What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Healing Phase

Background and aim Dental tourism, which reflects the provision of health care services abroad, also includes a travelling component. Air travel after dental intervention may cause barotrauma and barodontalgia. This paper aimed to provide guiding principles regarding the minimal time interval between dental procedures and air travel to prevent these adverse effects.

Methods A literature search was performed to reveal information with regards to complications related to flights following dental treatments. There is little research in this area and most of it has been conducted on the military aircrew population, which has different characteristics of flight and personnel than civilian commercial flights.

Results The recommended time of flying is one week after most dental intervention and six weeks after a sinus lift procedure. The minimal time required between a procedure and flight is 24 hours after restorative treatment, 24-48 hours after simple extraction, 72 hours after nonsurgical endodontic procedure, surgical extraction, and implant placement, and at least two weeks after sinus lift procedure.

Conclusions The provided guidelines may serve as a starting point for the clinician's decision-making. The tailoring of an individual treatment plan to the patient should take into consideration the patient's condition, dental procedure, complications and flight characteristics. Further research based on commercial flights is needed to formulate more accurate guidelines for the civilian population.

• Dental tourism is characterised by a short staying time at destination, and frequent travel to and from the foreign dental office.
• After the procedure, due to pressure changes during the flight back home, the patient may suffer from sinus barotrauma or barodontalgia.
• In order to prevent postoperative barosinusitis and barodontagia, the patient should not fly back home immediately after procedure.

Introduction

Medical tourism is the travelling to another country for medical care. Millions of people travel annually for health care, mostly to get therapy at a lower cost in comparison to the traveller's country. To a lesser extent, patients travel to get care from a provider who shares the same culture/language, therapy that is not available or approved in the traveller's country, or a complex treatment where, in the traveller's country, there is no satisfactorily experienced or suitable specialist, or devices to perform treatment. 1 , 2 , 3 Before the COVID-19 pandemic, the medical tourism industry was estimated to constitute 5% of all tourist travel in European Union countries. 4

Unfortunately, as for dental tourism, there is no valid and checked monetary data. According to market research, the dental tourism market would grow by 12% and hit US$5.83 billion by 2025. 5

Dental care is one of the most common procedures in medical tourism, along with surgery and cosmetic surgery, fertility treatments, transplantation and oncologic treatment. 1 Although accounting for approximately one-third of medical tourism, international dental tourism has been surprisingly neglected in the medical literature, as well as in medical manuals of airlines and of the International Air Transport Association (IATA). 6 , 7

Dental tourists usually travel from Western European countries to Eastern European countries, such as Hungary, Greece and Poland. North American patients typically favour dental services in Mexico or Costa Rica, while Australian citizens tend to travel to Asian countries, including Malaysia, India and Thailand. 8 In all of the above destination countries, complex dental procedures are considerably less expensive than in the travellers' home countries. It is clear that for the sake of economic viability, dental tourism is performed when extensive oral rehabilitation is needed, including endodontic and pre-rehabilitative surgery, sometimes necessitating staged treatments (for example, implant surgery), requiring frequent travel and short stays at the destination. In general, a dental tourist stays abroad for approximately 15 days, with an average of eight days for a single intervention, spending an average of ten hours in the dental office on alternating days. 9

Obviously, dental tourism necessitates frequent air-travel. Despite the pressurisation of commercial airplanes' cabins to pressure conditions of altitude <8,000 ft, there is still a risk of a number of adverse physiological effects caused by the air pressure changes. Thus, the dental tourist may be exposed to various health risks; two of which are barotrauma and barodontalgia. 10 , 11 The aim of this paper is to present these phenomena to the dental practitioner in the context of dental tourism and to offer prevention guiding principles.

Methods and sources

The authors surveyed the relevant literature sources, including Medline, PubMed, PubMedCentral, Medscape, for the terms: air travel, bardontalgia, barotrauma, dental tourism and in-flight emergency. All the relevant medical literature mention barodonalgia and its triggering factors but none of them provide a guideline for waiting time. Neither the medical manual of the main 67 airlines, nor IATA and International Civil Aviation Organisation's medical manuals do give it, some of them even do not mention the pressure-related problems. Besides the literature, some webpages of dental practitioners were surveyed about the question: how long is the suggested waiting time after dental intervention? Later, the authors discussed, based on their own experiences, and formulated a guideline for the estimation of necessary waiting time after dental interventions.

