- The first step is to open Permission Click and click Start New Form
- Select the template that you want to use. For this example, we'll create a field trip permission slip.
- Enter a name for this form. For example, Museum of Science Field Trip
- Enter some basic information about the trip. This will be the first thing that parents see when they open the form.
- Scroll down and enter the Name of the location.
- Enter the Address of the field trip
- Scroll down and enter the name of the field trip Coordinator.
- Scroll down and enter the field trip date and the start & end time. This does not have to be exact - it is just to give parents an idea of how long the field trip will last.
- Scroll down and click Questions about Each Child or Parent if you would like to ask for additional information. This is where you can ask what students will be bringing for lunch, ask for chaperones, or ask if there are specific medical concerns.
- Click Message to Respondents to leave a closing message after parents have filled out the form.
- Scroll down and customize the text for the Yes/No section.
- Edit the form settings as needed. The default settings are recommended but feel free to adjust the settings.
- If you enter your Event Dates in this section, parents will receive an automated reminder email the day before the field trip.
- You can also choose to cut off responses on a certain date if want to set a deadline.
- Toggle this button to send a reminder email.
- Click Publish when you're ready to move on to the next step.
- Now that the form is ready, we'll need to decide how to send it to familes.
- The Share this form area gives you a link that you can send out to families. For example, you can copy this link into a TalkingPoints or SchoolMessenger message.
- Alternatively, you can use the Recipients Manager to automatically fill in your roster of students.
- Click Schools and select your school to include all students.
- PermissionClick will automatically add all students to your form.
- It will take approximately 15 minutes to add all students and then a banner will appear at the top with a button to email the form.
- We recommend using either the Shared link OR the Recipients Manager, not both. This may cause duplicate students if a parent fills out the form twice.
- That's it! As parents fill out the form, the numbers will change at the top and you'll see the students listed at the bottom with their permission.

What does it mean to serve your community, and your country? Examine the rewards and challenges of active citizenry through the lens of military service then & now. Compare the lives of an 1812 sailor and a current US Navy crewmember through primary sources, a live interview, and hands-on activities on board USS Constitution.

Self-Guided Visit
Don’t just learn about history, experience it! Your group can independently explore two floors of interactive, hands-on exhibits. Self-guided activities like scavenger hunts and quests are available to focus student learning. Students and their chaperones are encouraged to visit USS Constitution after exploring the museum.
All Ages │ 60 minutes recommended Chaperone Ratio 1:10
Please note: the ship USS Constitution is separate from the Museum and operated by the United States Navy. It can be visited during operating hours on a first-come, first-served basis. Please be aware that visitors must pass through a security inspection and show federal or state photo identification, such as a driver’s license or passport, before embarking on USS Constitution . Visitors under the age of 18 do not require a photo identification card.
Virtual programs are available Mondays – Fridays from 1:00 PM – 5:00 PM.

Virtually visit USS Constitution in Boston MA, the oldest commissioned warship afloat in the world. In this field trip, you’ll walk the decks of this historic ship with a U.S. Navy sailor and learn about life at sea during the War of 1812 first-hand! Day in the Life of a Ship’s Boy
Experience “Old Ironsides” from the perspective of the youngest 1812 crew member, a ship’s boy! This 360 virtual tour, facilitated by a Museum educator, will have students exploring every corner of the Ship, from the powder magazine in the depths of the hull to the fighting tops, one hundred feet above the top deck. As students navigate the virtual tour platform, pop-up boxes with historical artifacts, videos of today’s U.S. Navy crew, and games bring the spaces to life. An on-demand, self-guided version of this virtual tour will be available soon.

Day in the Life of a Midshipman
This 360 virtual tour, facilitated by a Museum educator, follows a day in the life of an 1812 midshipman: a teenage officer in training. Students will navigate through every deck of the Ship in this virtual tour, encountering pop-up boxes with historical artifacts, videos of today’s U.S. Navy crew, and games along the way. An on-demand, self-guided version of this virtual tour will be available soon.

