new terminal with a fivefold increase in capacity. In the year ahead, new international flight routes are expected to make the country even more accessible, with flights to Nigeria and South Africa first in line.

Perhaps the most exciting development is Sierra Leone’s push to safeguard and showcase its diverse wildlife, with new experiences for travelers. Spearheaded by Freetown-adjacent Tacugama Chimpanzee Sanctuary , the initial phase of the country’s first ecotourism circuit opens in 2024 (keep an eye on the Tacugama website for details). It will offer first-ever accommodations at two important sites: A pair of one-bedroom ecolodges are slated to open at southeastern river island of Jaibui, where the elusive Pygmy Hippo lives, by spring 2024, and tent stays at Loma Mountains National Park will follow. Loma is home to Mount Bintumani, the tallest peak in West Africa west of Mount Cameroon, with an astounding array of rare animal and bird species like the bizarre-looking white-necked rockfowl, “ the avian Holy Grail ” for birdwatchers. In 2024, construction will begin on a world-class research and innovation center at Tacugama, with an exhibition hall dedicated to Sierra Leone’s national animal, the chimpanzee.

A host of new accommodations elsewhere in the country offers access to Sierra Leone’s coastline. Jethel Garden Eco-Resort opens in December 2023 near Freetown at Tasso Island , an important stop on the country’s slave trade history route. Further south, a previously inaccessible chain of remote sandbar islands in the Atlantic, Turtle Islands , is adding new accommodations and better boat transfers with project Tomatribe, which includes glamping-style bell tents opening by the start of 2024.

More is coming out of Sierra Leone in the next few years: The Peninsula City , a public-private partnership in conjunction with W Hotels, is building a resort and marina city just south of Freetown, and Sherbro Island City , a long-term initiative funded by Sierra Leone’s brand ambassador Idris Elba, has plans to transform this tropical island into the “Afro-dynamic eco-city.” — Yulia Denisyuk

A volcanic island in the South Atlantic Ocean, St. Helena is a British overseas territory about the same size as Walt Disney World Orlando , with a population under 4,500.

Until the first commercial flights touched down on the island in 2017, the remote isle could only be reached via a five-day boat journey from South Africa. Weekly flights from Johannesburg eventually made that journey easier, despite a pandemic pause, which ended in 2022. As of November 2024, St. Helena departures will be even more frequent, taking off twice a week from Cape Town throughout the summer and effectively making the island much easier to visit. The reason to go? To get that remote island feel , while still taking advantage of the infrastructure—which rivals the neighboring islands of Ascension and Tristan da Cunha.

There’s a rich history to dig into once you land. Uninhabited when the Portuguese arrived in 1502, St. Helena was later claimed by the Dutch before becoming a British territory—today, residents, known as Saints, have ancestral links to Britain but also to China , Madagascar, India , and West Africa . Tours detail the island’s past, from its enslaved African population to Napoleon Bonaparte’s exile and death. In the historic capital Jamestown, the Mantis St. Helena remains the smartest hotel on the island. Whale sharks visit St. Helena’s clear waters from December to March; tour operators like Sub-Tropic Adventures run trips to swim with the gentle marine giants. Snorkelers might spot turtles and devil rays, and there are several wrecks to explore by scuba. Enjoyed the trip, saw lots of different animals, history and culture. Spent a lot of the time driving but the variety of game drives and optional activities created a good balance. The 2 groups I was a part of were great people and good company. The meals however were poor, felt our guide didn't try his best, majority of meals were cheap and our meal allowances were equivalent to $2-3 when given to eat on our long drives. He definitely could of done better with options especially after several comments and suggestions from our group. I didn't feel the food we got was anywhere the price I paid for the trip. Breakfasts consisted of cornflakes, weetabix, fruit and occasional yoghurt which was hidden and rationed along with museli. There were some days closer to the end where we got eggs, sausage and pancakes. Some individuals didn't even eat much overall because there wasn't much selection. Lunches consisted of processed meat sandwiches, apples and bananas for the most part. Dinners were rice, pasta, some vegetables and bread. The meat options were fine and more catered to I found. Unfortunately I felt that our guide wasn't organized with activities either, left everything to the last minute. He became more involved and active on the 2nd half of our journey with the group, for the first half he was a little reserved. Our driver was amazing and very accommodating and friendly. I would definetely do another G trip but would like to have a more outgoing and accommodating guide.

