In 2019, there were an estimated 24 million cruise passengers on the world’s seas and oceans. The industry is expected to continue growing as more travelers take to the seas for their holidays.

This growth in popularity has also had an effect on the hiring practices of cruise lines. Many are now looking for more medical personnel to provide medical care and support to their passengers. This includes doctors, nurses, medical technicians, and other health professionals.

How Has the COVID-19 Pandemic Hit the Cruise Ship Industry?

Prior to the pandemic, the cruise ship industry was booming, with an estimated 30 million passengers worldwide taking cruises each year. The sector employed some 1.2 million workers and generated revenues of around $150 billion annually.

However, the spread of COVID-19 and the subsequent lockdown of many countries have hugely impacted the industry. In April 2020, nearly all global cruise ships were forced to suspend their operations due to port closures, quarantine restrictions, and travel bans imposed by governments. 

This led to a massive reduction in revenue and employment, with an estimated 600,000 people losing their jobs and $50 billion in losses for the cruise line operators.

But a newly released report from Cruise Lines International (CLIN) paints an optimistic picture of its recovery heading into 2023. The past year was determined to be a “pivotal transition year” for the industry, with “full recovery projected in 2023.”

They expect nearly all of their cruise ships to be operating fully by this summer, and, at this time, approximately 60% of cruise-related businesses and organizations are hiring, which is great news for doctors looking for a career change.

They Do Hire Doctors

As the cruise ship industry continues to expand, the need for qualified medical staff is growing. Cruise ships need doctors on board to take care of the needs of their passengers and crew. 

Cruise ships with more than 100 passengers onboard are required to have medical staff onboard. These staff members need to meet the guidelines set out by the American College of Emergency Physicians (ACEP).

Doctors must be certified by the U.S. Coast Guard to practice medicine onboard, and they must have experience in emergency medicine, general medicine, and other specialties. The qualifications to become a doctor on a cruise ship vary by company but generally include a degree in medicine and a valid state license.

The duties of a doctor on a cruise ship include performing checkups and treating illnesses, injuries, and medical emergencies as they arise. In addition, they may also provide general medical advice and preventive health care measures. A cruise ship doctor may also oversee other medical staff, such as nurses, EMTs, and paramedics.

Cruise ship doctors are classified as emergency physicians. They are trained in emergency care but also need to be qualified in family and internal medicine. 

The types of doctors typically employed onboard cruise ships are general practitioners, obstetricians/gynecologists, pediatricians, dermatologists, and dentists. Additionally, some cruise lines may also hire psychologists and psychiatrists.

Medical care must be available 24/7; typically, a cruise ship will have two doctors onboard — a senior doctor and a junior doctor or nurse. The larger the ship, the more medical staff is required.

Senior doctors on a cruise ship are considered “officers,” meaning they will need to attend any galas and other officer-related events unless they care for a patient who needs their help.

Life as a Cruise Ship Doctor

A doctor’s work schedule on a cruise ship typically consists of a minimum of three months at sea followed by two weeks of vacation back home. During their time at sea, doctors typically work eight-hour days, with additional hours expected during peak times, such as when the vessel is docked or in port.

However, like any job, there are some drawbacks. One of these is the long hours. 

Doctors on cruise ships may work 12-14 hour days and be on-call 7 days a week, with no days off during the entire duration of their contracts. This means that there is little time for leisure activities or even rest, as doctors must remain available in case of an emergency or sudden illness. 

As a doctor on a cruise ship, your responsibilities will include:

• Providing medical care to passengers and crew members
• Performing routine medical checks on passengers and crew before they embark on the vessel
• Monitoring passengers and crew for signs of illness or injury
• Prescribing medications and treatments as needed
• Maintaining accurate medical records
• Reporting any illnesses or injuries to the captain and medical staff onboard

A typical day for a doctor onboard a cruise ship begins with a quick check-in at the medical center. This is followed by rounds on the deck to assess any new cases that may have come up overnight. 

After this, the doctor will conduct physical exams and tests on any patients who require it. Throughout the day, the doctor will also keep track of any ongoing illnesses or injuries and provide counsel and advice to passengers or crew who may need it. 

In addition to this, the doctor may also have to participate in meetings with other medical staff and give lectures or seminars on health-related topics. At the end of the day, the doctor will close out their shift and prepare for the next day’s duties.

Despite the long hours, the pay is generally good and allows doctors to save money and plan ahead financially.

Salaries, Benefits, and Perks

According to Cruise Mummy , senior doctors on a cruise ship can earn between $10,000-$15,000 each month, while junior doctors can expect to make between $7,500-$10,000 each month. Contracts usually last around four months, with two contracts “permitted” each year. In total, doctors on cruise ships work a maximum of eight months and have four months of vacation time over the course of the same year.

Very few, if any, cruise ships are single companies. Most are part of a larger parent or umbrella company that has multiple cruise lines within its stable of cruise ships.

Ranked by passengers and revenue market share , the five giant cruise companies with their annual U.S. dollar revenue are:

• $5,054.21 Royal Caribbean Cruises Ltd.
• $2,996.28 Norwegian Cruise Line Holdings Ltd.
• $1,539.65 Mediterranean Shipping Company
• $1,318.47 TUI Group (50%) and Royal Caribbean Group (50%)

That’s good news for doctors wanting to work on a cruise ship. Rather than apply to work on a single ship — the acceptance of your application will allow you to work on any of the parent company’s ships. 

