In 2019, the average annual premiums are $7,188 for single coverage and $20,576 for family coverage. The average premium for single coverage increased by 4% since 2018 and the average premium for family coverage increased by 5%. The average family premium has increased 54% since 2009 and 22% since 2014.

PREMIUMS FOR SINGLE AND FAMILY COVERAGE

  • The average premium for single coverage in 2019 is $7,188 per year. The average premium for family coverage is $20,576 per year [ Figure 1.1 ].
  • The average annual premium for family coverage for covered workers in small firms ($20,236) is similar to the average premium for covered workers in large firms ($20,717). [ Figure 1.2 ].
  • The average annual premiums for covered workers in HDHP/SOs are lower for single coverage ($6,412) and family coverage ($18,980) than overall average premiums. The average premiums for covered workers enrolled in PPOs are higher for single ($7,675) and family coverage ($21,683) than the overall plan average [ Figure 1.1 ].
  • The average premiums for covered workers with single coverage and for family coverage are similar across regions for all plan types [ Figure 1.3 ].
  • The average premium for single coverage varies across industries. Compared to the average single premiums for covered workers in other industries, the average premiums for covered workers in the Manufacturing and Retail categories are relatively low and the average premium for State and Local Government workers are relatively high [ Figure 1.4 ].
  • The average premiums for covered workers in firms with a relatively large share of lower-wage workers (where at least 35% of the workers earn $25,000 annually or less) are lower than the average premiums for covered workers in firms with a smaller share of lower-wage workers ($6,189 vs. $7,233 for single coverage and $17,633 vs. $20,709 for family coverage) [ Figure 1.6 ].
  • The average premiums for covered workers in firms with a relatively large share of older workers (where at least 35% of the workers are age 50 or older) are higher than the average premiums for covered workers in firms with a smaller share of older workers ($7,485 vs. $6,941 for single coverage and $21,491 vs. $19,807 for family coverage) [ Figure 1.6 ].
  • The average premiums for family coverage for covered workers in firms with a relatively large share of younger workers (where at least 35% of the workers are age 26 or younger) are lower than the average premiums for covered workers in firms with a smaller share of younger workers ($19,094 vs. $20,753) [ Figure 1.6 ].
  • Premiums also vary by type of firm ownership. Covered workers at private for-profit firms have lower average annual premiums than covered workers at public firms or private not-for-profit firms for both single and family coverage [ Figure 1.6 ].

PREMIUM DISTRIBUTION

  • Seventeen percent of covered workers are employed in a firm with a single premium at least 20% higher than the average single premium, while 19% of covered workers are in firms with a single premium less than 80% of the average single premium [ Figure 1.7 ].
  • For family coverage, 19% of covered workers are employed in a firm with a family premium at least 20% higher than the average family premium, while 19% of covered workers are in firms with a family premium less than 80% of the average family premium [ Figure 1.7 ].
  • Seven percent of covered workers are in a firm with an average annual premium of at least $10,000 for single coverage [ Figure 1.8 ]. Seven percent of covered workers are in a firm with an average annual premium of at least $28,000 for family coverage [ Figure 1.9 ].

PREMIUM CHANGES OVER TIME

  • The average premium for single coverage has grown 19% since 2014, similar to the growth in the average premium for family coverage (22%) over the same period [ Figure 1.10 ].
  • The average family premiums for both small and large firms have increased at similar rates since 2014 (28% for small firms and 20% for large firms). For small firms, the average family premium rose from $15,849 in 2014 to $20,236 in 2019. For large firms, the average family premium rose from $17,265 in 2014 to $20,717 in 2019 [ Figures 1.11 and 1.12 ].
  • The $20,576 average family premium in 2019 is 22% higher than the average family premium in 2014 and 54% higher than the average family premium in 2009. The 22% family premium growth in the past five years is similar to the 26% growth between 2009 and 2014 but slower than the 34% premium growth between 2004 and 2009 [ Figure 1.14 ].
  • The average family premiums for both small and large firms have increased at similar rates since 2009 (59% for small firms and 51% for large firms). For small firms, the average family premium rose from $12,696 in 2009 to $20,236 in 2019. For large firms, the average family premium rose from $13,704 in 2009 to $20,717 in 2019 [ Figures 1.11 and 1.12 ].
  • For covered workers in large firms, over the past five years, the average family premium in firms that are fully insured has grown at a similar rate to the average family premium for covered workers in fully or partially self-funded firms (18% for fully insured plans and 20% for self-funded firms) [ Figure 1.13 ].
  • Premium growth continues to outpace both inflation and increases in workers’ earnings [ Figure 1.14 ].