What an absolute adventure 77 days from Kenya to South Africa! Safaris, lakes, lions, beaches, historical sites, village visits, camping, long drives with windows down, gorillas, family style cooking, washing dishes, early mornings, hippos, boat safaris, peeing on the side of the road, cold showers, card games, photos, are some of the words that instantly come to mind from thinking about the last 3 months. Memories for a lifetime. Originally I was booked from just Nairobi to Zanzibar and I’m so glad I ended up extending all the way to South Africa! Our group was on the larger side as it was during the busy season July-Oct and we had about 20-25 people on the bus during the 3 months. The amount of people can vary as there are smaller sections of the absolute overland trips where people will be with you for different legs of the trip. (Ex: some people started and finished Zanzibar or started Vic Falls to Cape town, etc.) The best thing I can write for a review would be a recommendation for those wanting to go on the trip. To have an enjoyable trip is all about having the right expectations & mindset as these overland trips are definitely not for everyone. After the 3 months I believe I met around 40 different people on the safari who had stayed for a section or the whole safari and I could tell that most absolutely loved the overland trip, however, there were a few people that struggled as they did not have the right expectations. As I already mentioned, flexibility is key for this trip. I believe they make this very clear on their website but somehow some people still show up with a very rigid mindset. Some days go as planned, some days go better, and some go worse. But that is all part of the African experience. People on these trips are from all different backgrounds, with different personalities, work ethics, and ways of doing things. Just roll with it and it will be an amazing time. FOOD. Family style & simple. Please be flexible. You must remember that if there is a group of 20-25 everyone has to be catered to for their dietary needs. Most lunches you are on your own. There will be a supermarket stop and you can get as you wish. But for dinner there will be a cooking team for that specific night so depending on the dietary restrictions there might have to be 3 different meals if you have vegetarians, vegans, and meat eaters. So I would definitely recommend being flexible when it comes to preferences. I wouldn’t recommend this trip for strict dietary restrictions (vegetarian ok but vegan, etc. not so much). Remember you are in Africa and there are weeks where the food options and supermarkets are limited. Staff. We had an amazing guide Peter for the 77 days and 1 driver for the first 3/4 of the trip and then last 1/4 a different driver. Our guide and both drivers were from different parts of Africa and all had 15+ years experience doing these tours. They are all very knowledgable & such helpful resources. They all speak multiple languages and handled the borders crossings and logistics very professionally. At times things were a bit up in the air but like I said it is the nature of Africa as things change very frequently. They are the first people you see up in the morning and last people to bed making sure everything is in order. Our guide did a really good job at balancing all the different preferences of food, itinerary, etc. We had some unique characters in our group which made it pretty difficult for him to handle but he did a great job at balancing it for everyone. A great sense of humor, always had a positive outlook, and always enjoyed telling us the history of local areas. Truck. There are several trucks with some being newer and some being older. Our first truck Kifaru was a bit older, could use some cosmetic repair on the interior but worked like a charm. The seats were comfy and there were plenty of outlets to charge electronics. The fridge wasn’t ideal as it was smaller especially after the group food was in put in but did all was needed. Good variety of 8 seats facing backwards with tables & 20 seats facing forwards. Money. This is by far the most budget friendly overland tour I saw when doing research. Plan to spend more than you think though. You can do this trip with just doing the minimums but when everyone is going on an extra safari or an extra activity, you are going to want to do it as well. Easy to say you won’t do something at home but different story when you are there. Maintain a positive attitude & energy. Although you don’t need to be “physically fit” this trip is demanding especially if you are doing the 77 days. There aren’t any necessary physically fit needs, but there are many early wake up calls, tearing down & setting up your tent everyday, and some nights you will be on cooking / cleaning duty, etc. It all adds up and you’re constantly on the move for months. That is all part of the adventure. You are going to meet tons of different people so you can imagine positive attitude is key. Some may look at how busy it is and see it as a negative but you can also see how some of those things also are a highlight of the trip for others. All about the outlook. If you want a vacation to sit and chill on the beach this isn’t the trip for you. If you are ready to have an adventure of a lifetime and one of the most unique experiences traveling on an overland truck through Africa with people from all around the world, you won’t be disappointed. Best advice have a flexible and positive outlook and you will have a great time! So glad I did it! Hindsight I would 100% recommend Absolute Africa especially knowing what I know now. Rushing down to South Africa or rushing the itinerary like some of the other tour companies isn’t something I wanted to do & I am glad I chose AA. Unforgettable memories! Hope that helps!