While varying sources differ on which cruise line is the best to work for, Royal Caribbean International comes out on top of several lists. It’s owned by Royal Caribbean Group, which also owns five other cruise lines.

The Royal Caribbean International fleet currently has 27 ships and sails to six of the seven continents.

Cunard Line is one of the oldest cruise lines, founded in 1839, and is currently part of Carnival Corporation. They have a small fleet of three ships (a fourth ship is currently under construction) and are famous for transatlantic crossings.

Doctors working on this cruise line are offered a permanent contract with 12 weeks on/6 weeks off rotation pattern. They are unique in the cruise line world as they operate as a GMC-designated body, which lets you revalidate your medical license and maintain your registration.

Contracts are slightly longer, ranging between five and seven months, with a two-month break between contracts. They offer an online self-assessment test to see if an onboard life is right for you.

Disney Cruise Line was bought by Carnival Corporation in 1991 for $372 million. This is the fourth cruise line bought by Carnival (the others are Holland America Lines and Windstar Sale Cruises).

Disney has five ships in its line: Disney Magic, Disney Wonder, Disney Dream, Disney Fantasy, and Disney Wish. They value customer service and encourage staff in their career development and advancement.

While most cruise ships are aimed at rest and relaxation, AIDA Cruises stands apart by targeting younger, more active vacationers. The cruise line launched in Germany in the early 1960s and, since 2003, has been owned and operated by Carnival Corporation.

Fourteen ships sail under the AIDA Cruises name, and the line is consistently named one of Germany’s best employers, most recently recognized by the Top Employers Institute in 2022.

• Valid accredited ALS/ACLS certificate.
• PALS certification before completing the first year of employment.

They stayed for three months onboard, the family had crew and guest privileges, and their kids frequented the cruise ship kids club, so it’s definitely possible!

The industrial standard of salaries for junior doctors working on cruisers can be anything from $9,000 to $12,000 per month. For senior physicians, it can start at $12,000 and go as high as $14,000 monthly.

Not all cruise lines offer the same salary for ship doctors, but you can expect anything from $8,000 up to $14,000. Some doctors will stay with a specific cruise line for many years to enjoy the benefits of the job.

This will include free food and an officer’s single cabin as accommodation, and they get to see many places around the world. 

The average benefits for cruise ship doctors are usually the same as most cruise lines, including free vacation travel. Junior doctors earn as low as $7,500 per month at some cruise lines, while senior doctors earn $12,000 and more.

Contracts for working on some cruise lines are typically four and two months of vacation, but not all are the same.

What Cruise Lines Are the Best To Work For

One of the very popular cruise lines to work for is the Royal Caribbean, as it has very competitive salaries and a rewarding work environment. Working with this cruise line is great if you want to see many different places when you are working with them.

They also have a variety of ship sizes available, and if you are on a larger ship, your life will be full and fast-paced. Another cruise line is Princess Cruises, which also offers a lot of jobs for the medical profession and especially for doctors.

Most cruise lines provide very close or slightly higher salaries than the industry standard for doctors working on cruise ships. You will not get a lot of leave per year, which in some cases is only two weeks per year while working for them.

Other cruise lines, such as Carnival Cruise Lines and Costa Cruises, occasionally look for medical personnel. Then there are also Norwegian Cruise Lines and Morella Cruise Lines, to name only a few of a very long list of cruise lines.

There are hundreds of cruise lines worldwide, and all of them need doctors on board to look after the passengers. 

Who Can Apply for These Jobs

Anyone qualified as a medical doctor can apply for a ship’s physician’s job on a cruise ship. The requirements are a professional medical degree and at least three years of experience in specific fields of the medical profession.

Other requirements include fluency in the English language and basic and advanced life support certifications.

• Current, unrestricted license to practice medicine with at least three years post-graduation experience
• Majority of experience in one or more of the following areas: Emergency Medicine, Family Medicine, Critical / Intensive Care, Cardiology, Anesthetics, Remote Medicine. Additional qualifications such as Diploma in Primary Emergency Care, Diploma in Anesthetics or FCEM are preferred
• Additional skills such as Ultrasound are advisable and certificates such as Advanced Trauma and Pediatric Life Support are also required by some cruise lines

In addition, applicants for cruise line physician jobs MUST have:

• Fluency in English – all candidates for the cruise ship doctor position must be able to communicate fluently in English as a medical professional

In-patient care encompasses the management of a wide variety of clinical problems including cardiac diseases (thrombolysis and cardiac monitoring), trauma/orthopedic problems and acute medical and surgical emergencies.

What to do if you want to be a cruise ship doctor

In 2013 I began searching for ways I could change my career to reduce my workload, but not give up medicine altogether. During that time I took a cruise and looked at various jobs I could do on a cruise ship. One of the jobs I was qualified for, I thought, was to be a Cruise Ship Doctor.

After talking with the ship’s doctor to find out what it was like to be the doctor on a ship, I realized that I would enjoy that life. When I arrived back home I sent in an application to a cruise line. I was promptly informed that general surgeons were not qualified to be a ship’s doctor. They only accept physicians who practice emergency medicine, family practice, or internal medicine. So a ship doctor was out for me. But you might be qualified and enjoy the change of pace, so here is what I learned about being a cruise ship doctor.

