The researching part of the outreach is not something you should rush through.

Once you have found the blogs that you would like to reach out to, you need to do a bit of investigative work on their website.

Things to keep in mind before reaching out are:

  • When was their last post
  • Do they have comments and likes on their posts
  • Are they active on social media
  • Have they got guest posts already
  • Do they say how big their mailing list is

Drafting the outreach email

Make sure the subject and preview of your email are intriguing but not clickbaity or spammy.

When it comes to pitching a blog tour or a guest post it’s important to make sure that the pitch has information in it that is personal to each blog so that the readers don’t think it is just a spam email. Put an interesting and personal spin on your idea.

This is why the research part is so important. Find something you love about each blog and mention it. 

Also, mention how the guest post or tour could benefit the blog. 

You can also pitch a couple of ideas you would like to write about if you want to give some options. 

Now that you’ve reached out and pitched the ideas and they’ve been accepted, it’s time to create the content. 

When creating the content make sure that it is 100% original and written by you. Don’t be boring or generic. Use your own voice and unique author brand . The whole point of this book blog tour is for a new audience to get to know you and your books.

What is it?

It’s a marketing strategy frequently used by self-published authors when launching new books. Essentially, i t’s a virtual tour by an author, around a number of blogs.

What Constitutes an Online Book Tour?

Virtual tours consist of a number of different content formats, including:

· Guest blog posts

· Articles for your blog

· Interviews

· Radio and podcast appearances

· YouTube videos

· Book reviews

· Book giveaways

· Social media contests

· Book excerpts

· Live chats in both video and text format (e.g., Facebook Live, Google Hangouts, Facebook Events, Twitter Chats, etc.)

The goal is to obtain as much coverage of your book release as possible while remaining relevant to both yours and your host’s audience.

When it the best time to do one?

R un a blog tour in the week before your publication date to get some online publicity ahead of the launch event. By sending your book out to bloggers in advance, you’ll get their reviews and can include some of their testimonial words on your website and author pages. However, if you’ve already published then it’s not too late. You can still arrange a blog tour for a book that was published a while ago because the bloggers’ interest will increase the visibility of your book on social media.

Can I Organise my Own?

Yes! Have a strategy, do your research and plan it well! Here is a list of ‘to do’s from the beginning of your tour to the end!

1) Identify your ideal reader. When you know precisely who you want to connect with, you will be able to narrow down which blogs to reach out to.

2) Connect with bloggers that have an active community. It won’t do you or your book any good if no one is reading the blog or taking part in conversations. See how often they post on the blog, if there are comments on their posts, and if they have shares/likes/comments on social media.

3) Create a list - find at least 10-20 bloggers to start with and compiling them into a list. Not all of them will work for you, so gather more than you’ll need.

4) Determine your resources. Bloggers might request different information and resources from you to be featured. Some might want you to write a guest blog post, others might want to review your book, and some will do a quick Q&A.

5) Create your content – you may need to write additional content, answer questions they submit, and decide what chapters you would like to use as excerpts.

6) Design a blog tour poster which includes the author bio and any other marketing material. Include an image that you can share that has the blog stops for your tour along with the URL for each stop.

7) Work out a time frame. E stablish a potential start and finish date. How long a blog tour lasts really depends on how much time you can dedicate to it.

8) Contact your bloggers giving plenty of notice. The more established book bloggers are usually “booked” up at least 3 months in advance

9) Stay organised. Good organisation helps make sure that everything goes smoothly.

10) Consider giveaways. Free incentives can help get your book in front of new eyes - who doesn’t love to win something? And bloggers love to give things away to their readers.

11) Announce the tour. Once you have several blog stops confirmed, announce the blog tour on your website, newsletter, and social media. Create an image that you can share that has the blog stops for your tour along with the URL for each stop. Tag people to RT or share

12) Follow through the delivery. Be considerate of each blogger’s schedule and send your post and information at least a week ahead of time. Considering their schedule (and timeframe) is key in creating lasting relationships with these bloggers, which will help the next time you want to do a blog tour. Make sure you send an email of thanks!

Do I Pay my Bloggers?

No. No money changes hands between authors/publishers and bloggers. If you have hired an organizer, then they will be paid for the actual administration involved, but bloggers take part voluntarily.

How Long is a Book Blog Tour?

3 to 14 days? Depends on how many bloggers accept the invitation to read your book and get involved. Each blogger is given a specific date for when they’ll write a piece about your book. If your book is in a popular genre, you could have a longer two-week blog tour, where you might get two reviews hitting social media per day. The bloggers will happily share their review, although you can’t specify the time, because it has to fit around their other commitments.

Should I Hire a Book Blogger Organiser?

It’s your choice. Y ou will have to pay for blog tours, but you’ll usually get your book in front of targeted readers for your genre. Each service is different, and a blog post, cover reveal, or author interview won’t necessarily send you shooting up Amazon’s charts, but it could get you some visibility. And considering someone has to see your book 7 to 8 times before they buy it, this is a good thing.