From the very start he was welcoming, helpful and made the trip entirely worry-free. He generously shared so much information about the countries we visited and animals we were fortunate enough to see. We shared many stories and laughs. The tour was very well organized. Everything worked up quite well and everyone took good care of us. It was a magnificent time in Tanzania's wild nature. Also the tour company took care of all changes online (one day early arrival and extra shira tour booking) in a flexible and timely manner. Our Safari guide and driver Israel did really a great job. He worked hard to find all the animals for us as good as possible. This wasn't an easy job as we were there during rain season. He kept us safe and well informed all the time. We saw many wild animals such as cheetahs, lions, giraffes, buffalos, elephants, hippos, hyenas and of course tons of Gnus, zebras and all kinds of antilopes, monkeys and birds. We could even see some of the big five like the lion from eye to eye, roaming around less than 1m from our vehicle! Of course there's no guaranty of which wild animals you'll see as we're talking about natural parks, but you can rest assured the guides are doing their best to show you what nature is offering during your visit. All the staff in the hotels, parks and people in general were very kind and very welcoming! The food was always good and safe. FYI, the actual safari is 5 days + 2 days arrival and departure. Nevertheless within these 5 days we saw so many places and had a unique nature experience. "Luxury" as mentioned in the title is a bit too much said, especially when it comes to accommodation. On the other hand all the hotel names/ratings and the exact itinerary were mentioned clearly so no surprises. The luxurious part was actually that we were only on our own since it's low season :) Our guide Israel was flexible and fulfilled every possible wish we had. The Maasai village tour in the way to Serengeti costs an extra 50$ which is a fair price but they will also offer you some overpriced hand-made souvenirs to buy. The tour itself was very interesting and nice including a welcome song and dance basically from the whole village :) The Serengeti Safari lounge was quite spacious and had a big balcony to enjoy the endless view of the Serengeti. That was definitely the best accommodation of the trip. We woke up to a great show of a huge group of Gnus and Zebras directly in front of our balcony. The very ones who were singing for us all night :) Last but not least: if you're wondering about the prices even for a day tour, you should know that the park fees including conservation fees are quite high. So you're basically contributing to the well-being of these parks as well as the many wonderful people protecting them.
A bit late with the review... The trip was November 2019 and it was the best trip ever!!! Simon best guide, stunning places, welcoming locals and amazing experiences! the flight over the Okavango Delta is highly recommended.

Experience Africa showcases a refreshingly wide range of inspiring travel products and enticingly curated experiences, all being sold right now across the incredible continent. This essential B2B event will present the very best Africa product portfolios, assuring connectivity between key European buyers and handpicked quality exhibitors, maximising exposure in a way never seen before in the UK.

The show will be taking place from 01 to 03 July 2024  and will be #ThePlaceToConnect for the African tourism industry.

As usual, attending will give you access to a diary of pre-scheduled appointments with up to 42 meeting slots available over the course of the three days. Each day will be broken up with morning and afternoon networking coffee breaks along with a longer networking lunches. Want to take an affordable vacation in 2024? You’ve come to the right place. Although travel costs are on the rise this year ( up 3.1% ), it’s still possible to save money in 2024—if you know where to go.

A new travel report from the U.K.’s Post Office —a British-based mail and financial services company — has just unveiled some of the most budget-friendly destinations for 2024. And although the annual report is aimed at U.K. travelers, it’s a helpful list for Americans, too.

By analyzing currency strengths, local prices and essential tourist expenses in 40 popular vacation destinations around the globe, the Holiday Money Report 2024 highlights some of the cheapest places to travel. The report ranks the destinations based on a barometer that takes the local price of eight tourist essentials into consideration—including coffee, beer, cola, wine, water, sunscreen, insect repellent and a three-course meal. And while the report doesn’t include factors like hotels and airfare, it helps give a good sense of where your dollars will stretch the furthest.

This is the 18th year that the Post Office has done this report. And the good news: Prices have fallen since last year in 25 spots on the list. (A note that the original report included prices in pounds sterling, which have been converted into dollars here, using the exchange rate on the date of publication of 1.26.)

