As the world moves into 2023, the Medical Tourism industry is expressing cautious optimism that the sector can begin to recover from the setbacks due to Covid travel restrictions and global economic uncertainty.

Looking forward, most travel restrictions have been lifted, the populations of wealthier countries are multiple vaccinated, and there is a pent-up demand for delayed medical treatment combined with more economic confidence.

Medical Tourism crashed in 2020 as Covid concerns made travel more complex. For many, there was also increased economic insecurity. In terms of market value worldwide, medical tourism fell by 34% from US$105 billion in 2019 to US$71 billion in 2020 according to Glasgow Research & Consulting. It has since grown around 15% annually, and in 2023 the market value should exceed 2019 levels for the first time.

Relaxing travel restrictions

The year began with travel restrictions in most of the world, including the need to be vaccinated and have a negative test less than 24 hours before travel. In some cases, quarantine for up to fourteen days was required on arrival. Controls were gradually relaxed during the course of the year, and by mid-2022, travel restrictions for visitors to Europe and the US were largely removed. In December 2022, the EU recommended that there should be no travel restrictions on visitors from outside the EU.

The increased freedom to travel meant that medical tourism gradually rebounded, and several countries have introduced measures to take advantage of this.

In India, the government is planning a Heal in India promotional campaign to boost medical tourism in the country. A digital portal has been established to guide international patients through Indian healthcare options and project a global presence. The portal promises a simplified digital Medical Visa and emphasizes the availability of traditional Ayurvedic holistic treatments and conventional medicine.

India also hopes to profit from Russian visitors. In the wake of Russia’s invasion of Ukraine, the Asian powerhouse is one of a handful of countries, with Iran, China, South Korea, Turkey and Belarus, that accepts flights from Russia.

Major markets ready for rebound

Malaysia medical tourism.

The revenue from medical tourists to Malaysia was growing by 16% annually prior to the pandemic, and the number of visitors reached 1.2 million in 2019, the majority being from Indonesia and China. However, overall tourist arrivals and receipts fell by 80% in 2020. Healthcare tourism revenue was effectively zero in 2021, but the country re-opened up fully to overseas visitors in 2022 and hopes to return to 2019 levels in 2025.

Thailand medical tourism

Neighbouring Thailand is a key competitor for Malaysia in the medical tourism market, and it saw a similar successive fall in visitors of more than 80% in 2020 and 2021. However, Thailand has a much bigger share of the world market, up to 9% according to some measures, and attracts visitors from the USA and Europe as well as Asia. To encourage a bounce back in the sector, the government is planning to introduce a one-year medical visa at a reduced cost from January 2023.

Thailand aims to expand health and wellness tourism on the country’s largest island, Phuket. Recently, the government approved a 5 billion baht ($131 million) budget to develop an international medical hub in Phuket.

Turkey medical tourism

Turkey saw remarkable growth in medical tourism in 2021, with the number of visitors rebounding from just 390,000 in 2020 to 640,000, almost as many as the 660,000 visitors in pre-Covid 2019. That rapid increase appears to have continued in 2022, with 880,00 visitors reported in the year’s first three quarters. The Turkish government offers a range of support measures for companies in the industry, including part-payment for overseas diagnostic centres, salaries of interpreters and costs to bring patients from abroad.

Recent warnings from the UK government about poor treatment received by British Nationals in Turkey may dampen this growth. The Foreign Office recommended that patients refer to the Health Turkiye portal website for medical providers who the Turkish Ministry of Health approves.

Upcoming medical tourism destinations

As overseas patients return in 2023, post-Covid competition will be fierce. Several countries with a relatively small footprint in the industry will be looking to grow their market share.

Brazil is a large and populous country, but it has a relatively small footprint for medical tourism. Nevertheless, it has developed a niche reputation for cosmetic surgery and dental tourism, with eponymous treatments such as the BBL or Brazilian Butt Lift. 1.3 million cosmetic surgery treatments are carried out in the country annually (estimated 2020), although the majority of these are for nationals. Treatment costs are about half of those in the US, from where most overseas healthcare visitors originate. Costa Rica and Mexico also compete with Brazil for the cosmetic surgery market in the Americas.