According to Boyle's law, at a constant temperature, gas volume varies inversely and directly to the pressure change. In the body areas with trapped gas, these gas volume changes may cause adverse effects called barotrauma. One such condition is barosinusitis (or sinus barotrauma), which may affect one or more of the paranasal sinuses. This inflammation develops due to the inability to balance pressures between the air in the sinuses and the air in the external environment because of upper respiratory disease, polyps, nasal injury etc. 12 Therefore, during flight, when the external atmospheric pressure increases (for example, during descent), the gas trapped in the sinus will be sucked into the sinus along with other mucous secretions and may cause intense pain and the development of inflammation.

Implications

As mentioned, commercial flights involve atmospheric pressure changes, despite cabin pressurisation. In the context of dental tourism, flying shortly after a sinus lift procedure may cause an inability to balance pressures, therefore exposing the patient to barosinusitis during in-flight pressure changes. 13 Moreover, in the case of sinus lift surgery, established barosinusitis may result in failure of the procedure. Therefore, when determining a treatment plan that includes a sinus lift for a dental tourist, the patient should be guided about the risk and the need to plan the journey back home by ground (or sea, if relevant). If air travel is unavoidable, it is advisable to wait at least two weeks from the time of the procedure until the flight and to make sure there are no sinus symptoms before air travel. 12

Barodontalgia

Barodontalgia is a toothache that occurs as a result of atmospheric pressure changes, 14 usually in teeth with previous disease or necrotic dental pulp. This phenomenon was reported on transport (military) flights, where the conditions were the same as commercial flights; 15 7% of the crews on these flights reported at least one event of barodontalgia. 16 Close to one-third of cases are due to recent dental restorative treatment (that is, up to one week between dental treatment and flight), termed post-operative barodontalgia, and nearly one-fifth were due to referred pain of barosinusitis. 16

To prevent post-operative barodontalgia, flying should be postponed for at least 24 hours (although one week is recommended) after restorative dental treatment. 12 It should be noted, however, that despite the pain, exposure to air pressure changes after restorative dental treatment has no long-term impact on treatment success or prognosis. The recommended waiting times after dental interventions, before air-traveling back home, are listed in Table 1 . 17 If air travel is inevitable, the dental practitioner should equip the patient with an effective analgesic, such as a combination of non-steroidal anti-inflammatory drugs (NSAIDs) with an other non-NSAID analgesic, such as paracetamol or dipyrone, where available (provided that there is no contraindication). 18

Recommended waiting times after some dental interventions before air-traveling back home

Medical tourism, including dental tourism, is not without controversies and complications. 8 , 19 , 20 The latter may originate from the medical procedure itself (such as quality of care) or from the travelling. The American Centre for Disease Control and Prevention (CDC) indicate that flying after surgery may increase the risk of blood clots, including deep vein thrombosis. Therefore, in order to prevent complications related to pressure changes, the CDC recommends to avoid air travel for at least ten days after chest or abdominal surgery, and for 7-10 days after face, eyelids, or nose surgery (such as cosmetic or laser procedures). 1 In the absence of research in the field of dental tourism, no recommendations have been published to date regarding air travel after dental care for dental tourists. Therefore, we present, for the first time, our recommendations based on what is known in the scientific literature and our experience ( Table 1 ). Most of the knowledge in the field was obtained from studies in populations of military aircrews. The flight conditions of military transport flights are similar to the flight conditions of commercial flights. Earlier, we found a higher barodontalgia rate in the civilian population in comparison to the military population. 21 Moreover, it might be assumed that the prevalence of barodontalgia among dental tourists is higher due to the poor dental status in tourists compared to military aircrews, who are required to have periodic dental examinations, 22 , 23 and because of tourists flying shortly after intensive dental care, compared to grounding of military air crews after dental treatments. 24

In addition, the CDC recommends that if the patient plans to take advantage of travelling abroad and include a vacation in the trip, there are activities, such as swimming or taking tours, which may not be permitted after medical treatment. 1 In the context of the above-mentioned dental care, it can be said that diving, mountaineering or other activities that may expose the patient to atmospheric pressure changes should be avoided. 25