U.S. Secretary of Commerce Gina Raimondo will travel to Nairobi, Kenya from April 23 to April 25.  Secretary Raimondo will lead a delegation of private sector companies from the President’s Advisory Council on Doing Business in Africa (PAC-DBIA). She will highlight the U.S.-Kenya bilateral commercial and investment relationship and flourishing digital space in Kenya, and throughout Africa, that is helping to shape the world for the better and create exciting opportunities for partnerships with the United States. While in Nairobi, Secretary Raimondo will meet with Kenyan government officials, as well as trade and digital ministers from across Africa. She will also engage with U.S. and Kenyan private sector representatives and speak with women and youth leaders in science, technology and creative industries. The Secretary’s visit builds on the Department of Commerce-wide Africa strategy announced by Deputy Secretary Graves last fall at the AGOA forum in South Africa and underscores the enormous potential that comes with technological advances to support our societies and economies to address global challenges.

Measles is a highly infectious febrile rash illness and was declared eliminated in the United States in 2000. However, measles importations continue to occur, and U.S. measles elimination status was threatened in 2019 as the result of two prolonged outbreaks among undervaccinated communities in New York and New York City. To assess U.S. measles elimination status after the 2019 outbreaks and to provide context to understand more recent increases in measles cases, CDC analyzed epidemiologic and laboratory surveillance data and the performance of the U.S. measles surveillance system after these outbreaks. During January 1, 2020–March 28, 2024, CDC was notified of 338 confirmed measles cases; 97 (29%) of these cases occurred during the first quarter of 2024, representing a more than seventeenfold increase over the mean number of cases reported during the first quarter of 2020–2023. Among the 338 reported cases, the median patient age was 3 years (range = 0–64 years); 309 (91%) patients were unvaccinated or had unknown vaccination status, and 336 case investigations included information on ≥80% of critical surveillance indicators. During 2020–2023, the longest transmission chain lasted 63 days. As of the end of 2023, because of the absence of sustained measles virus transmission for 12 consecutive months in the presence of a well-performing surveillance system, U.S. measles elimination status was maintained. Risk for widespread U.S. measles transmission remains low because of high population immunity. However, because of the increase in cases during the first quarter of 2024, additional activities are needed to increase U.S. routine measles, mumps, and rubella vaccination coverage, especially among close-knit and undervaccinated communities. These activities include encouraging vaccination before international travel and rapidly investigating suspected measles cases.

Introduction

Measles is a highly infectious acute, febrile rash illness with a >90% secondary attack rate among susceptible contacts ( 1 ). High national 2-dose coverage with the measles, mumps, and rubella (MMR) vaccine led to the declaration of U.S. measles elimination* in 2000 ( 2 ). However, this elimination status was threatened in 2019 because of two prolonged outbreaks among undervaccinated communities in New York and New York City; these outbreaks accounted for 29% of all reported cases during 2001–2019 ( 2 ). To assess U.S. measles elimination status after the 2019 outbreaks and to provide context for understanding more recent increases in measles cases in 2024, † CDC assessed the epidemiologic and laboratory-based surveillance of measles in the United States and the performance of the U.S. measles surveillance system during January 1, 2020–March 28, 2024.

Reporting and Classification of Measles Cases

Confirmed measles cases § ( 1 ) are reported to CDC by state health departments through the National Notifiable Disease Surveillance System and directly (by email or telephone) to the National Center for Immunization and Respiratory Diseases. Measles cases are classified by the Council of State and Territorial Epidemiologists as import-associated if they were internationally imported, epidemiologically linked to an imported case, or had viral genetic evidence of an imported measles genotype ( 1 ); cases with no epidemiologic or virologic link to an imported case are classified as having an unknown source ( 1 ). For this analysis, unique sequences were defined as those differing by at least one nucleotide in the N-450 sequence (the 450 nucleotides encoding the carboxyl-terminal 150 nucleoprotein amino acids) based on the standard World Health Organization (WHO) recommendations for describing sequence variants ¶ ( 3 ). Unvaccinated patients were classified as eligible for vaccination if they were not vaccinated according to Advisory Committee on Immunization Practices recommendations ( 4 ). A well-performing surveillance system was defined as one with ≥80% of cases meeting each of the following three criteria: classified as import-associated, reported with complete information on at least eight of 10 critical surveillance indicators (i.e., place of residence, sex, age, occurrence of fever and rash, date of rash onset, vaccination status, travel history, hospitalization, transmission setting, and whether the case was outbreak-related) ( 5 ), and laboratory-confirmed.