‘Trust nurses, not AI’: Workers protest use of artificial intelligence at Kaiser hospitals

Clad in red shirts and holding signs reading, “Trust nurses, not AI,” hundreds of nurses from across California picketed outside Kaiser Permanente's San Francisco Medical Center on Monday to protest the use of artificial intelligence by the major hospital system—and the greater health care industry.

Nurses who rallied outside the hospital on Geary Boulevard as part of the protest organized by the California Nurses Association argued that the technology is untested and unregulated and said they fear it will make patient care worse.

“There are some uses of AI that are not acceptable,” Michelle Gutierrez Vo, a family medicine nurse, said during the demonstration.

The action coincided with the start of the KP International Integrated Care Experience conference in the Bay Area. The three-day event includes discussions on the use of advanced analytics and AI in health care, according to a public itinerary posted online.

According to the union, Kaiser Permanente is one of the earliest adopters of AI in the health care industry. The California Nurses Association, which represents 24,000 Kaiser nurses, is demanding that workers and unions be involved in the development of the technology and decisions on how AI is used in patient care.

“Nurses are all for tech that enhances our skills and the patient care experience,” Gutierrez Vo said in an earlier statement . “But what we are witnessing in our hospitals is the degradation and devaluation of our nursing practice through the use of these untested technologies.”

Cathy Kennedy, a neonatal intensive care nurse at Kaiser Roseville, said nurses understand that the new technology “is here to stay,” but she and her colleagues are concerned that Kaiser is using AI to cut costs and boost profits, rather than to improve patient care.

"We need to pause and really take time to say, is this truly what you want to do?" Kennedy told The Standard in a phone interview ahead of the protest. "We're questioning the validity of whether it's that important to continue increasing the amount of technology they are asking us to utilize as we're trying to take care of patients."

Kennedy, who has worked as a nurse for four decades, said some existing technologies already cause care delays, like electronic records that link staffing needs to real-time charting. If nurses don't log records immediately, the next shift could be short-staffed.

In a statement emailed to The Standard, Kaiser said the company uses AI tools to empower nurses and that the technology does not have the power to make health care-related decisions.

“We have consistently invested in and embraced technology that enables nurses to work more effectively, resulting in improved patient outcomes and nurse satisfaction, and we will continue to do so,” the statement said. “As an organization dedicated to inclusiveness and health equity, we ensure the results from AI tools are correct and unbiased; AI does not replace human assessment.”

But Gutierrez Vo said Kaiser currently gives patients the option to talk to an AI chatbot in lieu of calling an advice nurse. The problem with that is the chatbot uses medical jargon as keywords to sort and prioritize patients, so if patients describe symptoms indicative of a heart attack but not with medical terms the AI is trained on, they might not be directed to seek emergency care in a situation when a real nurse would.

“The AI might direct them to the pharmacy, not to a doctor,” Gutierrez Vo told The Standard.

Kaiser nurses are arguing that any new technologies need to be properly evaluated before they are deployed. The union is planning to invoke bargaining rights to negotiate the hospital system’s use of any new AI tools before they are rolled out.

Nurses at other health care systems in California said AI tools at their hospitals are making their jobs harder, not easier.

Melissa Beebe, a nurse who takes care of cancer patients at UC Davis Medical Center in Sacramento, said the hospital currently uses AI to monitor patients’ vitals. The  “bio button” tool alerts nurses if there are irregularities like an abnormal heart or breathing rate, but it often issues false alarms, Beebee said.

“The AI will say the respiratory rate is odd, but then I’ll look, and it’s fine,” Beebe said. “It’s not doing what it’s supposed to do, and we’re having to do double work.”

UC Davis Medical Center spokesperson Steve Telliano told The Standard Beebe’s concerns amounted to “fear-mongering,” saying that tool was introduced in May 2022 and allows patients’ vitals to be monitored while they are outside the hospital and to alert nurses of abnormal vital signs.

“The concern is that this [technology] will take jobs, it will not,” Telliano said. “It will make their jobs easier.”

But Beebe had another idea for how the hospital could make her job easier—and improve patient care.

“They could set up another oncology unit,” Beebe said. “But that wouldn’t save money.”

Health insurance giant Kaiser will notify millions of a data breach after sharing patients’ data with advertisers

U.S. health conglomerate Kaiser is notifying millions of current and former members of a data breach after confirming it shared patients’ information with third-party advertisers, including Google, Microsoft and X (formerly Twitter).