Here are a few tips for helping to decide which tour works best for you:

  • If you are a new author or promoting a new release and are okay with writing unique content posts (interviews, guest post etc) book a Blog Tour .
  • If you don’t have a lot of time to write unique content posts (interviews, guest post etc), but do want to do a book tour and get reviews, book a Book Blitz Deluxe . Or pick the Blog Tour and offer premade interview/ guest posts only.
  • If your book isn’t released yet and you want to promote it before release day, consider doing a Cover Reveal (if your cover isn’t revealed yet) or a Review Opportunity (these work great for not yet released books).
  • If you don’t have a lot of budget, pick the Review Opportunity if you want reviews only, the Book Blitz if you want a tour/ exposure.
  • If your focus is on Instagram and you want bookstagrammers making pretty pictures of your book, pick the Blog Tour , Book Blitz , Book Blitz Deluxe or Review Opportunity and offer physical copies for review. Having square graphics also helps.
  • If your main and only goal is reviews or if your goal is as many reviews as possible, pick the Review Opportunity .
  • If you want to book multiple tours for the same book start with the Blog Tour , followed by a Review Opportunity once the tour has ended (you can also swap the order of these two). You can follow this up later by a Book Blitz / Book Blitz Deluxe at a later date to promote a discount for example.

Book Blog Tours That Accept Self-Published Authors

A book blog tour is when you “appear” on a number of blogs over a week or two (some authors go bananas and schedule a different blog each day for a month) to promote your book. This usually involves being interviewed or writing a guest post for the site. Some blog owners will also read and review your book.

You can arrange tours yourself and hand-pick the blogs, keeping in mind that some people won’t respond or be interested in hosting you, or you can pay someone else to arrange things for you. Prices vary, as do the quality of the blogs that participate (naturally, you want to appear on established sites with a solid readership).

It’s been a while since I did a book blog tour (almost three years), but I may check into them again this summer, since I’m working on some new series. As I recall, the two or three tours I did weren’t all that useful insofar as selling books, but they did result in me getting some much needed reviews back in the days when I didn’t have any readers yet. Several of the hosts reviewed my book on their own sites and also posted the reviews on Amazon and Goodreads.

I’m fairly certain most bloggers and tour operators don’t guarantee reviews, but it’s natural that some of the bloggers will be curious about the authors they host and might check out your book of their own accord (you can also include a free copy with your post or interview).

In case you’re interested in trying out a book blog tour for yourself, I’m posting a list of some of the tours that accept indie/self-published authors and that aren’t hugely expensive (thanks, Elise, for putting the list together for us!):

Reading Addiction Blog Tours and “I’m a Reader, Not a Writer” Book blasts have both been really successful (and affordable) for me. Not only do tours provide exposure, but you get reviews, which for new authors is vital. Both accept self-published authors, typically in the YA or romance space, but they do cover some SciFi Fantasy outside of paranormal.

I had a totally different experience with Reading Addiction a couple months back.

I am not an indie author, but a publicist, and I placed 5 books on 5 different book blog tour companies. All had the same number of stops (30), but Reading Addiction Virtual Book Tours is the only company I felt I was mislead by.

I paid for the Major Addiction tour which trumpeted 30 stops over 6 weeks, with 10-15 reviews. This was for an award-winning YA dystopian novel. While this is subjective, compared to the quality of books placed with other tour companies, I would rank this particular novel #2 out of the 5.

These were the biggest issues with Reading Addiction:

1. More than half of the blog sites hosting stops were either very obviously run by Reading Addiction or were sites that ONLY hosted Reading Addiction blog tours. Their tour hosts had little to no other content besides Reading Addiction Virtual posts. I don’t think it needs to be pointed out that book blogs with nothing but content from a single tour company (or only content from tour companies in general) are suspect.

2. Out of the 10-15 review stops, there were—in actuality—only 7 bloggers selected the book for a review. Out of those 7, 2 never posted their reviews (and were never heard from, meaning the tour days were lost). Out of the other 5, only a single blogger cross posted onto Amazon. SO, out of 10-15 review stops advertised, there were only 5 completed and only 1 that made it off a hosts blog site.

3. Reading Addiction is really disorganized. It was left up to me to check the stops daily and to notify them when a committed post on the tour wasn’t made. Sometimes it was posted within an hour or two after, sometimes it wasn’t at all. Worst of all, when it was NOT posted for the day I never heard back from Reading Addiction. This was frequent. Nearly ¼ of the scheduled stops weren’t posted until I inquired about them. Not that it makes much difference (see #1).

There are other issues, but these should be enough to discourage authors and publicists from using Reading Addiction Virtual Book Tours. The other 4 companies I worked with all commented on stops along the way (which showed they were clearly checking for posts themselves), all had a variety of quality book blogs with both organic and tour content, and all maintained the advertised number of reviews with excellent cross-posting on both Amazon and Goodreads.

Needless to say, my experience with Reading Addiction was not a good one. But the simultaneous tours with Sage’s, Xpresso, and Goddess Fish were all perfect.

For those of you who are a little more DIY, there’s also Blogtour.org. It’s free. Full disclosure- I run it, and I built it specifically so I wouldn’t have to cold call blogs that wouldn’t be interested in having authors do tour stops.

I think for struggling authors this is a real good option that’s overlooked. Many tour sites only charge $50 to $100 and a whole lot charge under $50.

Like you said, sometimes these are good options for new books that don’t have reviews. You’ll often pick up a few, and that could give you enough to get picked up by a larger promo site like BookBub or BookBlast.

Based on the tours I see them host, Goddess Fish does more indie/self pub/small press titles.

