Private member’s bill seeks changes to northern health travel grant

Algoma-Manitoulin MPP Michael Mantha wants to see changes to the northern health travel grant.

That program reimburses northern Ontario patients for part of their expenses when they must travel more than 100 kilometres for medical treatment.

Mantha introduced Bill 13 after hearing from constituents about the high cost of gas, food and accommodations. Those costs make it difficult for anyone who must travel for medical treatment.

“Getting a commission established to look at improving, not just the cost or the reimbursement of the northern health travel grant, but the delivery of it,” Mantha said.

“Is there a way that we can improve or enhance it a lot quicker so that people don’t have to worry about it coming in late?”

If passed, Mantha’s bill would create a committee to look into the program’s effectiveness, including seeking feedback from northerners.

“The travel grant does not cover all costs -- in fact, it doesn’t cover your meals and it doesn’t cover the majority of your accommodation expenses considering how high hotel costs are nowadays,” said Donna Stewart, executive director of Manitoulin-Sudbury District Services Board.

Stewart said the travel grant is especially used by patients who may be on social assistance, but when other expenses increase, health care tends to take a back seat.

“I would fear that our clients do not access medical care as a result of the fact that they can’t afford it,” she said.

Private member’s bills are almost never approved, because they are usually introduced by opposition MPPs and lack support in the legislature. But Mantha said he’s aiming to be an exception.

“I’m hoping that the government is going to be receptive along with my colleagues from the opposition -- from all parties. I’ve been working with them,” he said.

Bill 13 will be debated at its second reading when MPPs return to Queen’s Park next week.

New direct deposit payment option

The Ministry of Health now offers direct deposit, also known as Electronic Funds Transfer ( EFT ), for the Northern Health Travel Grant ( NHTG ) Program. This payment option supports the Ontario government’s efforts to improve service delivery and make programs more convenient and easier to access.

Direct deposit is:

  • faster and secure
  • convenient and reliable
  • an environmentally friendlier way of doing business

Direct deposit is optional, not mandatory. Cheque payments will remain available to NHTG recipients.

NHTG application form changes

A new section has been added to the NHTG application form for “Payment Preference”. This section provides applicants the option to choose direct deposit or remain with mailed cheque payment. Detailed instructions are provided on the form as well as on the NHTG web page for applicants who wish to receive direct deposit.

Please order copies of the new version of the NHTG Application form by calling Ontario Shared Services at 416-679-8657 or by emailing the Ontario Shared Services .

Please discard all copies of outdated NHTG application forms once you have received copies of the new version of the NHTG Application form. Previous application versions with assigned form number E (2019/01) do not contain the new Payment Preference section. 0327-88

EXPLANATORY NOTE

The Bill enacts the Northern Health Travel Grant Advisory Committee Act, 2022 . The Act requires the Minister of Health to establish an advisory committee. The committee’s mandate is to make recommendations for improving the facilitation of reasonable access to health services for people in Northern Ontario by means of reasonable, realistic and efficient reimbursement for travel costs. The committee is required to consult with all relevant stakeholders, including, at minimum, the stakeholders specified in the Bill. The committee is required to report its recommendations to the Minister. The Minister is required to inform the Assembly of the recommendations that the Minister will implement.

An act to enact the northern health travel grant advisory committee act, 2022.

The Canada Health Act declares that “the primary objective of Canadian health care policy is to protect, promote and restore the physical and mental well-being of residents of Canada and to facilitate reasonable access to health services without financial or other barriers”.

The Northern Health Travel Grant helps people get to the healthcare they need. The program provides financial support for eligible applicants to travel to their closest medical specialist or ministry-funded health care facility. However, many people in Northern Ontario are unhappy with the program’s operation. In particular, they are dissatisfied with inadequate customer service, insufficient coverage and low rates of reimbursement. Improving the Northern Health Travel Grant will help Ontario meet a key objective of the Canada Health Act for Northern Ontario residents.

Therefore, Her Majesty, by and with the advice and consent of the Legislative Assembly of the Province of Ontario, enacts as follows:

1 In this Act,

“Northern Ontario” means the districts of Algoma, Cochrane, Kenora, Manitoulin, Nipissing, Parry Sound, Rainy River, Sudbury, Thunder Bay and Timiskaming.

Northern Health Travel Grant Advisory Committee

2 (1)  The Minister of Health shall, within 60 days after this Act receives Royal Assent, establish an advisory committee to be known as the Northern Health Travel Grant Advisory Committee in English and Comité consultatif des subventions aux résidents du Nord de l’Ontario pour frais de transport à des fins médicales in French.

Composition

(2)  The committee shall be composed of persons that the Minister believes will make useful contributions to the committee’s work, including,

  (a)  recipients of Northern Health Travel Grants; and

  (b)  health care providers working in Northern Ontario.

(3)  The Minister shall make all reasonable efforts to ensure that the composition of the committee reflects the diversity of the residents of Northern Ontario and, in particular, includes,

  (a)  women;

  (b)  Indigenous people;

  (c)  members of racialized groups;

  (d)  persons with disabilities;

  (e)  seniors; and

   (f)  persons of diverse sexual orientation, gender identity and gender expression.