This paper aimed to introduce the risk of barotrauma and barodontalgia with regard to flying shortly after dental treatment. It should be mentioned, however, that the post-operative effect of pressure change is not limited to these phenomena. Although quite rare, pressure changes may cause subcutaneous emphysema. For example, during multi-visit endodontic treatment, damaged temporary restoration, which causes open (unfilled) root canals, may predispose the tourist patient to subcutaneous emphysema. 26 Another example of potential risk for subcutaneous emphysema is the exposure of the post-extraction socket to the pressure changes of diving or flight. 27 Therefore, if the endodontic treatment is performed in more than one session, the cavity should be well-sealed with a temporary restoration material using the double seal method, in which a material with improved sealing abilities be placed first inside the cavity, followed by a second heavy layer of a material with improved compressive strength. 28 Alternatively, in such cases, the dental surgeons may consider using a material intended for permanent restoration as a temporary restoration.

In conclusion, dental practitioners who treat dental tourists should be familiar with the post-operative risks involved in air travel and should advise their patients on how to prevent or reduce these risks. The provided guiding principles may serve as a starting point for the clinician's decision-making. The tailoring of an individual treatment plan to the patient should take into consideration the patient's condition, dental procedure, complications and flight characteristics. Further research, based on commercial flights, is needed to formulate more accurate guidelines for the civilian population.

Stary Oskol is a city in Russia located in the Belgorod region, standing on the banks of the Oskol River, about 142 km northeast of Belgorod, 632 km south of Moscow.

History of stary oskol.

In 1300, there was a village called Ugly on the the territory of the present city. Today, it is one of the districts of Stary Oskol. At that time, the village was part of the Grand Duchy of Lithuania and was a border crossing point on the border with the Golden Horde.

The settlement of Oskol was founded by Russian Tsar Fyodor Ioanovich’s order (Ivan’s the Terrible son) to protect the southern borders of the Russian kingdom in 1593. The bulk of the population were peasants and the military.

In 1617, the town was burned by the Poles. In 1625, 1642 and 1677, the Crimean Tatars approached Stary Oskol, but could not capture it. The town was renamed Stary Oskol (meaning Old Oskol) in 1655, when the town of Tsaryov-Alexeyev, located down the river, was renamed Novy Oskol (New Oskol). Fortifications of the town existed until the 18th century.

More Historical Facts…

Gradually, Stary Oskol turned into peaceful, commercial and merchant town - the center of an agricultural district. Industrial production was represented mainly by enterprises on processing agricultural products.

In 1780, the town got its coat of arms. In 1784, Stary Oskol like most Russian towns of that time was rebuilt according to a new plan signed by Empress Catherine II. A large number of brick houses were built, mostly two-story buildings. The historic center of Stary Oskol has been preserved to our time.

In the second half of the 19th century, the town had a wide network of schools, libraries, reading rooms, printing, secondary schools. In 1894, construction of the railway began, trade and industry began to develop rapidly.

Until the 1930s, there were eleven Orthodox churches in Stary Oskol. During the Second World War, Stary Oskol was occupied by the Germans from July 2, 1942 to February 5, 1943. In 1954, the city became part of the newly formed Belgorod region.

In the late 1960s - early 1970s, in connection with the active development of the Kursk Magnetic Anomaly (the most powerful iron ore basin in the world), Stary Oskol became a major center of iron and steel industry.

Stary Oskol was declared a city of three top-priority Komsomol construction projects. The largest enterprises: Lebedinsky Mining and Processing Plant, Stoilensky Mining and Processing Plant, Oskolsky Electrometallurgical Plant became not only the city-forming enterprises, but also the largest industrial enterprises of the country. Stary Oskol experienced a new birth.

Today, Stary Oskol is a rapidly developing city, one of the leading mining and metallurgical centers of Russia.

The coat of arms of Stary Oskol is a shield divided in half diagonally, in the red field there is a rifle, in the green field - a golden plow. It reflects both military and agricultural importance of the town.

Stary Oskol is among Russian cities with the youngest population. The average age of its citizens is about 35 years. The main reason lies in the history of the city. In the second half of the 20th century, a large number of members of the Young Communist League came to Stary Oskol to participate in the construction of large industrial enterprises.

Unique reserves of iron ore, developed industry and infrastructure, high scientific, technical and industrial potential, fertile land form the basis of Stary Oskol economy.