TLC seems to have more established authors mixed in. They also do tours for books that have been out a year or so.

Are blog tours actually helpful? I get regular requests via email from people asking if I’ll host an author’s post on my website. But, I’m a pretty weak sauce blogger and my website mostly just has my product updates, like a glorified catalog. Hosting on my website doesn’t seem like it would benefit these authors. I feel like there’s something about blog tours that I don’t get, at a deeper level.

That sounds more like a guest post with little behind it other than the person’s desire to be on your site. Now if they comment a lot maybe it’s a good thing.

The good blog tour sites will usually fit your book with sites that want that genre. People visiting those sites usually do so because they like hearing about fantasy book, for instance.

A regular blog tour might just be your book cover and blurb and that’s about it.

A virtual book tour will be you writing out a guest post, author interview, and maybe even character interview or description of your world.

You’re encouraged to reply to comments people make on your post. Really, interaction is key in my opinion.

Sure, some sites throw you to the wind, take your money and that’s it. The good ones, however, send you a list of blogs you’ll be on, your banner ads, and other things a month or more in advance.

Goddessfish is a good example of this, and I think for the little authors spend (usually under $100) you could get a lot of benefits.

Honestly, the sites I don’t like are the ones that offer review only tours, guaranteeing you 6 to 8 reviews or something. They’re out there.

The better sites do marketing in such a way that those reviews happen organically, and you get more of that long-tail benefit. Well, that’s the idea.

I think they’re most helpful for getting reviews. If you have a big fan base already and get reviews on your new stuff right off the bat, then you might not get much out of a blog tour. (Though it’s something I might consider doing again when jumping into a new genre — one’s current readers might not necessarily be the biggest fan of what one’s trying next!)

Lindsay, curious about your thoughts on blog tours these days. Is it something you would/do use for new releases?

Wesley, I haven’t signed up for any in at least four years, so I’m not sure what’s out there and what’s effective now. I think it can still be a way to get some early reviews though. I’ve heard of people contacting bloggers and setting up their own tours, and that may be more effective, since you can pick and choose blogs that are relevant and get at least a little traffic.

5 Tips for A Successful Book Blog Tour

A book blog tour is a virtual tour in which book bloggers present your latest work with their readers, giving you an extra burst of hype every time you want it and bringing more fans onto your network. These virtual tours are fun and celebratory, but launching a virtual tour yourself comes with a lot of work and needs careful attention to work effectively.

If you want to run a book blog tour, you have to find the bloggers, contact them, offer whatever they’re looking for, and stay on track with a set schedule (most tours last two to six weeks). Of course, there are paid tour companies to help you out , but some of these services are scams or have few genuine interactions. Paid companies also won't take the time to match you to custom blogs as well as you could arrange for yourself, making the process a risk and likely not worth the expense invested.

The best tours are often the ones that you put together yourself. When you pinpoint the right bloggers, send them custom submissions and share your book with book blogs that will rave over it, you'll walk away with a far more positive tour experience and a longer list of readers who are actively excited about your latest release.

Here are five tips to increase your odds of bloggers taking you in and to simplify the book blog tour process so that you can spend your time relaxing and socializing with readers instead of screaming at the computer and pulling your hair out.

There are more blogs out there than you could dream of submitting to, so don’t waste your time on any that won’t care about your book. You want to look into other factors as well, since many reviewers will take time away from reviewing or go on hiatus to pursue other interests. If it looks like a blogger isn’t right for you, don’t waste your time.

When researching to see if a blog would be a good fit for your tour, the top places to look include:

Reviewers are human. Reviewers also like to know that you’re human, too, and spamming everyone with the exact same message will make your entire submission fall flat.

For most tours, you will need to pitch a broad range of book reviewers, and staying fresh every time can be a challenge. Easy ways to personalize your submission are to follow all submission guidelines carefully, respond to something that you like about the blog or reviewer, or react to a recent post.

This is very much like the process of pitching an agent for traditional and hybrid authors, and many of the steps are identical. The good news is that reviewers read books more for fun than as a profession, and they say yes much more readily to submissions that catch their attention.

This is a good time to refresh your skill with spreadsheets. Keep track of every blog you contact, every response you get, and the dates that you set for the posts themselves. This will come in handy as you get closer to your tour dates and will want to know who to follow back on and how busy you can expect to be on various dates. You have hooked the blog host and now you have to hook their readers into buying your book.

You don't need to do all virtual book tours and book blog tours at once. Pick out one or two virtual book tours a month or every two weeks depending on your financial situation. Make sure they offer different promotional tools. You don't want duplicate book tours at the same time. You want to make sure that you cover the book market with different book offerings, book giveaways, book reviews, book and author interviews, spotlight radio interviews, podcasts, webinars that create wonderful book buzz. Once you are finished with the guest blog interview or book review and it has been added to the host blog's website, you need to link back from your website to their website. Your next step is to share with your social media networks and the host blog website shares with their social media networks. This is how your book buzz and audience grow.

Remember, you need to create book buzz 24 hours a day even when you are sleeping. That is why virtual book tours are a powerful promotional tool for authors because they create book buzz while you are sleeping.