Remuneration and expenses of committee members

(4)  The Lieutenant Governor in Council may by regulation prescribe remuneration and payment of expenses for the committee members.

3 (1)  The Northern Health Travel Grant Advisory Committee’s mandate is to make recommendations for improving the facilitation of reasonable access to health services for people in Northern Ontario by means of reasonable, realistic and efficient reimbursement for travel costs.

(2)  In particular, the committee shall inquire into the following matters respecting the Northern Health Travel Grant and may make recommendations about them:

   1.  Ensuring the rates of reimbursement reflect actual travel and accommodation costs incurred by persons required to travel to receive health services.

   2.  Enhancing service standards for communication with applicants.

   3.  Enhancing service standards for timeliness of the assessment of claims.

   4.  Broadening the types of health services for which travel costs are reimbursed.

   5.  Introducing pre-payment for costs, in particular for repeat visits to health care providers.

   6.  Introducing direct payment to providers of lodging and transportation.

   7.  Introducing payment for accommodation expenses incurred by a travel companion.

   8.  Simplifying and modernizing the process for proving expenses, including introducing electronic submission of receipts.

   9.  Ensuring the rules and policies relating to the Northern Health Travel Grant are reviewed.

Consultations

(3)  In carrying out its mandate, the committee shall consult with all relevant stakeholders, including, at minimum, the following:

   1.  Primary care providers, including the following:

           i.  Community Health Centres.

          ii.  Aboriginal Health Access Centres.

         iii.  Nurse Practitioner-Led Clinics.

         iv.  Family Health Teams.

   2.  The Ontario Medical Association.

   3.  The Northern Ontario School of Medicine.

   4.  The Association of Ontario Midwives.

   5.  The Heart and Stroke Foundation of Canada.

   6.  The Canadian Institute for Health Information.

   7.  The Ontario Autism Coalition.

   8.  Each public health unit in Northern Ontario.

   9.  All other provincial health agencies, including the following:

           i.  Cancer Care Ontario or its successor.

          ii.  Local Health Integration Networks in Northern Ontario or their successors.

(4)  On or before the day that is six months after the committee is established, the committee shall report its recommendations to the Minister.

Implementation of Advisory Committee’s recommendations

4 On or before the day that is 90 days after the Advisory Committee reports its recommendations to the Minister, the Minister shall inform the Assembly of the recommendations that the Minister will implement.

Commencement

5 This Act comes into force on the day it receives Royal Assent.

Short title

6 The short title of this Act is the Northern Health Travel Grant Advisory Committee Act, 2022 .

Your Health Travel Companion Program Northern Health Travel Grant (NHTG)

We know that accessing medical specialist services in Northern Ontario can be quite a journey. That's why Kasper Transportations is here to share some exciting news about the Northern Health Travel Grant (NHTG) Program. Get ready to embark on a smoother, more affordable, and stress-free healthcare adventure!

What is the Northern Health Travel Grant (NHTG) Program?

The NHTG Program is designed to help Northern Ontario residents who must travel long distances (at least 100 kilometers one-way) to reach medical specialist services or ministry funded health care facilities. If you qualify for the program, the Ministry of Health provides funding to cover a portion of your travel expenses.

How does the travel grant program work.

Great question! The NHTG Program offers financial assistance to cover travel costs based on the distance between your home and the nearest medical specialist or ministry funded health care facility. Whether you drive or use commercial transportation, you'll be reimbursed at 41 cents per kilometer (round-trip), after subtracting a deductible of 100 kilometers.

For example, if your one-way distance is 160 kilometers, the grant calculation would be ((160 x 2) - 100) x 0.41 = $90.20.

Accommodation Allowance: Rest Easy on Your Trip

When you need to travel at least 200 kilometers one-way to access a medical specialist or ministry funded health care facility, you might be eligible for an accommodation allowance. This helps cover your lodging expenses during the trip. The amount of the allowance varies based on the number of necessary lodging nights required by your healthcare provider.

For instance, the accommodation allowance can range from $100 for up to 2 lodging nights to $550 for 8 or more lodging nights. Remember, you must provide an accommodation receipt in your name to qualify for this allowance.

Who Qualifies for the Travel Grant?

To be eligible for the NHTG Program, you should meet the following criteria:

  • You are an OHIP-insured Ontario resident residing in specific Northern Ontario districts.
  • A healthcare provider from Northern Ontario has referred you to a certified medical specialist or ministry funded health care facility that is at least 100 kilometers away from your residence.

How to Apply?

Applying for the travel grant is a breeze! Just follow these steps:

  • Obtain the NHTG application form from your northern health care provider or online.
  • Fill out all required information in section 1 of the application form.
  • If you need a referral, get it from your northern health care provider. You typically need a new referral for each different specialist or health care facility.
  • Ensure your medical specialist or ministry funded health care provider completes section 3 of the application form, including the necessary number of lodging nights for your trip.
  • Submit your application form along with travel and accommodation receipts, if applicable, to the Ministry of Health, Claims Services Branch office in Sudbury.