Assessment of Chains of Transmission

Cases were classified into chains of transmission on the basis of known epidemiologic linkages: isolated (single) cases, two-case chains (two epidemiologically linked cases), and outbreaks (three or more epidemiologically linked cases). The potential for missed cases within two-case chains and outbreaks was assessed by measuring the interval between measles rash onset dates in each chain; chains with more than one maximum incubation period (21 days) between cases could indicate a missing case in the chain. This activity was reviewed by CDC, deemed not research, and was conducted consistent with applicable federal law and CDC policy.**

Reported Measles Cases and Outbreaks

CDC was notified of 338 confirmed measles cases with rash onset during January 1, 2020–March 28, 2024 ( Figure ); cases occurred in 30 jurisdictions. During 2020, 12 of 13 cases preceded the commencement of COVID-19 mitigation efforts in March 2020. Among the 170 cases reported during 2021 and 2022, 133 (78%) were associated with distinct outbreaks: 47 (96%) of 49 cases in 2021 occurred among Afghan evacuees temporarily housed at U.S. military bases during Operation Allies Welcome, and 86 (71%) of 121 cases in 2022 were associated with an outbreak in central Ohio. During 2023, 28 (48%) of 58 cases were associated with four outbreaks. As of March 28, 2024, a total of 97 cases have been reported in 2024, representing 29% of all 338 measles cases reported during January 1, 2020–March 28, 2024, and more than a seventeenfold increase over the mean number of cases reported during the first quarter of 2020–2023 (five cases).

Characteristics of Reported Measles Cases

The median patient age was 3 years (range = 0–64 years); more than one half of cases (191; 58%) occurred in persons aged 16 months–19 years ( Table ). Overall, 309 (91%) patients were unvaccinated (68%) or had unknown vaccination status (23%); 29 (9%) had previously received ≥1 MMR vaccine dose. Among the 309 cases among unvaccinated persons or persons with unknown vaccination status, 259 (84%) patients were eligible for vaccination, 40 (13%) were aged 6–11 months and therefore not recommended for routine MMR vaccination, and 10 (3%) were ineligible for MMR because they were aged <6 months. †† Among 155 (46%) hospitalized measles patients, 109 (70%) cases occurred in persons aged <5 years; 142 (92%) hospitalized patients were unvaccinated or had unknown vaccination status. No measles-associated deaths were reported to CDC.

Imported Measles Cases

Among all 338 cases, 326 (96%) were associated with an importation; 12 (4%) had an unknown source. Among the 326 import-associated cases, 200 (61%) occurred among U.S. residents who were eligible for vaccination but who were unvaccinated or whose vaccination status was unknown. Among 93 (28%) measles cases that were directly imported from other countries, 34 (37%) occurred in foreign visitors, and 59 (63%) occurred in U.S. residents, 53 (90%) of whom were eligible for vaccination but were unvaccinated or whose vaccination status was unknown. One (2%) case in a U.S. resident occurred in a person too young for vaccination, two (3%) in persons who had previously received 1 MMR vaccine dose, and three (5%) in persons who had previously received 2 MMR vaccine doses. The most common source for internationally imported cases during the study period were the Eastern Mediterranean (48) and African (24) WHO regions. During the first quarter of 2024, a total of six internationally imported cases were reported from the European and South-East Asia WHO regions, representing a 50% increase over the mean number of importations from these regions during 2020–2023 (mean of two importations per year from each region).

Surveillance Quality Indicators

Overall, all but two of the 338 case investigations included information on ≥80% of the critical surveillance indicators; those two case investigations included information on 70% of critical surveillance indicators. Date of first case report to a health department was available for 219 (65%) case investigations; 127 (58%) cases were reported to health departments on or before the day of rash onset (IQR = 4 days before to 3 days after). Overall, 314 (93%) measles cases were laboratory confirmed, including 16 (5%) by immunoglobulin M (serologic) testing alone and 298 (95%) by real-time reverse transcription–polymerase chain reaction (rRT-PCR). Among 298 rRT-PCR–positive specimens, 221 (74%) were successfully genotyped: 177 (80%) were genotype B3, and 44 (20%) were genotype D8. Twenty-two distinct sequence identifiers (DSIds) ( 3 ) for genotype B3 and 13 DSIds for genotype D8 were detected (Supplementary Figure, https://stacks.cdc.gov/view/cdc/152776 ). The longest period of detection for any DSId was 15 weeks (DSId 8346).