First of all, you must realize that this is not a well-paying job for an American doctor. The exact income of a cruise ship doctor wasn’t revealed, but one hour in a well-paid ED would be in the ballpark of one day’s pay on a cruise ship. Pay is not the reason to take this job, it’s the great fringe benefits.

Ship doctors are given a private room, usually with a window, but not a balcony. They can bring their spouse and children at no charge. That may be a benefit your spouse would love, especially if your kids are out of the nest. Imagine the two of you being paid to sail around the world with unlimited free food and entertainment.

The working hours are pretty good with this gig. Each ship has two doctors and at least two nurses. The doctors take turns carrying the beeper for 24 hours at a time. The senior ranking doctor is in charge of the passengers and the junior ranking doctor cares for the crew. When the ship is at sea, office hours usually consist of three hours in the morning and three hours in the afternoon. When the ship is in port, office hours are two hours early in the morning and two hours later in the afternoon or evening.

On non-call days, the doctor and their spouse can go ashore and enjoy the town in between office hours. What a great fringe benefit.

You will usually see 10 to 20 patients per day, mostly with minor problems. The top five most common issues seen (based on my informal poll of doctors I’ve talked to on ships) are; MI, stroke, fractures, lacerations and GI issues (not infectious). These are pretty typical problems faced by the elderly population that frequent these ships. I, at age 55, am younger than almost every paying passenger on the ship. There are some cruises though that cater to a younger crowd, like Disney for example. On those cruises, some pediatric problems might be seen.

Ships have a mini hospital on board. They are required to have at least one hospital bed per 1,000 passengers. Most ships have an ICU bed and a few regular beds. They keep the usual assortment of medications, simple X-ray services, minor surgical equipment, lumbar puncture tray, EKG machine and the basic equipment found in a medical ICU, including the ability to intubate. When a ship is at sea, the ship needs to carry everything needed to get the patient through their problem until the ship arrives at the next port. At that time, the patient is evaluated to determine if the patient’s problem can be treated well on the ship, or if the patient needs to be off-loaded to a local hospital.

Sending a patient to a local hospital is not easy and many patients don’t want to leave the ship for medical treatment. First of all, the local hospital may not have an English-speaking doctor. There are also insurance issues. Most regular health insurance policies don’t cover patients traveling abroad, so cruising passengers need to purchase travelers insurance to cover their medical needs outside their insurance coverage area. The spouse of the patient also has a dilemma. Do they continue the cruise or get off with the patient in a strange country. Most disembark with their spouse when transferred to a local hospital. Leaving the ship is very challenging, especially for elderly couples, or those traveling alone, thus, some patients prefer to remain on the ship and have the ship doctor take care of them.

Malpractice insurance is usually covered by the hiring company, the same as a locum tenens doctor. The possibility of a lawsuit is rare since cruise passengers understand that onboard doctors have limited resources and are only able to treat minor ailments and temporize major cases until they can be transferred to a bigger facility. Most shipboard issues are minor or acute. Missing a breast cancer diagnosis, for example, is not among a ship doctor’s jurisdiction. A big risk, for a ship doctor, might be missing a fracture, which doesn’t tend to have long-term consequences. The patient may get a free cruise for their inconvenience, but the doctor is unlikely to be the victim of a lawsuit.

Physicians are usually scheduled on a ship for increments of four months on and two months off. They will likely be rotated from ship to ship in the same cruise line. That sure seems like a nice schedule to me. Work while I travel all over the world with my spouse. Then have two months off to go home and do some CME and visit friends and family. Where do I sign up? (Oh yeah, I already tried.)

There is one thing I heard from a cruise ship doctor that all doctors should know. If a patient of yours asks you to clear them to go on a cruise, please look at the cruise itinerary before giving your OK. Some cruises travel to a different port every day, each with a hospital. Some cruise itineraries take passengers across the ocean and may have multiple days in a row at sea with no way to get to advanced medical or surgical care. If the itinerary they have chosen doesn’t have adequate medical availability for their condition, please advise them to adjust their travel plans to take into consideration their medical needs.

You will likely be asked to send them your CV online. They will then interview you via Skype, during which you will be asked questions about how you would handle different clinical situations. It’s similar to taking an oral board exam. Upon passing the interview, have a wonderful time getting paid while traveling the world.

Do Cruise Ships Have Doctors and Medical Facilities? Explained

Boarding a cruise ship brings with it the excitement of the sea, but have you ever wondered what happens if someone falls ill onboard? The recent COVID-19 pandemic has starkly highlighted the need for robust healthcare systems in these floating cities.

Take P&O Cruises, for example. They’ve ramped up their health protocols, ensuring their medical centers are equipped and ready 24/7, with qualified staff to handle everything from seasickness to more serious concerns.

In this article, I will guide you through the healthcare services aboard a cruise ship. We’ll dive into the facilities available, the expertise of the medical staff, and how cruising giants have adapted to keep us safe in turbulent times.

Do Cruise Ships Have Doctors?

