Peaceful Passing

The Peaceful Passing Program exists to enhance the final hours of the patient and family’s life. By utilizing the expertise of each member of the hospice team at the patient’s bedside, we are often able to neutralize the fear around these final moments and create a much more supportive environment.

Complimentary Therapies

We believe in providing resources to go beyond the normal scope of the hospice benefit to our patients. This includes our offering of complementary therapies to all patients that provide opportunities to find freedom from pain and discomfort during the end-of-life journey. The include massage therapy, pet therapy, music therapy and much more.

Wish Program

We believe that hospice care is an opportunity for those nearing the end of life to regain control and live life to the fullest. It is our job as end-of-life caregivers to ensure that our patients have every chance to fulfill their dreams, and leave behind the legacy they choose. Every one is unique, and so are their wishes. That is why we do everything we can to facilitate dreams and help create moments that last a lifetime.

Where is Journey Home?

Journey Home is located at 994 Long Pond Rd. in Rochester, NY 14626. In the town of Greece on the corner of Vintage Lane and Long Pond.

Our driveway is on Long Pond Rd. (There is a light on the corner.)

It is easily accessible from 390 north, taking the Vintage Lane exit- turn west. Or north on Long Pond down from Ridge Rd. West. Turn just before you get to Vintage Lane. Heading South on Long Pond, we are just after you go through the intersection of Vintage Lane and Mill at Long Pond Rd.

We are one of three homes on the west side of the city, the only home in Greece, NY. Nearby is Mt. Carmel House on Planet St., in the city and Aurora House is in Spencerport, west of Journey Home.

Volunteer Opportunities

Volunteers add a special dimension to the care and comfort provided to our patients and their families. These specially trained, sensitive, caring individuals are often called the Heart of Hospice, exemplifying the spirit and mission of Sacred Journey Hospice.

We’re always looking for new volunteers who are interested in helping with the following:

Journey Home Hospice – a program of the Saint Elizabeth Foundation – provides specialized 24/7 hospice care for people experiencing homelessness and structural vulnerability at end-of-life. Our interdisciplinary team of staff and volunteers receives enhanced training in trauma-informed, culturally safe, and harm-reducing approaches to care to better serve our patients and their loved ones. All services at Journey Home Hospice are offered no cost to patients through the generous support of the community.