【PHR普及推進協議会】PHR座談会第1回「患者と医療者をつなぎ支えるPHRサービス」

個人の生活に紐づく健康・医療・介護等に関するデータであるPHR(Personal Health Record)は、患者への適切な医療の提供、質の向上だけでなく、現場の負担軽減にもつながることから、医療機関を中心に注目を集めています。PHR普及推進協議会はこのほど、「患者と医療者をつなぎ支えるPHRサービス」をテーマにした座談会を開催。3社の賛助会員企業が参加し、医療機関とPHRサービス事業者の連携や、PHRと医療機関の連携が患者様や現場の方々にもたらすメリットについて語りました。PHRの普及推進につながるヒントが多く出てきた座談会の様子をお伝えします。

患者の治療記録を一元管理できるスマートフォンアプリを開発  

石見:まずは皆様の事業内容をご紹介いただければと思います。

田中:アストラゼネカは、大きく分けて3つのPHR関連の開発に取り組んでいます。 

1つ目は、患者さん自身の健康管理を支えられるシステムの開発です。システムは予防と治療の2段階に分けられ、予防では、ビッグデータに基づくAIアルゴリズムにより、患者さんの健康状態を予測し、罹患を防ぐことを目指します。治療では、早期介入することで、重症化を防ぐことを目指した 仕組みとなっています。 

2つ目は、個人健康情報管理プラットフォームサービスを提供するパートナー、例えば株式会社Welbyと連携して開発・提供するスマートフォンアプリです。共同開発するスマートフォンアプリは、患者さんが日々の状態を記録することで自身の治療記録を一元管理し、可視化も図ります。 

3つ目は、薬の副作用を事前検知するシステムです。その一例は、患者さんにウェアラブルデバイスを装着していただき、血中の酸素濃度等から体調をモニタリングします。さらに、モニタリングしたデータを解析し、健康状態が悪化するとアラートを発出することに繋げる仕組みを考えています。 

安達:テルモは、糖尿病をお持ちの方向けサービスとして「メディセーフデータシェア」というクラウド型データマネジメントシステムを提供しています。糖尿病の治療ステージがいくつかある中で、メディセーフデータシェアは、インスリンを投与している方に向けたサービスという位置付けです。 

メディセーフデータシェアは、NFC(近距離無線通信)を搭載した弊社の血糖測定器と連携させることで、簡単に血糖値を記録できます。また、食事や運動などの生活習慣記録や、他のバイタルデータも記録可能です。さらに、患者さん、医療機関の双方のアカウントを連携させることで、医師が診察の中でも活用できるサービスとなっています。 

電子カルテとPHRのデータを統合する重要性 

石見:アストラゼネカのアラートシステムにせよ、テルモのメディセーフデータシェアにせよ、家庭で収集したデータをいかに医療者につなぐかが大事な役割になるのかなと思います。家庭で収集したデータを医療者につなぐという役割について、マイクロソフトはどのようにお考えですか。 

石川:データを総合的に集めていく重要性については昔から言われていますが、医療機関の多くはAI(人工知能)を駆使するためのデータを集めることに苦労しています。データの細分化により、電子カルテとPHRのデータ・情報が分かれてしまっているためです。これらをいかに統合するかが重要ですね。 

さらに、テクノロジーのサイロ化により、医療データの収集や分析、活用といった各領域にいるプロフェッショナルな人材が分断されているという課題があります。このような状況のため、最初のデータ収集で時間がかかり、AIの活用がなかなか進みません。 

あとはユーザーインターフェース(UI)も課題に挙げられます。各データ領域で、UIが違うので、現場のスタッフや患者さんの思考がストップしてしまっています。この現状を踏まえると、今後は、患者さんが自ら操作する時でも自動的にデータが1つのプラットフォームに吸い上げられていくといった世界観の実現が求められています。未来図は見えていますので、各システムで収集したデータを統合し、いかに医療者につなげていくかが鍵になると思います。 

PHRと医療機関の連携によって、患者の治療に対するモチベーションが維持される  

石見:続いて、PHRと医療機関がつながると、患者さんにどういったメリットがあるかについてお伺いできればと思います。 

田中:メリットは短期、長期の両方で切り分けられると思います。短期的には、服薬状況や体温、副作用といった日々の状態を病院側と共有することで、患者さんと担当医とのコミュニケーションが円滑になるのがメリットといえるでしょう。 

長期的には、データの経過的な観察により、疾病の長期的なリスクを特定できるのがメリットです。疾病のリスクは、ピンポイントのデータだけではわかりません。日々の患者さんのデータが手元にあって初めて、変化が現れます。

安達:PHRと医療機関の連携によるメリットは、患者さんの糖尿病治療に対するモチベーションが維持されることです。 

糖尿病をお持ちの方の中には、就業者を筆頭に、受診間隔が1カ月、長い方で3カ月くらい空いてしまう方が少なくありません。観察できるデータのタイムラグが起きると、そうした方は徐々にモチベーションを低下させてしまいます。 

一方、PHRを活用すると、受診間隔の長短に問わず、患者さんがデータを記録・観察できます。例えば、メディセーフデータシェアには、患者さんがアップロードした写真を介して医師とやり取りできる機能があります。この機能により、患者さん側は優しく見守ってもらっている気持ちになり、治療のモチベーション維持につながります。この点、PHRは、受診間隔が空いてもその間隔を埋めてくれる存在ともいえると思います。 