One of the most common questions I get asked about cruise life is, “Do ships have doctors?” The answer is a resounding yes, and the level of care they provide is more comprehensive than you might expect. The medical standards on cruise ships, particularly those operating from the U.S., are rigorous.

The American College of Emergency Physicians , alongside the Cruise Lines International Association, lays down stringent guidelines for both the medical personnel and the facilities onboard. This ensures a high level of care, similar to what you’d find in many land-based medical facilities.

Doctor Qualifications

The qualifications of cruise ship doctors are particularly impressive. Under the ACEP standards, they must have at least three years of post-graduate experience in fields like general and emergency medicine, or be board-certified in specialties such as advanced life support practices, medical emergencies, family, minor surgical procedures, or internal medicine. They’re also required to be certified in advanced cardiac life support.

Pediatric Care

For ships carrying children under 12, there’s an additional layer of care required. At least one physician is required to have pediatric training or equivalent emergency training involving kids, ensuring that our youngest cruisers are in safe hands in the ship’s medical center.

Beyond the cruise ship doctor, the medical team includes highly trained nurses, often with paramedic experience. All nurses are experienced registered nurses, forming a robust team ready to tackle a range of medical needs.

On-Board Facilities

Many larger cruise ships, like those operated by Princess Cruises, have hospital-like treatment rooms within their onboard medical centers. The size of the medical team varies with the ship’s size – the largest vessels may have two doctors and up to six nurses. Smaller river ships, staying close to shore, may have fewer staff, relying on easy access to land-based facilities in emergencies.

All medical practitioners, including doctors and nurses, are accountable to a chief medical officer stationed at the cruise line’s central office, who is available round the clock to assist in case of emergencies.

On my very first cruise, I remember being pleasantly surprised to discover the extent of medical facilities onboard. The infirmary, usually tucked away on a lower deck, is a hub of medical expertise. While it typically operates during regular hours, as listed in the ship’s daily program, the medical staff is always on call for emergencies, ensuring round-the-clock care.

Walking into the infirmary is much like entering any doctor’s office. There’s a waiting room where you fill out medical history forms before heading to one of the exam rooms . In my experience, the staff is as professional and thorough as any land-based clinic.

It’s only in more serious medical situations that you’re taken to the separate area housing hospital beds , a small surgical suite for minor surgeries, or the ICU and isolation rooms . Thankfully, I’ve never needed these services, but it’s reassuring to know they’re there.

The equipment in these shipboard medical centers is impressive. Onboard, you will typically find cardiac monitors, defibrillators, ventilators, x-ray machines, oxygen tanks, and even laboratory equipment. On some Carnival Cruise Line ships, for instance, there’s advanced technology like thrombolytic therapy for blood clots and pulse oximetry for oxygen level testing.

Carnival Cruise Line ‘s medical facilities have specific hours:

On the day the ship embarks, they’re open from 8 to 9 a.m. and 3 to 5 p.m. On sea days, the facilities are open from 9 a.m. to noon and 3 to 6 p.m., while on port days, their hours are from 8 to 10 a.m. and 4 to 6 p.m. This ensures medical care is accessible to all passengers, regardless of the day’s activities.

Pharmacies On Cruise

When seasickness hit me unexpectedly, I was relieved to find that the ship had its own well-stocked medical facility, equipped to provide immediate relief.

Onboard pharmacies carry a surprising variety of medicines, ensuring passengers have access to essential medications for common health issues. The inventory, which varies by ship, includes everything from antibiotics and seasickness pills (often complimentary) to aspirin and other over-the-counter drugs.

These pharmacies are well-equipped to cater to a diverse array of medical requirements, maintaining a supply of medications for cardiovascular and gastrointestinal ailments, infectious diseases, respiratory conditions, and even vaccines. Whether you’re dealing with a minor ailment or something more significant, there’s a high chance the pharmacy can help.

While the convenience is unmatched, it’s worth noting that the cost of medications on board can be higher than your home pharmacy, but generally comparable to prices you’d find if you purchased them in a port. This includes seasickness medications, which are readily available and can be a lifesaver for those who are unsure if they’ll be affected by motion sickness . This is a small price to pay for the convenience and peace of mind provided by these pharmacies.

Having this resource at hand on a cruise ship reassures passengers that their health and comfort are taken seriously, making for a safer and more enjoyable journey.

Common Medical Issues Handled on Cruise Ships

I’ve seen firsthand how the medical team on cruise ships expertly handles common health concerns. According to the World Health Organization , the most common issues are respiratory and gastrointestinal illnesses , like influenza and norovirus. They also skillfully manage cases of motion sickness and various injuries.

It’s important to understand that a ship’s medical facility is more of an infirmary than a full-fledged hospital. This means while they’re equipped for a range of medical situations, they’re not designed for highly complex medical procedures. The staff’s expertise in quickly addressing these common issues is truly commendable, ensuring that passengers get the care they need to continue enjoying their journey.

Procedures for Handling Emergencies and Serious Medical Conditions.

Cruising is generally a smooth experience, but it’s comforting to know that if a medical emergency arises, the cruise ship doctors and the team on board are well-prepared. This is particularly reassuring for seniors who are on a cruise  and may have concerns about health care while traveling. Most medical issues on ships are routine, yet the team is adept at providing life support services and stabilizing anyone seriously ill.