Dubai and Abu Dhabi

Dubai is a leading hub for medical tourism in the Middle East, with a buoyant healthcare and wellness tourism market. According to the 2020 Medical Tourism index issued by the International Healthcare Research Centre (IHRC), Dubai is the number one medical tourism destination in the Arab world. The capital of the United Arab Emirates, Abu Dhabi, is an emerging option for tertiary treatments with well-established hospitals in Cleveland Clinic Abu Dhabi and Burjeel Holdings.

South Korea

South Korea is a growing player in Asia and has had strong government support to provide a sound foundation for medical tourism. The  Korea Health Industry Development Institute (KHIDI)  is a government-affiliated organization that promotes Korean medical services overseas. Korean medicine is renowned for excellence in oncology, cardiology and organ transplantation, as well as aesthetic-cosmetic treatment that gained popularity with the ‘k-beauty’ trend.

Visitor numbers were considerably lower than many neighbouring countries at 600,000 in 2019 but have recovered more rapidly. Russia, Mongolia and Kazakhstan are popular sources for patients, and the insecurity in the former Soviet bloc regions after the Ukraine invasion threatens this revenue. 

Longevity wellness programs

It is widely recognized that the isolation caused by lockdowns, working from home, travel restrictions, and the existential threats of a global pandemic has brought mental health issues to the forefront. Demand for wellness retreats and holistic therapies is expected to grow by 20% annually in the coming years. Switzerland, India and Thailand are some countries leading the way with medical-based wellness programs. The science of longevity is an emerging theme in the global wellness market, and longevity research is attracting significant investment.

Boom Segment: Tourism for medical and health travel reasons

The medical and health tourism sector has established itself as an important and constantly growing segment at ITB Berlin in recent years. Medical providers such as clinics, hospitals, as well as hotels and destinations with corresponding healthcare offerings can present their products and services here. Buyers and agencies are specifically targeted to visit the Medical & Health Tourism Pavilion at ITB Berlin.

This year, in addition to the other returning exhibitors, the German tour operator FIT Reisen and the European Historic Thermal Towns Association, which has significantly expanded its presence with an attractive action area with spa cinema and water tasting, as well as mud treatments. The European Spas Association, which, in collaboration with the Slovenian Tourist Board and other partners, offers also water tasting as well as oxygen therapy with the Hyperbaric System to visitors, is also represented again.

After the official pavilion opening, on the first day of the trade fair in the pavilion, interested journalists and editors have the opportunity to receive current information from the exhibitors first hand. This year the Media Lunch is presented by Barcelona Medical Destination, Health Tourism Croatia and the Slovenian Tourist Board.

Medical & Health Tourism Educational Hub

On Pavilion's own segment stage international trade visitors can find out about new developments and trends in this growth market through presentations, expert interviews and discussion rounds. The focus here is on current developments in the areas of digitalization, AI, but also sustainability. Dental sleep medicine and the topic of snoring guards are also the focus of this year's top-class medical presentations and discussions. Thermal treatments in the field of medical and health tourism are of course again an important focus of the program, with various contributions from partners of the European Spas Association, the European Historic Thermal Towns Association and the Great Spa Towns of Europe. Bulgaria will focus on cooperation in the field of medical tourism with German health insurance companies. Bulgarian Tourism Minister Zaritza Dinkova will also be present And in addition to this year's media partner Medical Tourism and Ayurveda Magazine from India, Dr. Prem Jagyasi from India will give a key note speech on the topic “Innovation and AI for growth of Medical Tourism and Wellness”.