The main branches of local industry are mining, ferrous metallurgy, machine building and metalworking, building materials, food industry. There are more than 140 large and medium industrial enterprises, which employ more than 80 thousand people.

Attractions of Stary Oskol

The city has more than a dozen Orthodox churches and chapels. Alexander Nevsky Cathedral (Tokareva Street, 4/1) is the main church of Stary Oskol. St. Trinity Church (Bolshevistskaya Street, 17/15), built in 1730, is the oldest church in the city.

The Zoo was opened in Stary Oskol in 2008. Bears, lions, tigers, monkeys, wolves, camels, ostriches, and other species of birds and animals live there.

Museum of Local Lore (Lenina Street, 50). The museums, founded in 1923, has exhibitions about the history of Stary Oskol. It is located in a building which is a monument of history and culture of the 19th century once owned by the merchants and brothers Likhutins.

The museum organizes tours of the city and the area including visits to an ancient Russian settlement of Kholki, an underground monastery in Melovy mountains, and a biosphere reserve “Yamskaya steppe”.

Art Museum (Lenina Street, 57). The collection of local masters of folk clay toys (O.M.Goncharova and N.M.Goncharova) is a real pearl of the museum. Stary Oskol folk clay toys production was known from the end of the 18th century.

The price of pain: Questionable billing by doctors rife in Australia

A major investigation into the chronic pain industry has uncovered how Australia's healthcare system is being left open to financial exploitation by some medical practitioners.

Trudi King is always in pain.

"The only relief you get is when you go to sleep," the 60-year-old says.

In 2021, she had spinal fusion surgery at a private hospital in Sydney after struggling with back pain for four years.

"All I wanted was to live a pain-free life," she says.

When she woke up, she couldn't feel her feet. 

"I was terrified … no one really told me what was going on," she says.

Trudi had a second surgery the next day. There were serious complications and she was in a coma for almost three weeks.

When she finally got home, she had to use a wheelchair.

As for the pain, it's now even worse.

Trudi is one of more than three million Australians grappling with the relentless agony of chronic pain. Back pain is the leading cause of disability and lost productivity in Australia and globally.

It has spawned a multi-billion-dollar industry offering treatments including spinal surgeries and medical devices — the majority done in the private sector.

For the past six months, Four Corners has been investigating the trail of misery it has left behind. For some it's meant more pain, paralysis, and even death.

Spinal fusion costs tens of thousands of dollars. Trudi's experience cost her health fund a fortune.

What she now knows, after the investigation by Four Corners, is that not only did her surgery cause her harm, but some of her medical bills raise concern.

An analysis of Trudi's records by medical billing expert Margaret Faux found questionable billing from anaesthetists involved in her surgery.

Dr Faux, the founder of medical technology company Synapse, did not have access to Trudi's full medical records, but analysed her private health insurance and Medicare bills for the time she was in hospital, including the period she was in a coma.

"There are red flags because during that same period, a number of doctors came into the intensive care unit," she says.

"The services that they billed are services that require the doctor to have a discussion with the patient; not their relatives, not their colleagues — with the patient — because you have to explain treatment to the patient.

"I do not understand how it is possible to have a discussion with an unconscious patient who is in a coma."

It's not an isolated case.

Four Corners has obtained the private health insurance billing records of more than 23,000 patients who underwent spinal surgery in Australia between November 2017 and May 2023.

Across the full data set, the analysis found:

• 77 per cent were billed for more complex services than provided
• 10 per cent were billed for longer services than provided ; and
• 7 per cent were billed for spinal fusions for chronic lower back pain without a diagnosis , in breach of a Medicare rule

The de-identified data was supplied by six private health funds, representing 25 per cent of the private health fund industry.

The records were sent for analysis to Dr Faux and a team at Synapse, as well as a team at Kirontech, a UK-based health fraud investigation company that used its specialist detection software.

They cross-checked the analysis and wrote a report. Together, they found more than 20 per cent of the bills they assessed raised questions.

The experts state in their report there is a strong suggestion of "a serious fraud, waste and abuse problem requiring urgent attention".

The terms "fraud, waste and abuse" are used widely in the industry and can cover conduct ranging from errors, to overservicing, to fraud.

Rachel David, CEO of Private Healthcare Australia, says if the findings were applied to the whole industry for spinal surgery, it would signal a huge draining of the health system by some medical professionals.