In cases where a patient needs more intensive care than the ship can provide, medivac procedures come into play. The patient would typically be medically disembarked at the next port and transferred to a hospital on a stretcher.

But, if a critical emergency occurs at sea, the U.S. Coast Guard, or the local equivalent overseas, is ready to assist, even airlifting patients if necessary. It’s a bit sobering, but ships are equipped with morgue facilities too, just in case. This level of preparedness is part of adhering to the American College of Emergency Physicians’ standards.

Cruise lines have clear policies on when passengers should be medically disembarked. The medical team aboard the ship possesses the authority to determine whether a passenger is not fit to continue their journey at sea. For instance, as MSC Cruises outlines, passengers are disembarked if their condition could endanger their health or safety.

These decisions are always made with the passenger’s well-being in mind. It’s important to emphasize that air evacuation is exclusively reserved for the most critical cases. This procedure introduces additional physical and emotional strain, and the medevac units are manned by highly trained medics specializing in emergency treatment.

Case Studies or Examples of Medical Incidents Handled at Sea

Let me share a story from one of my cruises that stuck with me. Everyone was soaking up the sun, some by the pool, others with a book in hand. That’s when something unexpected happened.

Out of nowhere, a fellow passenger, an older gentleman enjoying his time by the pool, suddenly slumped over. It was one of those moments that made your heart skip a beat. But what happened next was truly remarkable.

The medical team, they were like superheroes in disguise. They appeared almost instantly, maneuvering through the crowd with a stretcher and a medical bag. Their efficiency and calmness amidst the sudden rush of worried onlookers were incredible.

They quickly got to work, checking the gentleman’s vital signs, and administering first aid. It was like watching a well-choreographed dance, each move precise and confident. The way they handled the situation, with such professionalism and care, it was reassuring to know that we were in safe hands.

In less than an hour, the gentleman was stabilized and taken to the infirmary for further observation. The whole incident was a powerful reminder of the unexpected turns life can take, even in the middle of the ocean, and the importance of having a competent medical team on board. It’s these behind-the-scenes heroes that make cruising not just enjoyable, but safe for everyone.

How Passengers Can Access Medical Care on Board

Ever wondered how you can get medical help while cruising? It’s simpler than you might think. On a cruise, if you ever need medical attention, here’s how it works.

To begin, it’s common to find the medical center situated on a lower deck of the cruise ship. This is a strategic choice – away from the hustle and bustle of crowded areas and conveniently close to crew facilities. What Happens If You Get Sick on a Cruise Ship?

W hether you prefer family cruises or adults-only cruises , no one wants to get sick while they're on vacation. This is especially true when you're on a ship—after all, it's tricky to cut your trip short if you're floating in the middle of an ocean. Plus, falling ill on a cruise ship can have bigger implications these days. There are considerations related to cruise ships and COVID-19 that are designed to keep everyone aboard safe and healthy.

If you're planning a trip on one of the best cruise lines or one of the most affordable ones, you need to know what to do if you feel under the weather on a cruise ship, whether it's a little sea sickness or a more serious illness or injury. Consider this advice some of the essential cruise tips that all passengers should keep in mind before they board.

Are there still pre-boarding health checks?

Yes. To protect you as well as your fellow passengers and the crew, cruise ship staff screens passengers for health issues prior to boarding. You'll be asked to fill out a form that addresses health questions and illness symptoms like fever, vomiting and diarrhea. You may also have your temperature taken by walking through a thermal-imaging scanner. Checks for certification of COVID-19 vaccination or negative tests have, at least for now, ended (here are other recent changes that might affect your cruise ). A staff member may ask you additional questions, and you might be sent for secondary screening by a doctor. Should they determine that your or anyone else's health would be jeopardized, they may not let you board.

"It's always important to answer any pre-boarding health questionnaires truthfully and to report any signs of illness to the ship's crew as soon as symptoms arise," says Cruise Critic's editor-in-chief, Colleen McDaniel. Don't worry—it doesn't necessarily mean you won't be allowed to board or you'll wind up quarantined. Being honest not only affects your health and the health of everyone on the ship, but it could also affect the care you receive and any compensation either from the cruise line or your travel insurance if something goes wrong. We'll give you more details about insurance below.

What kind of medical facilities are on a cruise ship?

Choosing a cruise line that's a member of the Cruise Lines International Association (CLIA)—the largest trade association for the cruise industry—means you'll know the ship meets or surpasses a minimum standard for health and safety. CLIA requires infirmaries to have an equipped examination room and a room for intensive care. Equipment is required to assess and monitor vital signs, administer medications and IV fluids and process some lab tests like checking blood sugar and electrolyte levels and diagnosing some infections. Other typical onboard equipment includes defibrillators, ventilators, cardiac monitors and stretchers. Some ships even have X-ray capabilities.

CLIA members must follow mandatory guidelines for their oceangoing ships, but many non-CLIA members have great medical facilities too. Check any cruise line's website before you book. The number of medical staff on board depends on the size of the ship. Larger ships usually have more than one doctor, as well as a combination of paramedics and nurses. All crew, regardless of their jobs, need to be trained in safety and first aid too.

What happens if you get sick on a cruise ship?