成功体験を積み重ねることで、ロールモデルを構築できる 

石見:家庭で収集したデータを医療者につないで、患者さんのエンゲージメント向上につなげるというアプローチについては、かなり活用されていますね。 

安達:ただ、医療者側の認識としては、PHRの連携を多用すると、業務負担につながるという懸念があります。このため、PHR連携による成功体験を患者さん、医師の双方で積んでもらうのが、有用ではないかと思います。現在の普及状況から考えると、何人かに対象者を絞って使っていただいた上で成功体験を積んでもらうのがベターです。成功体験を積んでもらった後、PHRと医療機関の連携が広がるかと思います。 

石見:今はPHR連携による成功体験を積み重ねる段階ですね。成功体験の積み重ねにより、ロールモデルができると思います。 

PHRは、患者さんが継続的な検査をしているかを測定するうえでも有用です。検査の実施状況のデータもPHRに残っていれば、患者さんが医療機関に訪れた際に過去のデータが共有され、実施すべき検査が明確化されます。 

安達:石見先生のお話を聞くと、PHRによって異なる診療科の先生が、既往歴を一目で確認できる世界観が実現できるのではないかと思います。例えば、眼科の先生が、糖尿病の既往を把握するといった形です。これにより、患者さんが検査の必要性に気づく頻度が増え、重症化予防の領域が進むのではないでしょうか。 

石見:おっしゃる通りですね。私は循環器内科が専門ですが、普段は電子カルテを読み込み、別の診療科の診療録を調べるといった行為はしません。しかし、PHRがあれば、簡単に他科での診療経過も共有できるようになると思います。 

古屋:診療録の共有は、電子カルテが統合されていなくても、別のソフトウェアを立ち上げるだけでも可能なのでしょうか。 

石見:診療録の共有は、段階があると思います。最終的には電子カルテを通じた情報共有が理想ですが、電子カルテは運用上の制約が多々あります。このため、短期的には先ほどの成功体験のような話で、簡易のタブレット等を経由して情報が伝わる形で十分かと思います。 

PHRの普及には、標準化されたプラットフォームの構築が鍵

石見:しかし、今はPHR連携の成功体験がないので、医療機関側に電子カルテの改修といったモチベーションがありません。マイクロソフトはその点についてどのようにお考えでしょうか。 

石川:厚生労働省は、「医療DX令和ビジョン2030」の中で標準型電子カルテの開発に本年度から取り組む予定ですが、「PHRを取り込む仕組みを入れよう」といった意見は聞こえていません。 

今は病院ごとにPHR連携の仕組みを構築するのが難しいので、PHRを取り込む前提で標準型電子カルテを作っていけば、PHRと医療現場に溶け込んだ世界が実現されるかと思います。いずれにせよ、PHRの普及に向けては、国が音頭を取って、PHRを取り込むプラットフォームを構築するのが重要です。 

石見:おっしゃる通りで、標準化されたプラットフォームの構築が本日の議論の鍵になるかと思います。標準化されたプラットフォームへの連携を想定しながら、まずは生活習慣病や救急災害といったPHR連携のメリットが分かりやすい領域にフォーカスし、日常的なデータを複数の医療者と共有して医療の質が高まるといった実績を積み重ねるのが理想的と考えています。 

標準化するのはプラットフォームだけではありません。PHRの指標や基準を標準化するのも重要ですよね。例えば、酸素飽和度や体重といった指標の基準を標準化しておき、どのPHRサービスでも基準を見られるようにするといった形です。 

そのうえで、受診する医療機関に関係なく、各医療機関の医療者がPHRを見られるようにします。これにより、ガン専門の医師が気づけない疾患を、循環器の医師が気づけるといったメリットも生まれるでしょう。 

安達:ただ、こうしたPHR連携が進むと、患者さんの病状の変化を見逃した時の責任の所在が問題になるかと思います。PHR連携を進めるだけなく、PHRを通じた病状のスコア化、可視化も重要です。そのスコアもガイドラインで規定されていれば、医療機関によるPHRサービスの普及が進むと思います。

PHRによる責任の増加を懸念していては、医療が永遠に発展しない  

石見:おっしゃるようなPHR連携の課題については、医療が発展していく過程の中での課題ではないかと思います。このような課題は、AED(自動体外式除細動器)が普及する過程でも起きました。 

AEDは2024年7月に、市民による使用が許可されて20周年を迎えます。AEDは当初、学校に設置されていないのが当たり前だったので、使わなくても責任を問われませんでした。 

しかし、今はAEDが当たり前になったので、学校がAEDを適切に使用できなかったら責任を問われます。AEDの普及活動を始めた当初、弁護士さんから「基準が変わるので、責任を問われる方が出てくることも覚悟しなければいけませんよ」と言われていましたが、基準の変化により、責任範囲が広がるのは、医療が発展するということだと思います。 

PHRの場合も、データ連携により見える範囲が広がり得るので、「医療者の責任が問われやすくなる」といった懸念はあります。しかし、責任増加の懸念により、PHRを普及させないという姿勢では、医療が永遠に発展しません。

古屋:そういう意味では、AEDも当初は医療者が使えなかった一方で、現在はわれわれも使えるようになっています。もしかしたら、PHRも医療者だけではなく、一般人のわれわれも一定の判断基準のもと、使えるようになると、すごく世界が広がるのではないかと思いますね。 

