Child and Adolescent Well-Care Visits (W30, WCV)

Well-Child Visits in the First 30 Months of Life : Assesses children who turned 15 months old during the measurement year and had at least six well-child visits with a primary care physician during their first 15 months of life. Assesses children who turned 30 months old during the measurement year and had at least two well-child visits with a primary care physician in the last 15 months.

Child and Adolescent Well-Care Visits: Assesses members 3–21 years of age who received one or more well-care visit with a primary care practitioner or an OB/GYN practitioner during the measurement year.

Why It Matters?

Assessing physical, emotional and social development is important at every stage of life, particularly with children and adolescents. 1 Well-care visits provide an opportunity for providers to influence health and development and they are a critical opportunity for screening and counseling. 2

Results – National Averages

Well child visits in the first 15 months, well child visits in the first 30 months of life (15 months – 30 months), well-child visits (ages 3-6 years): 1 or more well-child visits, child and adolescent well-care visits (total):.

What is a well-care visit.

It's important to take your child to the healthcare provider when your child is ill. Or when your child needs an exam to take part in a sport. But routine well-care visits are also advised.

Well-care, well-baby, or well-child visits are routine visits to your child's healthcare provider for the following:

Physical exam

Mental health screening

Immunization updates

Tracking growth and development

Finding any problems before they become serious

Providing information on health and safety issues

Providing information on nutrition and physical fitness

Providing information on how to manage emergencies and illnesses

Your child's healthcare provider can also provide guidance on other issues, such as:

Puberty and concerns about the teen years

When should well-care visits be scheduled?

Your child's healthcare provider will give you a schedule of ages when a well-care visit is suggested. The American Academy of Pediatrics advises well-care visits at the following ages:

Before a newborn is discharged from the hospital, or at 48 to 72 hours of age

3 to 5 days

2 to 4 weeks

Each year, from 3 to 21 years

Missed Opportunities for Adolescent Immunizations at Well-Care Visits During the COVID-19 Pandemic

The Coronavirus Disease 2019 pandemic disrupted healthcare, but the impact on vaccination missed opportunities (MOs, vaccine-eligible visits without vaccination) is unknown. We evaluated pandemic-related trends in MOs at adolescent well-care visits for three vaccines: human papillomavirus; quadrivalent meningococcal conjugate; and tetanus, diphtheria, and acellular pertussis (Tdap). We analyzed electronic health record data from 24 pediatric primary care practices in 13 states from 1/1/2018 to 12/31/2021. Segmented logistic regression estimated risk differences for MOs during the pandemic relative to prepandemic trends. Among 106,605 well-care visits, we observed decreases in MOs prepandemic followed by an increase in MOs during the pandemic for all three vaccines. Relative to prepandemic, MOs increased for human papillomavirus (+15.9%, 95% confidence interval [CI]: 11.7%, 20.1%), meningococcal conjugate (+9.4%, 95% CI: 5.2%, 13.7%), and tetanus, diphtheria, and acellular pertussis (Tdap) (+ 8.2%, 95% CI: 4.3%, 12.1%). Increases in vaccine MOs during the pandemic equaled or exceeded pre-pandemic decreases. Reducing MOs in adolescent well-care could raise vaccine coverage.

Measure Description

The percentage of members 3–21 years of age who had at least one comprehensive well-care visit with a PCP or an OB/GYN practitioner during the measurement year.

Numerator Compliance

One or more well-care visits during the measurement year. Either of the following meet criteria:

  • A well-care visit.
  • An encounter for well-care.

The well-care visit must occur with a PCP or an OB/GYN practitioner, but the practitioner does not have to be the practitioner assigned to the member.

Preventive services may be rendered on visits other than well-child visits.

Visits to school-based clinics with practitioners whom the organization would consider PCPs may be counted if documentation that a well-care exam occurred is available in the medical record or administrative system in the time frame specified by the measure.

Services that occur over multiple visits, as long as all services occur in the time frame specified by the measure, may be counted.

NOTE: Services rendered during an inpatient or ED visit or services that are specific to the assessment or treatment of an acute or chronic condition do not count toward the measure.

Best Practices

  • Take advantage of every visit, including sick visits, to capture the components of this measure
  • Schedule visits within the recommended time frames
  • Use standardized templates in charts and EMRs
  • Set care gap “alerts” in your electronic medical record
  • Consider the parent’s work schedule as a barrier to the visit, and offer extended evening or weekend hours
  • Encourage parents/patients to maintain the relationship with a PCP to promote consistent and coordinated health care
  • Educate parents/patients on the importance of having preventive care visits
  • Consider offering extended practice hours to increase care access
  • Remind patients of their appointment by making calls or sending texts
  • Make outreach calls and/or send letters to advise members of the need for a visit
  • Partner with Horizon Healthy Journey Program to assist with targeted outreach activities

It's important to take your child to the healthcare provider when your child is ill. Or when your child needs an exam to take part in a sport. But routine well-care visits are also advised.