The medical staff on cruise ships can diagnose and treat some illnesses on board and prescribe medications too. While cruise ships typically stock a broad range of meds, including antibiotics, there's not always a large supply. Doctors may give you a prescription that you can fill at a pharmacy in the next port.

If medical staff are concerned about your condition, they may advise—and even require—you to seek the advice of specialists on shore, says William Spangler, MD, global medical director with AIG Travel. Port agents will often arrange a taxi or even an ambulance to the treatment center for you. In the best-case scenario, your appointment will be quick, your issue resolved or aided by the specialist, and you can hop back on board to your cruise cabin to heal while watching the scenery go by.

But that's not always a guarantee. "It's really up to the physician as to how comfortable they feel with keeping you aboard, versus disembarking you to make your way home or to a nearby facility where you can be definitively cared for," says Dr. Spangler. Sometimes it's the ship's itinerary that poses the problem. For example, if the ship has a number of days at sea before it reaches the next port, the ship's doctor and captain might be worried that you couldn't get the help you need if your condition worsens and could deny you continuing with your cruise.

Here are some of the more common illnesses and how they are typically handled onboard.

If you get seasick

Though ships are equipped with stabilizers to maximize comfort, seasickness can happen to even those with the strongest of stomachs in rough waters. Many cruise ships offer seasickness medication to passengers—sometimes it's free of charge and you can take pills from a basket outside the infirmary or at the purser's desk. Medication is also available at the gift shop.

However, it's wise to make a cruise packing list and travel with the medications or remedies that you know work for you, like Dramamine, Gravol or a motion sickness patch. Many people rely on sea sickness bracelets that activate a pressure point on the wrist. Ships' doctors can prescribe stronger medications if needed.

If you have a cold or other mild illness

Before COVID-19, colds and other minor respiratory illnesses were not taken as seriously as they are now. If you have any respiratory symptoms on a cruise, you may be asked to take a COVID test—likely on multiple days and with swabs of both the mouth and nostrils—to rule it out. If COVID-19 is ruled out and the ship's doctor thinks you likely do have just a cold , no one else on board wants to get a cold or other mild illness either. So be extra diligent about washing your hands often, and wear a KN/N95-type mask to keep your germs from spreading.

The ship's medical center will likely give you the same advice as you would get at home—drink lots of (nonalcoholic) fluids and rest. If the doctor suspects your symptoms are caused by bacteria rather than a virus, they may prescribe an antibiotic.

If you have COVID-19

If you come down with COVID-19 while you're on your cruise, you'll likely be quarantined in your cabin. It's important to prevent the spread, because it also affects the blood vessels, and one in 10 infections is causing long-term problems to the heart, circulatory system and brain. If you're quarantined, says Dr. Spangler, "they bring you your meals and they check on you once or twice a day accompanied by one of the ship's nurses." There's likely medication on board that will help ease some of your symptoms, but don't assume that the ship (or the country you're traveling in) will be able to provide antivirals like Paxlovid, even by prescription.

Dr. Spangler also notes that patients may be treated differently. He says a healthy, young person with COVID-19 would likely be quarantined in their cabin, but "if you're 75 years old, with a condition like COPD, hypertension or diabetes, you will likely be disembarked." Cruise ships rarely take chances with passengers that are very ill—or somewhat ill with the likelihood of getting worse.

Your key card will likely be deactivated while you're under quarantine. If you disobey the doctor's orders and leave your cabin anyway, you could be removed from the ship, and the cruise line may ban you from ever traveling with them again. Everything is "at the discretion of the ship's doctor," says Dr. Spangler, "and policies vary on different ships." Some ships will quarantine you for five days, and others will while you're showing symptoms, plus for another 72 hours beyond that to ensure you're symptom-free. Often you'll need to test negative on two antigen tests taken on subsequent days before your quarantine ends.

If you have norovirus

If you haven't heard of this one before, it's also known as viral gastroenteritis, which is highly contagious and presents with symptoms we commonly associate with food poisoning . It's not the stomach flu, though many people use this term to describe it. The CDC estimates there are about 20 million cases of norovirus in the United States annually. It can occur anywhere, and because it is very contagious, it spreads particularly easily when people are in close quarters—like on an all-inclusive cruise . You might hear about norovirus on cruise ships, because cruise lines are required to track outbreaks of gastrointestinal disease, though hotels and restaurants are not.

As with COVID-19, you might be quarantined in your cabin if the ship's doctor suspects you have a contagious disease like norovirus. The doctor may prescribe medications to prevent vomiting and diarrhea and give you advice to drink lots of fluids and rest as much as you can, but there is no treatment for norovirus, and it usually clears up after a few days.

When there are suspected cases of viral gastroenteritis onboard, many cruise ships change their operating procedures. This includes more frequent cleaning of high-touch items like door handles and railings, insisting that passengers sanitize their hands when entering dining rooms, and changing buffets so that it's only staff who touch the serving utensils. You might want to pop on a KN95-type mask too, as there's increasing evidence that norovirus is airborne.

If you have a medical emergency

If you have a serious problem like a heart attack or a stroke or need surgery, the ship's crew will do their best to treat and stabilize you until you can get to a facility at the nearest port that has the resources you need. Someone who has a heart attack or a stroke might be initially treated on board with a defibrillator or thrombolytic medications (which can help break up a blood clot). However, cruise ships' infirmaries are not hospitals, and they are not equipped to treat every health problem. Should the ship's doctor decide that you need care at an onshore hospital, they'll keep you as safe and comfortable as possible until you get there.