ITB Medical & Health Night

This networking event, has also become an integral part of the Medical & Health Tourism segment of ITB Berlin; a network event for all exhibitors, trade visitors and media interested in the segment. The renowned ITB Medical & Health Tourism Award will be presented again this evening. This year's award goes to Slovenia for its outstanding commitment to medical health care and medical thermal treatments. This year's Medical & Health Night partners are the Zamber patient agency, Health Tourism Croatia and the Slovenian Tourist Board. The event is for invited guests only. PAHO conducts an NCD data analysis workshop with Aruba health authorities to improve health policies using the STEPS survey results

Washington D.C., 19 April 2024. The Pan American Health Organization (PAHO) received public health officers, policy advisors, and epidemiologists from the Ministry of Tourism and Public Health and from the Department of Public Health of Aruba to participate in a data analysis meeting to examine the results, and implications, of the STEPS survey conducted in the country in 2023.

Aruba implemented the STEPS household NCD survey between March and July of 2023. It covered areas such as tobacco use, alcohol consumption, diet, physical activity, hypertension, diabetes, cholesterol, lifestyle counseling, cancer screening, and mental health. The results illustrate the high burden of these diseases and their common risk factors, which jeopardize the health systems' ability to manage and implement customized policies and interventions adequately. Aruba health authorities will officially launch the results of the STEPS survey in a public event on May 17 th .

The 3-day workshop sought to interpret the survey results, and discuss strategies to help health services plan priorities and monitor and evaluate population-wide interventions while strengthening national capacity to conduct surveillance. This workshop included experts on all NCDs and risk factors areas within the organization, such as tobacco control, harmful use of alcohol, nutrition, physical activity, and primary health care.

Among the main areas for further action identified during the workshop, the national team highlighted overweight and obesity, alcohol consumption, and disease management, especially hypertension, diabetes, and cancer screening. To evaluate possible lines of action, participants learned and considered implementing  PAHO's new initiative, Better Care for NCD, an integrated approach to improve NCD management in primary health care. Better Care for NCDs is PAHO's flagship initiative, which nurtures and brings together the different initiatives that many countries already implement, such as HEARTS for cardiovascular disease, MPOWER for tobacco, SHAKE and REPLACE for nutrition, the Global Diabetes Compact, and the strategies for cervical, and breast cancer.

Acting at the first level of care and utilizing standardized diagnosis and treatment protocols in the early stages will help improve diagnosis, treatment, and continuous follow-up care to prevent early mortality and tackle the country's expressed main concerns. Better Care for NCDs strongly emphasizes identifying population needs to improve primary care for NCDs, engaging with communities, increasing primary care providers capacity and access to essential NCD medicines and technologies, and strengthening facility-based data collection and monitoring of NCD diagnosis, treatment, and outcomes. The 2023 Aruba STEPS survey is a great accomplishment in understanding the population’s health status regarding NCDs and their risk factors.

How does the STEPS survey work

The STEPwise approach to NCD risk factor surveillance (STEPS) is a simple, sequential, standardized method for collecting, analyzing, and disseminating data on key NCD risk factors in countries. STEPS  is a household survey that gathers information through a face-to-face interview (step 1), physical measurements of blood pressure, height, and weight (step 2), and collection of urine and blood samples for biochemical analysis of glucose and cholesterol (step 3). Implementing STEPS allows the comparability of data within and between countries due to its standardized data collection.

Spring reliably brings a whirlwind of number-crunching and form-filing as Americans finish their tax returns. Altogether, the IRS expects to process more than 160 million individual and business tax returns this season.

Ahead of Tax Day on April 15, here are seven facts about Americans and federal taxes, drawn from Pew Research Center surveys and analyses of federal data.

Ahead of Tax Day 2024, Pew Research Center sought to understand Americans’ views of the federal tax system and outline some of its features.

The public opinion data in this analysis comes from Pew Research Center surveys. Links to these surveys, including details about their methodologies, are available in the text.

The external data comes from the U.S. Office of Management and Budget and the IRS Data Book . Data is reported by fiscal year, which for the federal government begins Oct. 1 and ends Sept. 30. For example, fiscal 2024 began Oct. 1, 2023, and ends Sept. 30, 2024.