The Northern Health Travel Grant is failing to properly compensate patients from Northern Ontario who must travel to southern Ontario for medical care, Sudbury MPP Jamie West says.

“We are in the midst of an affordability crisis,” West told the provincial legislature last week. “These individuals have faced the challenges of travelling long distances for specialized care, often at significant personal and financial costs.  

“People cannot afford to take two-three days off work, uproot their lives, and wait to be reimbursed.”

The Northern Health Travel Grant is designed to assist Northern Ontario residents in accessing necessary medical services outside of the region, but West said it falls short of addressing the financial challenges faced by patients from Northern Ontario.  

One of the key issues is the requirement for individuals to pay for accommodation and travel up front and then wait for reimbursement, “worsening an already stressful situation.”  

West, an NDP MPP, cited the case of two seniors living on fixed incomes who must travel to Toronto to visit neurologists.  

Despite their financial constraints, Denise and Stephan (West did not provide their last names) have no choice but to travel to Toronto. “To deepen their troubles,” West said the Northern Health Travel Grant only provides $100 per night for a hotel, while the actual discounted hospital-hotel rate is $250 per cent night.  

In addition, the mileage rate is a “mere” 41 cents a kilometre, but they only get compensated after the first 100 km.  

“Premier (Doug Ford) must commit to increasing mileage and accommodation compensation so seniors like Stephan do not have to bear out-of-pocket expenses to access essential care,” West said. “Northern Ontarians should not have to sacrifice their financial well-being or endure undue stress when seeking essential medical care.”

Sudbury MPP Jamie West is pushing the Ontario government to make the Northern Health Travel Grant more accessible for financially strapped patients who may not be able to afford the up-front costs for medical travel.

Speaking at Queen's Park Thursday during the final Question Period for the current sitting of the Legislature, West said many Northerners need to travel to Southern Ontario to receive specialized medical treatment that might not be available to them at home.

In order to qualify for the medical travel grant, a Northern Ontario patient must fill out a five-page form to determine if they qualify for financial assistance. Patients must fill out a new and separate application form for each trip they take. Under the current system, patients must pay for trips and accommodation on their own, and then submit a request to have the costs reimbursed to them.  

Patients must also submit their claim within 12 months from the date of travel.  When he spoke in the legislature Thursday, West described the system as broken.

"The broken Northern Health Travel Grant system forces patients to cover those travel costs and accommodations up front, and then they have to wait to be reimbursed," said West. 

 He gave the example of older residents of limited financial means.

"For example, Denise and her husband, Stephan, are seniors on a fixed income, and they need to come to Toronto to see Stephan’s neurologist. Denise told me Stephan will get 41 cents a kilometre, but only after the first 100 kilometres," West told the legislature.

West asked the question, when would the premier be able to remove the "cost-prohibitive barriers" that make medical travel difficult for many.

Health minister Sylvia Jones responded to West, saying she was aware there were some unique challenges for Northerners, adding her government had invested more than $48 million in the travel grant program.

Jones said one of the improvements is that the health ministry now allows individuals to apply for the grant electronically and have their reimbursements deposited directly into their bank accounts. 

West said another concern is that the Northern Health Travel Grant allows only $100 a night for hotel accommodation, an amount, he said, that doesn't even cover the rate for discount hotels.

The travel grant application form revealed there is an allowance schedule for overnight lodging, which allows $100 per night for the first two nights. The allowance drops after that to $250 total for three nights, then $500 total for four to seven nights, and then $550 total for eight or more lodging nights. 

“However, the “discounted” hospital-hotel rates are now $250 a night. So although the minister is saying the system works better, the reality is that Stephan and his wife are going to be out $150 at a minimum every night they’re here. That’s unfair for somebody who needs medical attention, said West.

West asked when the government is going to come around to increasing mileage rates and accommodation allowances. He said that seniors should not be out of pocket just to access medical care. 

Jones told West the health ministry is working to make it easier for citizens to get reimbursed for their travel. She said it is also a key reason why the government is investing to expand hospital capacities in the North.

NDP pushing for an emergency fund for the Travel Grant

The NDP is asking the province to set up an emergency fund for people needing to use the Northern Health Travel Grant.

Health Critic Frances Gelinas says some communities have help available, but only for a select few.

“If you are a member of a First Nation, if you are on Ontario Works, or Ontario Disability Support Program or a child registered with Easter Seals, there is a bit of help for you,”

“But for most people there is no help available.”

Gelinas says the province needs to make sure there is some financial assistance available.

“People should not have to come to see me desperate for care, but not able to afford a bus ticket to Toronto to get the care they need.  It’s clear Ontario needs an emergency fund available to the people of the north facing these circumstances.”

Gelinas says under the Health Travel Grant system, people have to pay their costs up front, and wait for reimbursement, that could take several weeks.