Sometimes that means being transferred by boat or helicopter to shore, even diverting the ship from its planned itinerary to reach shore faster. When you're too sick to be treated onboard, Dr. Spangler says, "the ship will disembark you at the next port, and it may not be anywhere near a major metropolis with highly rated medical facilities." It's a wise idea to have a travel insurance plan with robust medical evacuation coverage, since you must follow their orders if the ship's doctor decides you must be medically disembarked.

What happens if you get hurt on a cruise ship?

Dr. Spangler says that really depends on the size of the cruise ship as well as the type of injury. A cut needing a few stitches can usually be addressed on board, as can minor burns. And for something like a minor fracture or a broken arm, "they might provide a splint and pain medication to get you through the end of the cruise," says Dr. Spangler.

More serious bone breaks—like when the bone protrudes through the skin—will be handled differently, according to Dr. Spangler. "Obviously, with an open fracture, you're going to be disembarked after first aid is administered."

How much does it cost to see the doctor on a cruise ship?

Doctors on cruise ships are independent contractors and set their own rates. You can expect a basic visit to cost at least $100. Tests, medications and the use of equipment, like X-rays, will be charged separately.

If you do get sick or injured aboard a cruise ship, any medical costs will be added to your shipboard account. You'll then need to submit receipts to your insurance company to determine what will be reimbursed.

Will your regular medical insurance cover these expenses?

Your everyday medical insurance is unlikely to cover any medical expenses aboard a cruise ship, even if that ship is in U.S. waters. That means it's important to have good travel health insurance. It's a small price to pay for peace of mind and, if you do get sick or injured, coverage of potentially hundreds of thousands of dollars of medical expenses.

Prior to your trip, check your existing insurance to see what it covers and determine any additional insurance you need to purchase. Both third parties and cruise lines offer insurance, but insurance purchased from the cruise line may not be as comprehensive, and reimbursement might be in the form of credit for a future cruise rather than a cash refund. Every insurance policy is different; it pays to read the fine print.

Your insurance may or may not cover a full or partial refund if, for example, you're denied boarding, quarantined in your cabin or have to leave the cruise partway through. That depends both on the reason for not being able to continue with your cruise and the type of insurance you've purchased.

A good travel health insurance company can help you find the closest medical facility that's best suited to treat your condition and, assuming it's not nearby, help you get there, even if it requires expensive transportation. If your condition is very serious, many travel health insurance policies will help a family member get to you or arrange a medical evacuation for you to get home. If you had to pay for these services out of pocket, Dr. Spangler says the price tag could range "from $20,000 easily into the six figures."

How can you stay healthy on a cruise?

Cruise lines' crew work hard to keep everything clean to minimize the spread of communicable diseases, but it's up to you to be cautious as well.

It may bring you peace of mind to know that many cruise lines updated their HVAC and ventilation systems in the first few years of the COVID pandemic. This helps with not just the spread of COVID-19, but also colds, flus and even gastrointestinal diseases. You can also hang out on the deck of your cruise ship rather than indoors whenever possible.

Washing your hands frequently is also important to prevent illnesses. Sanitizer is better than nothing, but washing with soap and water does a better job—though here are the ways you might be washing your hands wrong . Aboard many cruise ships, you'll be asked to sanitize your hands when you enter the dining room. Washing your hands after you leave the restaurant or bar minimizes the spread of your germs too. That keeps everyone, including you, much safer.

Another stay-healthy tip is to try to touch fewer things. No one is suggesting not using the handrail on a staircase, but get in the habit of pushing the elevator button with your elbow and not putting your hands on the bar when you order a drink. And just FYI, if you're sharing these things that germ experts wouldn't share , you're putting yourself at an increased risk of getting sick.

Whenever you travel, you should bring a first-aid kit packed with these essentials  so you have key products on hand whenever you need them. For a cruise, add seasickness medication (whether in tablet form like Bonine and Dramamine, or motion sickness patches). Many people find that anti-nausea acupressure wristbands work for them.

Wes Moore: no update on search and rescue efforts, vows to deploy all resources

Maryland governor Wes Moore said that there are no new updates on the search and rescue effort.

Moore added that rescuers have deployed air, land, and sea methods to help search for survivors.

“There is not a single resource that we will hold off on deploying,” Moore said during the press conference.

“This is very much still a search and rescue mission,” he added.

Six people remain missing in the wake of the bridge collapse, as state and national officials said a search and rescue mission by land, air and sea remained the top priority, and offered prayers and support to the families of the missing.

Local news outlets reported that eight construction workers repairing potholes on the bridge were believed to have fallen into the water, and only two have been rescued so far. One person who was hospitalized following the collapse has been released from the hospital.

The missing are “hard-working, humble men” from Guatemala, Honduras, El Salvador and Mexico. They are in their 30s and 40s and have spouses and children, another construction worker told the Baltimore Banner . Doctors from a local hospital trauma center said that it was unlikely that anyone could survive for multiple hours submerged in the harbor’s cold water.