"We're talking about half a billion dollars."

The team analysing the data ran complex algorithms to exclude the possibility of other medical explanations, but without each patients' medical records it's impossible to be conclusive.

Most of the questionable billing came from anaesthetists and surgeons.

Here's how it works …

Every time a patient is admitted to hospital in Australia, that admission generates a number of bills across the health system.

Among them are bills from individual medical practitioners, claiming payment from a patient's private health fund and Medicare for the services they say they provided.

Practitioners charge for their services using codes. Each code represents a specific service.

As it's based on an honour system, they are automatically paid without checks that the services took place.

Red flags can emerge when the codes used by different practitioners involved in the same surgery are compared.

For example, each of these surgeries involved a fluoroscopy , an imaging procedure often used in spinal fusions.

The billing code used indicates the surgeries took less than one hour.

But the time billed by the anaesthetists in those same surgeries is much longer.

The analysis applied a buffer to take into account anaesthetic times are always longer than surgical times.

This means the anaesthetists charged the health system more ...

… including one who billed for more than 14 hours of work at an added cost of over $2,500.

The analysis also uncovered questionable billing patterns among surgeons … including charging for advanced procedures in implausibly short times.

In these surgeries, the anaesthetist billed for 2.5 hours or less … but the surgeon charged for procedures that even the quickest practitioner would be unlikely to complete in the available time.

For each additional level of spinal fusion surgery, surgeons claim an extra $1,350 on average.

In this surgery, a patient allegedly underwent three complex procedures in what code 23041 indicates was 20-35 minutes.

An expert who analysed the data said it was highly unlikely even a fast surgeon would be able to complete all three in that time.

Kirontech's chief medical advisor Dr Simon Peck – who is also a former anaesthetist – says the findings are alarming.

"We've looked at data from all around the world, and I would say the indicators of misbilling … is at the top end of the scale," Dr Peck says.

"Medical bills are paid on the basis of trust … After a while, when people realise that they're not being checked up on, they start … to push the boundaries."

The system is hitting the patient twice, Dr Faux says.

"We are exploiting them clinically by offering them treatments and procedures that … can be harmful, and then a second time financially."

Last year, an Independent Review of Medicare Integrity and Compliance, commissioned by Health Minister Mark Butler, found "legislation, governance, systems, processes and tools are currently not fit for purpose and, without significant attention, will result in significant levels of fraud".

The review made several recommendations to be implemented to strengthen the Medicare system.

Trudi says she's now in worse pain than before her surgery.

"It's just a dark place you have to mentally deal with," the grandmother of three says.

"I was very active, and I liked fishing and walking. [Now] I've got three grand babies under five and I can't play with them like I want to.

"It's like, 'Nanny's always sick.'"

The need for surgery

Trudi regrets deciding to have the surgery.

Spinal fusion involves permanently joining two or more vertebrae to prevent movement between them.

Trudi had three bones fused. She says an X-ray had found disk degeneration.

Spinal fusion is an accepted surgery for treating people with congenital spinal disorders, those who have broken their back, and some other conditions.

Sometimes it is recommended as a treatment for chronic back pain but a rising number of health researchers and surgeons say it's not an effective treatment.

Orthopaedic surgeon Ian Harris says there have only been a handful of trials.

"Those trials, when put together, don't show a clear benefit of surgery," he says.

"Some surgeons are very unlikely to recommend the procedure … it tells us that someone's wrong."

Professor Harris says MRI scans pick up abnormalities in most people aged over 40.

"The more things you find, the more likely it is that you'll end up with somebody wanting to treat or correct that abnormality — even though correcting that abnormality might not help the patient because it might not be associated with their pain."

More than 70 per cent of spinal fusions are done in the private sector, the latest figures estimate. The more bones that are fused, the higher the cost of the surgery.

Professor Harris is concerned this financial incentive drives more surgeries.

"The more you get paid for a procedure, the more likely it is that that procedure will be done," he says.

"It's too easy for us to let these other incentives influence our decision making."

Trudi says she's now trapped in the pain industry.

"I'm never not in pain," she says.

"Now I'm on this cycle; pain doctors … neurologists …"

She says she'd like to ask the surgeon who operated on her, "What the hell did you do?"