石見:PHRの場合は、AEDよりもデータが複雑です。このため、一概に言えないのですが、PHRの運用にあたって医療者に対するサポートもないと、医療者側も二の足を踏んでしまいますよね。普及に際しては、多くの情報処理に追われることがないように医療者の負担を軽減する仕組み、機能の導入や、法律やガイドライン等で医療者に過度の責任を求めることなく、PHR連携の活用が進むようサポートしていくことも重要です。 

対話型AIが必要なデータの抽出を簡素化する

大嶽:やはり、PHRの普及にあたっては、インターフェースを1つに統一することが重要です。われわれは、医療者もPHRを活用できるように、インターフェースを統一したうえでデータ収集・分析といった1つ1つの作業を簡略化する開発姿勢を非常に重要視しています。 

1つのパッケージの中で、時にAIの力を借りながら、いろんなPHRサービスにあるデータを分析できるプラットフォームが理想です。こうしたプラットフォームであれば、利用者側に専門的なスキルが必要ありません。われわれは、医療者が簡単に患者さんの状態を確認できる形を目指し、プラットフォームづくりを進めています。 

石川:われわれは、双方向でやり取りできる対話型AIを実装した電子カルテについても、構想を描いています。構想する電子カルテでは、医師が質問すると、AIが必要なデータを可視化したり、現状の健康状態から逆算した検査数値の予測値を出したりしてくれます。対話型AIを実装した電子カルテのように、今後は、たくさんのデータからほしいデータを引き出せるソフトウェアの開発が重要だと思います。 

石見:その辺りがまさにインタラクティブAIがサポートしてくれる領域だと感じますね。 

古屋:医療者が必要なデータは限られていると思います。複数のデータをビジュアライズして見やすくする方法もありますが、一番重要なのは、必要なデータの抽出です。この点、AIが必要なデータを抽出する上で役に立つかもしれません。 

田中:それでも、AIが抽出するデータが現場の医師が使うデータと全く同じであれば、AIが大きく大きな発展しないのではないかと思います。 

AIで抽出したデータと現場の医師が使うデータの間に若干の違いが出てしまうのが、ある意味、理想です。AIを通じたデータの収集・分析を進める上では、通常の診察よりも広い範囲のデータを集めつつ、要所で大事なデータを取捨選択できるのが重要だと思いますね。 

現場の医師の意見をもとにしたPHRサービスの構築が重要

石見:PHRを医療現場とうまく連携させるためには、PHRサービス事業者と医療機関の双方がやり取りすることが必要です。多くの医療機関では現状、電子カルテを構築する際にも現場の医師の意見が十分に反映されていないように思います。 

先日、ガン治療を専門にする医師が、「多くの患者さんがしんどくても自宅にいたいので、すれすれの状態まで頑張って苦しいと言わないんです。そして、最後に耐え切れなくなって救急搬送されるケースも多いので、自宅での酸素飽和度や患者さんの自覚症状を把握できる機能が欲しい」とおっしゃっていてなるほどと思いましたが、こうした現場の医師の意見をもとにしたPHRサービス、医療との連携の構築が重要ですね。 

現在、ICT(情報通信技術)の発展により、診察時の情報だけでなく、1日単位の体重や血圧など、詳細なPHR(ライフログ)データの収集が可能になりました。これにより、医療情報が増えるという側面がありますが、「一定のラインを超えるとアラートが出る」といった機能を設けることで、現場の負担も減らしつつ、PHRデータを活用することが可能です。PHRサービス事業者の皆さまには、そうした点を意識しながら、サービス開発を進めてもらえればと思います。

National insurance will be cut, for the second time this year, from 10% to 8% on employee earnings between £12,570 and £50,270 from Saturday.

The change, announced by the chancellor in his March budget, impacts around 27 million payroll employees across the UK.

The cut is worth almost £250 to someone earning £25,000 a year and almost £750 for those earning £50,000

There are also national insurance cuts for the self-employed. This includes the scrapping of Class 2 contributions, as well as a reduction of the rate of Class 4 contributions from 9% to 6% for the £12,570 to £50,270 earnings bracket.

These will impact nearly two million self-employed people, according to the Treasury.

While many campaigners welcomed the national insurance announcement last month, they pointed out that the tax burden remains at record high levels for Britons - thanks in part to the threshold at which people start paying income tax being frozen, rather than rising with inflation.

Tesla has reported a sharp fall in worldwide sales amid increased competition and slowing demand for electric vehicles.

The carmaker said it delivered 386,810 vehicles during the first quarter of the year - down nearly 9% on the 423,000 it shipped from January to March last year.

Tesla said the fall was "partly" due to  disruption to shipping in the Red Sea region  and an arson attack at its gigafactory in Berlin.

It also blamed the phasing in of an updated version of its Model 3 sedan at its factory in Fremont, California.

Tesla's chief executive Elon Musk noted that rival electric carmaker BYD had also experienced a slump in sales.

He wrote on X: "This was a tough quarter for everyone."

However, some commentators blamed the controversy surrounding Mr Musk, along with increased competition from rivals and declining "excitement" about electric vehicles generally.

Wall Street tech analyst Dan Ives described the results as "an unmitigated disaster that is hard to explain away".

He said: "This was a train wreck into a brick wall quarter for Musk and co."