Joe Biden called the bridge collapse “a terrible accident” , pledged financial and logistical assistance to rebuild the bridge and highlighted the importance of Baltimore’s port to the local and national economies.

The bridge collapse is expected to have a major and sustained impact on supply chains because of the importance of Baltimore’s harbor to international shipping routes.

The National Transportation Safety Board will investigate the bridge collapse.

Maryland governor Wes Moore said quick action by officials as ship headed towards a collision with the bridge stopped additional vehicles from driving onto it, saving “innumerable” lives.

Wes Moore: quick action to stop additional cars going over bridge saved lives

Asked by reporters if there had been any vehicles on the bridge when it collapsed, Maryland governor Wes Moore said that was still under investigation.

But he said that “the quick work” of public and law enforcement officials to keep additional vehicles from driving onto the bridge as the collision unfolded “undoubtedly saved innumerable lives”.

There will be ‘major and protracted impact to supply chains ’, says Buttigieg.

At this afternoon’s press conference, transportation secretary Pete Buttigieg told reporters that the bridge collapse would have a national impact, because of the amount of cargo that comes through Baltimore’s port.

“There is no question that this will be a major and protracted impact to supply chains,” Buttigieg said. “It’s too soon to offer estimates on what it will take to clear the channel and reopen the port.”

Pete Buttigieg, the secretary of transportation, called Key Bridge one of the “cathedrals of American infrastructure” , during remarks at a Tuesday afternoon press conference.

“This is no ordinary bridge. This is one of the cathedrals of American infrastructure … so the path to normalcy will not be easy. It will not be quick. It will not be inexpensive, but we will rebuild together.”

Buttigieg added that government officials will need to “render all of the support that is needed at the federal level”.

“The federal government will provide all of the support that [is needed] for as long as it takes,” he added.

A body has reportedly been recovered from the water as search and rescue efforts are ongoing following the Key Bridge collapse , CNN reported.

Phylicia Porter, a Baltimore city councilmember, told the network that she had heard a body was pulled from the water.

“We are hearing one body was found,” she said.

The US Coast Guard, which has been leading search efforts, has not confirmed that report.

Men missing are 'hard-working, humble', says man who reportedly knows them

Six people are still missing after the Key Bridge collapse as search and rescue efforts are ongoing.

Jesus Campos, a construction worker who reportedly knows those working on the bridge, told the Baltimore Banner that they are all men from Guatemala, Honduras, El Salvador and Mexico.

Campos added that they are all in their 30s and 40s and have spouses and children.

“They are hard-working, humble men,” Campos said.

Homendy added that investigators are still working to verify if the ship lost power shortly before crashing with the Key Bridge.

On those reports, Homendy said:

We’ve heard the reports. We have been made aware of those same reports about there being a power outage. I’ve also seen statements, media releases from Singapore as well. It’s something that we take in, but something that we have to verify through our investigation that that was what was part of the contributing cause here. So too early to tell.

The NTSB will be “standing back” for the time being to allow the coast guard to continue its search and rescue.

“We chose not to board the vessel today to allow some time for the search and recovery which we did not want to interfere with,” Homendy said.

“That is first and foremost,” she said.

Homendy added that the NTSB has gathered some additional information on the accident that needs to be verified first before publication.

NTSB will investigate bridge collapse

The National Transportation Safety Board (NTSB) chair, Jennifer Homendy, confirmed that the agency will investigate the Key Bridge collapse , she said during a press conference.

The agency arrived to the scene at 6.00am on Tuesday to investigate the crash.

The US Coast Guard is also assisting with the investigation, calling the relationship between the two agencies “cooperative”.

“I want to extend our deepest sympathies to those who have been affect by today’s events,” Homendy added.

Homendy declined to comment on any potential fatalities associated with the bridge collapse, but noted that a search and rescue is still under way.

Bridge height is 'potentially lethal problem' among other concerns, says doctor

Doctors with the the R Adams Cowley Shock Trauma Center are speaking on possible injuries that victims of the Key Bridge collapse are facing.

The R Adams Cowley Shock Trauma Center, which is apart of the University of Maryland medical system, received one patient who was injured. That person was released this afternoon.

While doctors did not speak on that specific patient’s injuries, they said that patients from the bridge collapse could face injuries from falling off the bridge.

“This is a very high bridge … the height alone [poses] a potentially lethal problem,” Dr David T Efron said during a press briefing.

Being trapped beneath metal or concrete could lead to a crushing injury.

Doctors added that patients also face the possibility of hypothermia. “[It] doesn’t take very long for someone to feel the effects of [the water’s] temperature,” Efron added.

On hypothermia, Efron added that being submerged for more than hour is “almost invariably not going to be survivable”.

The White House will also be monitoring how the Key Bridge collapse will disrupt domestic and global supply chains further , Reuters reported.

Officials have already warned that the collapsed bridge could have a significant impact on shipping out of the east coast.

The global supply chain has already faced major disruptions amid two global wars – one in Ukraine and in Gaza. The climate crisis and rising interest rates have also made shipping more expensive, costs that ultimately fall to consumers.

“This comes at a particularly difficult time,” said Bindiya Vakil, chief executive of supply chain management company Resilinc to the Washington Post . “With interest rates high, companies are looking to keep products moving.”