In a statement shared with TechCrunch, Kaiser said that it conducted an investigation that found “ certain online technologies, previously installed on its websites and mobile applications, may have transmitted personal information to third-party vendors.”

Kaiser said that the data shared with advertisers includes member names and IP addresses, as well as information that could indicate if members were signed into a Kaiser Permanente account or service and how members “interacted with and navigated through the website and mobile applications, and search terms used in the health encyclopedia.”

Kaiser said it subsequently removed the tracking code from its websites and mobile apps.

Kaiser is the latest healthcare organization to confirm it shared patients’ personal information with third-party advertisers by way of online tracking code , often embedded in web pages and mobile apps and designed to collect information about users’ online activity for analytics. Over the past year, telehealth startups Cerebral , Monument and Tempest have pulled tracking code from their apps that shared patients’ personal and health information with advertisers.

Kaiser spokesperson Diana Yee said that the organization would begin notifying 13.4 million affected current and former members and patients who accessed its websites and mobile apps. The notifications will start in May in all markets where Kaiser Permanente operates, the spokesperson said.

The health giant also filed a legally required notice with the U.S. government on April 12 but made public on Thursday confirming that 13.4 million residents had information exposed.

U.S. organizations covered under the health privacy law known as HIPAA are required to notify the U.S. Department of Health and Human Services of data breaches involving protected health information, such as medical data and patient records. Kaiser also notified California’s attorney general of the data breach, but did not provide any further details.

The Kaiser Foundation Health Plan is the parent organization of several entities that make up Kaiser Permanente, one of the largest healthcare organizations in the United States. The Kaiser Foundation Health Plan provides health insurance plans to employers and reported 12.5 million members as of the end of 2023.

The breach at Kaiser is listed on the Department of Health and Human Services’ website as the largest confirmed health-related data breach of 2024 so far.

Emergency medical care is provided free to all foreign nationals in case of life-threatening conditions that require immediate medical treatment. You will be given first aid and emergency surgery when necessary in all public health care facilities. Any further treatment will be free only to people with a Compulsory Medical Insurance, or you will need to pay for medical services. Public health care is provided in federal and local care facilities. These include 1. Urban polyclinics with specialists in different areas that offer general medical care. 2. Ambulatory and hospitals that provide a full range of services, including emergency care. 3. Emergency stations opened 24 hours a day, can be visited in a case of a non-life-threatening injury. It is often hard to find English-speaking staff in state facilities, except the largest city hospitals, so you will need a Russian-speaking interpreter to accompany your visit to a free doctor or hospital. If medical assistance is required, the insurance company should be contacted before visiting a medical facility for treatment, except emergency cases. Make sure that you have enough money to pay any necessary fees that may be charged.

Travelers need to arrange private travel insurance before the journey. You would need the insurance when applying for the Russian visa. If you arrange the insurance outside Russia, it is important to make sure the insurer is licensed in Russia. Only licensed companies may be accepted under Russian law. Holders of a temporary residence permit or permanent residence permit (valid for three and five years respectively) should apply for «Compulsory Medical Policy». It covers state healthcare only. An employer usually deals with this. The issued health card is shown whenever medical attention is required. Compulsory Medical Policyholders can get basic health care, such as emergencies, consultations with doctors, necessary scans and tests free. For more complex healthcare every person (both Russian and foreign nationals) must pay extra, or take out additional medical insurance. Clearly, you will have to be prepared to wait in a queue to see a specialist in a public health care facility (Compulsory Medical Policyholders can set an appointment using EMIAS site or ATM). In case you are a UK citizen, free, limited medical treatment in state hospitals will be provided as a part of a reciprocal agreement between Russia and UK.

It is standard practice for expats to visit private clinics and hospitals for check-ups, routine health care, and dental care, and only use public services in case of an emergency. Insurance companies can usually provide details of clinics and hospitals in the area speak English (or the language required) and would be the best to use. Investigate whether there are any emergency services or numbers, or any requirements to register with them. Providing copies of medical records is also advised.

Moscow hosts some Western medical clinics that can look after all of your family’s health needs. While most Russian state hospitals are not up to Western standards, Russian doctors are very good.

In a case of a medical emergency, dial 112 and ask for the ambulance service (skoraya pomoshch). Staff on these lines most certainly will speak English, still it is always better to ask a Russian speaker to explain the problem and the exact location.

Ambulances come with a doctor and, depending on the case, immediate first aid treatment may be provided. If necessary, the patient is taken to the nearest emergency room or hospital, or to a private hospital if the holder’s insurance policy requires it.