Mr Ives also warned Mr Musk needed to turn around the company's fortunes. "Otherwise, some darker days could clearly be ahead that could disrupt the long-term Tesla narrative," he added.

It comes just days after market intelligence firm Caliber claimed Mr Musk's right-wing political views and controversial public statements on X were putting off some customers from buying a Tesla.

"It's very likely that Musk himself is contributing to the reputational downfall," Caliber chief executive Shahar Silbershatz said.

Tesla did not respond to requests for comment about the claims.

Revolution Bars shares were suspended from the London Stock Exchange yesterday as the bar chain mulled its restructuring plans.

The suspension on the AIM index came after the group said it was unable to publish its interim results for the second half of last year by the end of March.

The news comes after an exclusive report by City editor Mark Kleinman found Revolution was considering a plan to closure around 20 bars out of its 80 venues.

Revolution said it continues to "evaluate all the options available to it", including engaging with key stakeholders and potential investors over a possible fundraise.

Their struggles are part of wider difficulties in the night-time economy, which Money team reporter Emily Mee examined earlier this year...

London's FTSE 100 is down around -0.5% this morning after temporarily breaking the 8,000-point barrier yesterday.

There had been expectations that the index could this week break its highest-ever record of 8,012.53 points, which was set in February last year.

However, it looks like the wait will go on - for today at least. It's at around 7,890 points this morning.

The performance echoes a weaker close on Wall Street yesterday, following concerns from some investors that the US Federal Reserve may not cut interest rates by as much as previously expected this year.

Among the firms experiencing a slump on Wednesday is Topps Tiles. Its shares are down almost 3%.

It comes after the company warned of "subdued demand" in a trading update. The firm also said sales had fallen in the second quarter due to lower footfall.

In contrast, Dr Martens opened the day brightly and climbed more than 5% following a report that the footwear brand might explore a possible sale. However, those early gains have since fallen back.

Meanwhile, oil prices have been steadily climbing, with the cost of a barrel of Brent crude hovering around $89 (£71) this morning - a six-month high.

This follows a Ukrainian drone attack on a Russian refinery, coupled with ongoing tensions in the Middle East.

The currency markets are pretty flat - £1 will buy $1.26 and €1.17, similar to yesterday.

Gumtree has revealed the most common items for scam listings.

The online marketplace says smartphones, games consoles, digital cameras, headphones, dogs, collectables and guitars are the most common scam items.

There are also seasonal trends. At the start of the year job posting fraud and gym items for sale are more prevalent, while bikes tend to pop up in spring as people look to get outdoors.

During hot weather, you are likely to see more high-value outdoor items such as non-permanent swimming pools, jacuzzis, trampolines, air conditioning units and campervans.

New vehicle registrations in March and September always see a spike in bad listings, while rental fraud tends to increase when students look for accommodation for the year ahead – normally early summer through to September. 

This data comes as Santander saw cases of purchase scams jump by nearly a third (32%) in 2023.

80% of these started on online selling platforms.

Santander has teamed up with Gumtree to warn shoppers about what to look out for:

  • An item priced more cheaply than expected, or using generic images that could have been taken from an online search, could be warning signs of a scam.
  • Items for sale can often be filtered depending on postcode and location. Shop local, and make sure you see the item in person first.
  • Use secure payment methods where you can. PayPal and debit and credit cards may offer more protection than a bank transfer.
  • Attempts to communicate outside an official website should also be treated with suspicion.
  • Check how long the seller has been a member of the selling website for and look at their profile, including reviews from other buyers. If the seller has been posting from multiple locations within a short timeframe, that could be another reason to be cautious.

Gumtree says it removed 1,177,292 listings for failing to meet its standards and policies in 2023.

42,725 of these raised suspicions of fraud, a decrease of 31% annually - credited by Gumtree on an improved safety infrastructure.

Taxpayers could pay millions to dismantle Shell oil rigs in the North Sea.

Marine pollution regulators have said thousands of tonnes of waste from decommissioned oil platforms must be removed.

Shell has lobbied to keep three platforms in its Brent oil and gas field in place, arguing removing waste will hurt the taxpayer on the basis that the Treasury will have to refund the company for the cost, the Telegraph reports .

But Dominic Pattinson, executive secretary of the Ospar Commission, said Shell was obliged to remove the waste.

The commission is part of the Ospar Convention, an international agreement that includes the UK to protect the environment in the northeast Atlantic.

Removing waste from Shell's rigs could cost more than £400m, according to estimates.

Every Wednesday we get Michelin chefs, top bloggers or critics, to pick their favourite cheap eats where they live and at home. 

This week we've got the owner of London institution Pied à Terre, the legendary David Moore, whose Fitzrovia restaurant has held at least one Michelin star for pretty much forever.

Hi David, can you tell us your favourite places in London where you can get a meal for two for less than £40?

Fitzrovia, and especially Charlotte Street, are so well known for restaurants, fine dining and Michelin stars. There are no less than five Michelin stars on Charlotte Street alone and a gaggle more on adjoining streets. But, where will you spot the Michelin-starred restaurateur having a cheeky bite on his own turf… man can't live on tasting menus alone. A curry, a soup or a sandwich is much needed. So...