Well-care, well-baby, or well-child visits are routine visits to your child's healthcare provider for the following:

Physical exam

Mental health screening

Immunization updates

Tracking growth and development

Providing information on health and safety issues

The article presents an analysis of the work of the largest children's COVID-19 center in Moscow, organized on the basis of the Children's City Clinical Hospital named after Z. A. Bashlyaeva of the Moscow City Health Department. From March to November 2020 at the COVID-19 Center were hospitalized 2,837 patients with suspected/confirmed diagnosis of COVID-19, in total in 2020 1,876 children with a confirmed diagnosis of COVID-19 were treated, 58 (3%) children were in serious condition in the intensive care unit, of which children 11-18 years old were 25%. At the 2020 neonatal COVID-19 center, 215 newborns were observed with suspected COVID-19 diagnosis. The diagnosis of COVID-19 was confirmed in 18 children, while 8 newborns came from the home of COVID-19. In the Center for rehabilitation, where children aged 0 to 3 years old who were born with very low and extremely low body weight are observed, dispensary observation for children who have undergone COVID-19 is organized. 45 children who were observed fell ill with the new coronavirus infection. There were no deaths among children with COVID-19.

Emergency medical care is provided free to all foreign nationals in case of life-threatening conditions that require immediate medical treatment. You will be given first aid and emergency surgery when necessary in all public health care facilities. Any further treatment will be free only to people with a Compulsory Medical Insurance, or you will need to pay for medical services. Public health care is provided in federal and local care facilities. These include 1. Urban polyclinics with specialists in different areas that offer general medical care. 2. Ambulatory and hospitals that provide a full range of services, including emergency care. 3. Emergency stations opened 24 hours a day, can be visited in a case of a non-life-threatening injury. It is often hard to find English-speaking staff in state facilities, except the largest city hospitals, so you will need a Russian-speaking interpreter to accompany your visit to a free doctor or hospital. If medical assistance is required, the insurance company should be contacted before visiting a medical facility for treatment, except emergency cases. Make sure that you have enough money to pay any necessary fees that may be charged.

Travelers need to arrange private travel insurance before the journey. You would need the insurance when applying for the Russian visa. If you arrange the insurance outside Russia, it is important to make sure the insurer is licensed in Russia. Only licensed companies may be accepted under Russian law. Holders of a temporary residence permit or permanent residence permit (valid for three and five years respectively) should apply for «Compulsory Medical Policy». It covers state healthcare only. An employer usually deals with this. The issued health card is shown whenever medical attention is required. Compulsory Medical Policyholders can get basic health care, such as emergencies, consultations with doctors, necessary scans and tests free. For more complex healthcare every person (both Russian and foreign nationals) must pay extra, or take out additional medical insurance. Clearly, you will have to be prepared to wait in a queue to see a specialist in a public health care facility (Compulsory Medical Policyholders can set an appointment using EMIAS site or ATM). In case you are a UK citizen, free, limited medical treatment in state hospitals will be provided as a part of a reciprocal agreement between Russia and UK.

Pharmacies can be found in many places around the city, many of them work 24 hours a day. Pharmaceutical kiosks operate in almost every big supermarket. However, only few have English-speaking staff, so it is advised that you know the generic (chemical) name of the medicines you think you are going to need. Many medications can be purchased here over the counter that would only be available by prescription in your home country.

It is standard practice for expats to visit private clinics and hospitals for check-ups, routine health care, and dental care, and only use public services in case of an emergency. Insurance companies can usually provide details of clinics and hospitals in the area speak English (or the language required) and would be the best to use. Investigate whether there are any emergency services or numbers, or any requirements to register with them. Providing copies of medical records is also advised.

Moscow hosts some Western medical clinics that can look after all of your family’s health needs. While most Russian state hospitals are not up to Western standards, Russian doctors are very good.

In a case of a medical emergency, dial 112 and ask for the ambulance service (skoraya pomoshch). Staff on these lines most certainly will speak English, still it is always better to ask a Russian speaker to explain the problem and the exact location.

Ambulances come with a doctor and, depending on the case, immediate first aid treatment may be provided. If necessary, the patient is taken to the nearest emergency room or hospital, or to a private hospital if the holder’s insurance policy requires it.

Our teaching staff applies curriculum and play-based learning activities. Children spend their day in a nurturing, safe and happy environment that promotes natural curiosity and desires.

Accreditation and Licensing

Charlie Bear’s Child Care is licensed and inspected annually by the City of Moscow’s Health and Welfare Department. We comply with all requirements for a licensed child care center.

We strive for excellence according to the professional standards set forth by the Early Childhood Environmental Rating Scale. We are committed to meeting these national standards of quality education.

In addition to following all state and federal regulations, Charlie Bear’s Child Care participates in the Idaho Stars Quality Program. Although participation is not required, this is one more way we can go above and beyond for the children that attend our daycare.

Our educational program is based on Early Childhood Environmental Rating Scale guidelines. This comprehensive program includes many areas that contribute to positive child development. The curriculum includes opportunities for children to make their own choices among various learning areas in the classroom, as well as daily teacher-planned activities in language, math, science, art, music and fine motor and gross motor areas.

Importance of Family

You are encouraged to be part of your child’s day at Charlie Bear’s Child Care. Daily reports and information are provided, along with a Parent Resource Center with books, videos and community updates. Family events are held throughout the year to develop strong relationships among the staff, families and children.

In compliance with the City of Moscow regulations, the maximum teacher-to-child ratios are as listed:

Credentials and Education

The teacher in each classroom who plans activities must reflect Idaho’s Early Learning Standards and national standards, have a degree in early childhood education or a related field or have prior experience in early childhood programs. Assistants are also qualified by education or experience to work with young children.

All staff are required to have criminal background checks, child-abuse record clearances and annual health appraisals. Staff members are licensed through the City of Moscow. They are certified in First Aid and CPR, and they receive a minimum of 12 hours in annual service training related to their duties as early childhood educators.