Chains of Transmission

The 338 measles cases were categorized into 92 transmission chains (Table); 62 (67%) were isolated cases, 10 (11%) were two-case chains, and 20 (22%) were outbreaks of three or more cases. Seven (35%) of 20 outbreaks occurred during 2024. §§ The median outbreak size was six cases (range = three–86 cases) and median duration of transmission was 20 days (range = 6–63 days). Among the 30 two-case chains and outbreaks, more than one maximum incubation period (21 days) did not elapse between any two cases.

Because of the absence of endemic measles virus transmission for 12 consecutive months in the presence of a well-performing surveillance system, as of the end of 2023, measles elimination has been maintained in the United States. U.S. measles elimination reduces the number of cases, deaths, and costs that would occur if endemic measles transmission were reestablished. Investigation of almost all U.S. measles cases reported since January 2020 were import-associated, included complete information on critical surveillance variables, were laboratory-confirmed by rRT-PCR, and underwent genotyping; these findings indicate that the U.S. measles surveillance system is performing well. A variety of transmission chain sizes were detected, including isolated cases, suggesting that sustained measles transmission would be rapidly detected. However, the rapid increase in the number of reported measles cases during the first quarter of 2024 represents a renewed threat to elimination.

Most measles importations were cases among persons traveling to and from countries in the Eastern Mediterranean and African WHO regions; these regions experienced the highest reported measles incidence among all WHO regions during 2021–2022 ( 6 ). During November 2022–October 2023, the number of countries reporting large or disruptive outbreaks increased by 123%, from 22 to 49. Global estimates suggest that first-dose measles vaccination coverage had declined from 86% in 2019 to 83% in 2022, leaving almost 22 million children aged <1 year susceptible to measles ( 6 ).

As has been the case in previous postelimination years ( 7 ), most imported measles cases occurred among unvaccinated U.S. residents. Increasing global measles incidence and decreasing vaccination coverage will increase the risk for importations into U.S. communities, as has been observed during the first quarter of 2024, further supporting CDC’s recommendation for persons to receive MMR vaccine before international travel ( 4 ).

Maintaining high national and local MMR vaccination coverage remains central to sustaining measles elimination. Risk for widespread U.S. measles transmission remains low because of high population immunity; however, national 2-dose MMR vaccination coverage has remained below the Healthy People 2030 target of 95% (the estimated population-level immunity necessary to prevent sustained measles transmission) ( 8 ) for 3 consecutive years, leaving approximately 250,000 kindergarten children susceptible to measles each year ( 9 ). Furthermore, 2-dose MMR vaccination coverage estimates in 12 states and the District of Columbia were <90%, and during the 2022–23 school year, exemption rates among kindergarten children exceeded 5% in 10 states ( 9 ). Clusters of unvaccinated persons placed communities at risk for large outbreaks, as occurred during the central Ohio outbreak in 2022: 94% of measles patients were unvaccinated and 42% were hospitalized ( 10 ). Monitoring MMR vaccination coverage at county and zip code levels could help public health agencies identify undervaccinated communities for targeted interventions to improve vaccination coverage while preparing for possible measles outbreaks. As of March 28, 2024, a total of 97 confirmed measles cases have been reported in the United States in 2024, compared with a mean of five cases during the first quarter of each year during 2020–2023. Similar to cases reported during 2020–2023, most cases reported during 2024 occurred among patients aged <20 years who were unvaccinated or whose vaccination status was unknown, and were associated with an importation. Rapid detection of cases, prompt implementation of control measures, and maintenance of high national measles vaccination coverage, including improving coverage in undervaccinated populations, is essential to preventing measles and its complications and to maintaining U.S. elimination status.

Limitations

The findings in this report are subject to at least three limitations. First, importations might have been underreported: 4% of reported cases during the study period had no known source. Second, case investigations resulting in discarded measles cases (i.e., a diagnosis of measles excluded) are not nationally reportable, which limits the ability to directly evaluate the sensitivity of measles case investigations. However, surveillance remains sufficiently sensitive to detect isolated cases and outbreaks, and robust molecular epidemiology provides further evidence supporting the absence of sustained measles transmission in the United States. Finally, the date of first case report to a health department was not available for 35% of case investigations.

Implications for Public Health Practice

The U.S. measles elimination status will continue to be threatened by global increases in measles incidence and decreases in global, national, and local measles vaccination coverage. Because of high population immunity, the risk of widespread measles transmission in the United States remains low; however, efforts are needed to increase routine MMR vaccination coverage, encourage vaccination before international travel, identify communities at risk for measles transmission, and rapidly investigate suspected measles cases to maintain elimination.