Tongue and Brisket (Goodge Street):  Bring your appetite as portions are generous. I have a large salt beef on rye, pickles and mustard, £10 take away, a bit more to eat in. If dining in, try the Latka, fish balls and an excellent chopped liver, cucumber salad on side. No licence for booze… not a bad thing.

Shoop (Tottenham Street):  This tiny, hole-in-the-wall spot serves up some of the tastiest soups you will find this side of Tel Aviv, but it doesn't stop there. Falafel, £8.20; shakshuka, £8.20; and chicken schnitzel, £9.00, are my firm favourites. Everything is cooked fresh on the premises. I don't know how they make it work, but I'm thrilled that they do. There is very limited seating so I grab and go and sit in Fitzroy Square, Goodge Park, Crabtree Gardens, or the new urban gardens on Alfred Street.

Palms of Goa (Charlotte Street):  If I'm going to blow the whole budget of £40 for two, I would head to see Eugiene at Palms of Goa, 12 Charlotte Street.  They have been on Charlotte Street close to 30 years, you don't do that without solid regulars and good food. My favourite is king prawn masala, £15.95, or lamb biryani, £15.95 - plus a couple of naans at £2.95 each and some tap water just to stay on budget. I have to say I usually need a snooze after an Indian lunch… there are worse problems to have.

What's your go-to cheap eat for a night at home?

My go-to easy, cheap eat at home is the dish everyone knows but no one cooks, an omelette. I do a three-egg omelette and put whatever cheese, herbs, ham is in the fridge. Buy some frozen herbs, parsley and chive would be my preference.

Ingredients:

  • salt/pepper
  • generous knob of cold butter - chopped in several cubes 
  • herbs - grated cheese - ham

Crack your eggs into a bowl, season with salt, pepper and herbs - whisk lightly, don't over whisk at this stage as it will get mixed further in the pan when cooking.

Using a non stick pan, heat until smoking hot, move off the heat and add in the cubes of butter, swirl around the pan to coat evenly with the butter. 

When fully melted and lightly browning, add in the eggs, with a fork move the eggs around lifting from the bottom of the pan, repeat several times, and then leave to crust, add fillings to the centre or one side, cook for a further minute or to your desired consistency, fold and serve.

Ads for Nationwide featuring actor Dominic West have been banned for misleading consumers into thinking that the bank – unlike its rivals – would not be closing its branches.

After 282 complaints, the Advertising Standards Authority (ASA) investigated the TV, radio and press ads, in which West plays the boss of a fictional rival of Nationwide.

One of those complaints was from Santander, who said Nationwide had recently closed or reduced opening hours at a number of branches.

A voiceover at the end of the TV advert said: "Unlike the big banks, we're not closing our branches."

The same slogan was used in the press ad, with small print at the bottom saying: "If we have a branch in your town or city, we'll still be there until at least 2026."

The date referred to Nationwide's "Branch Promise" campaign, which last year was strengthened to guarantee it would not close any of its branches until 2026.

The ASA said Nationwide had closed 20% of their estate over 10 years, 152 branches - including two in 2023 - and "we considered that was a significant number".

"Because we considered that consumers would understand from the ads that Nationwide would not be closing branches in the long-term future and that they had not recently closed branches, we concluded that the ads were misleading."

In the 12 months before the ad campaign, Santander had closed fewer branches than Nationwide, the ASA noted.

A Nationwide spokesman said: "We recognise the ASA's decision and are delighted to have the opportunity to make even clearer our now extended branch promise to keep every branch open until the start of 2028."

Google has agreed to delete billions of records containing personal information collected from millions of people using "Incognito" mode.

The action will be taken as part of a settlement agreement in a lawsuit in the US, but applies outside the country too.

The lawsuit accused Google of tracking Chrome users' internet activity even when they had switched the browser to "Incognito".

Google disputed the claims as "meritless" and said the agreement was only required to "delete old personal technical data that was never associated with an individual and was never used for any form of personalisation".

Shares in Donald Trump's Truth Social platform have plunged more than 20% - wiping out gains made following its stock market debut last week.

The tumble came after Trump Media & Technology Group reported losses of more than $58m (£46m) in 2023.

The company also warned it would struggle to meet its financial liabilities going forward.

Royal Mail has appointed a senior Heathrow Airport executive as the group's new boss.

Emma Gilthorpe will join the postal service on 1 May, before succeeding Martin Seidenberg as chief executive in the summer, said owner International Distributions Services.

Her appointment comes at a critical juncture, with Ofcom considering options that could see Royal Mail cut its six-days-a-week service or extend how long it takes to deliver letters.

Annual shop price inflation almost halved in March to its lowest since December 2021, according to industry figures.

Food costs and competition between retailers drove the down rate from 2.5% in February to 1.3% in March, the British Retail Consortium (BRC)-NielsenIQ Shop Price Index found.

Food prices were 3.7% higher than a year ago, down from 5% in February, the slowest rate since April 2022.

Fresh food inflation slowed to 2.6% from 3.4% a month earlier.

Inflation on products other than food fell to 0.2% in March, down from 1.3%.

Promotional offers were to thank in part, with supermarkets trying to draw in customers for Mother's Day and Easter, said Mike Watkins, head of retailer and business insight at NielsenIQ.

Deals were also being offered on electricals, clothing and footwear, said BRC chief executive Helen Dickinson, adding that dairy prices were falling too.