A majority of Americans feel that corporations and wealthy people don’t pay their fair share in taxes, according to a Center survey from spring 2023 . About six-in-ten U.S. adults say they’re bothered a lot by the feeling that some corporations (61%) and some wealthy people (60%) don’t pay their fair share.

Democrats are far more likely than Republicans to feel this way. Among Democrats and Democratic-leaning independents, about three-quarters say they’re bothered a lot by the feeling that some corporations (77%) and some wealthy people (77%) don’t pay their fair share. Much smaller shares of Republicans and GOP leaners share these views (46% say this about corporations and 43% about the wealthy).

Meanwhile, about two-thirds of Americans (65%) support raising tax rates on large businesses and corporations, and a similar share (61%) support raising tax rates on households with annual incomes over $400,000. Democrats are much more likely than Republicans to say these tax rates should increase.

Just over half of U.S. adults feel they personally pay more than what is fair, considering what they get in return from the federal government, according to the same survey.

This sentiment has grown more widespread in recent years: 56% of Americans now say they pay more than their fair share in taxes, up from 49% in 2021. Roughly a third (34%) say they pay about the right amount, and 8% say they pay less than their fair share.

Republicans are more likely than Democrats to say they pay more than their fair share (63% vs. 50%), though the share of Democrats who feel this way has risen since 2021. (The share among Republicans is statistically unchanged from 2021.)

Many Americans are frustrated by the complexity of the federal tax system, according to the same survey. About half (53%) say its complexity bothers them a lot. Of the aspects of the federal tax system that we asked about, this was the top frustration among Republicans – 59% say it bothers them a lot, compared with 49% of Democrats.

Undeniably, the federal tax code is a massive document, and it has only gotten longer over time. The printed 2022 edition of the Internal Revenue Code clocks in at 4,192 pages, excluding front matter. Income tax law alone accounts for over half of those pages (2,544).

The public is divided in its views of the IRS. In a separate spring 2023 Center survey , 51% of Americans said they have an unfavorable opinion of the government tax agency, while 42% had a favorable view of the IRS. Still, of the 16 federal agencies and departments we asked about, the IRS was among the least popular on the list.

Views of the IRS differ greatly by party:

  • Among Republicans, 29% have a favorable view and 64% have an unfavorable view.
  • Among Democrats, it’s 53% favorable and 40% unfavorable.

On balance, Democrats offer much more positive opinions than Republicans when it comes to most of the federal agencies we asked about. Even so, the IRS ranks near the bottom of their list.

Individual income taxes are by far the government’s largest single source of revenue, according to estimates from the Office of Management and Budget (OMB).

The federal government expects to collect about $2.5 trillion in individual income taxes in fiscal year 2024. That accounts for nearly half (49%) of its total estimated receipts for the year. The next largest chunk comes from Social Security taxes (including those for disability and retirement programs), which are projected to pull in $1.2 trillion this fiscal year (24%).

By comparison, corporate income taxes are estimated to bring in $612.8 billion, or 12% of this fiscal year’s federal receipts. And excise taxes – which include things like transportation trust fund revenue and taxes on alcohol, tobacco and crude oil – are expected to come to $99.7 billion, or 2% of receipts.

American tax dollars mostly go to social services. Human services – including education, health, Social Security, Medicare, income security and veterans benefits – together will account for 66% ($4.6 trillion) of federal government spending in fiscal 2024, according to OMB estimates.

An estimated 13% ($907.7 billion) will go toward defense spending. Another 13% ($888.6 billion) will repay net interest on government debt, and 10% ($726.9 billion) will fund all other functions, including energy, transportation, agriculture and more.

The vast majority of Americans e-file their taxes, according to IRS data . In fiscal 2022, 150.6 million individual federal income tax returns were filed electronically, accounting for 94% of all individual filings that year.

Unsurprisingly, e-filing has become more popular since the turn of the century. Fiscal 2000, the earliest year for which comparable data is available, saw 35.4 million individual income tax returns filed electronically (including those filed over the phone). These accounted for just 28% of individual filings that year.

By fiscal 2005, more than half of individual income tax returns (52%) were filed electronically.