Since its founding by teachers over a century ago, Boston Children’s Museum has been a leading destination for memorable field trips and outside-the-classroom learning. In these challenging times, we are working hard to continue supporting our learning communities.
In-Museum Group Visits The Museum welcomes self-guided visits for schools, learning pods, community groups, afterschool programs, camps and childcare centers. Please contact our Groups Coordinator at [email protected]

Balls and Tracks (in Person) (School-year only)
Loop-de-loops, hills, tunnels, and more! There are many exciting parts to roller coasters and lots of things engineers have to keep in mind when designing them. Through trial and error and using their keen observation skills, students will work in teams to engineer a roller coaster track for a marble. This program is designed for 1st-5th grade and lasts about 45-55 minutes.
Money Matters: Earthquake Engineering (In Person) (School-year only)
Can you design a building to withstand an earthquake? Students will engage in financial literacy and engineering as they carefully spend their budget on materials to design an earthquake resistant structure. They will test their design on a shake table. This program is designed for 2nd-5th grade and lasts about 60 minutes.
Thanks to generous support from Citizens Bank, the Money Matters: Earthquake Engineering program can be facilitated free of charge for many schools. Within the interest form, you can indicate whether you would like your class to be considered for a sponsored visit.
Money Matters Virtual Museum Experience (School-year only) What are the businesses in our community? How do we and our neighbors make and spend money? Students will get the chance to explore these financial literacy concepts through role play, creating their own small businesses, and making shopping choices.
This program is broken into four parts, including two virtual visits from the Museum educator.
- Part 1 (teacher-facilitated): Students will be introduced to the idea of a small business through a storybook. The storybook will be provided by the Museum, but facilitated by the teacher.
- Part 2 (Museum-facilitated): A Museum staff member will virtually guest star (about 45 minutes) in the class to facilitate a discussion about businesses the students are familiar with and guide students in thinking about what small business they want to create.
- Part 3 (teacher-facilitated): The teacher will help students pick their businesses. Then, students will complete small business plans, make signs to promote their businesses, and make fake money to use in the following part of the program.

Field trips during the regular school day and Overnight within the State of Massachusetts
If your child takes medications at school those medications will be sent with a nurse or designated responsible adult as noted on the school medication administration form .
If your child has a special health care issue the school nurse will determine if field trip staff can be instructed in the care of your child. The school nurse will be responsible for that training. If the school nurse determines that your child requires the care of a registered nurse, coverage will be provided.
Overnight Out of State Field Trips
Registered nurses are licensed by the Board of Registration in Nursing in the state in which they practice. There is no national registration for registered nurses. For this reason the Norwood school nurses cannot delegate your child's nursing care or medication to field trip staff when students travel overnight outside of Massachusetts.
If your child has a special health care issue you should speak with the field trip staff about the care your child might require on the trip, or the possibility that you might accompany your child on the trip.
If your child will require medication on their trip please read and complete the out of state field trip medication administration form and return it to the staff person in charge of the field trip.
Out of the Country Field Trips
Again, registered nurses are licensed by the Board of Registration in Nursing in the state in which they practice. There is no international registration for registered nurses. For this reason the Norwood school nurses cannot delegate your child's nursing care or medication to field trip staff when students travel overnight outside of the United States.
It it is the policy of the Norwood Public Schools that if your child is mature enough to travel out of the United States on a field trip they are capable of being responsible for their own health care needs and medications. This policy also conforms to the United States Department of State and TSA regulations for taking medical equipment and/or medications out of or into the United States.
If your child has a special health care issue or will require medications while overseas please read and complete the out of country medication administration form and return it to the staff person in charge of the trip.
Any questions or concerns should be discussed with your child's school nurse .

While some schools may be lucky enough to have transitioned to electronic permission slips, many are still relying on the same photocopies that they’ve had for years. Field Trips
Appropriate resources existing in the community will be utilized by schools in planning and conducting the instructional program. Certain resources can be brought into the classroom to enrich the curriculum while others are best visited on field trips.
An instructional field trip is a planned visit outside the school taken by students, under the supervision of a teacher or other school official, to enrich and extend the classroom instructional program. Field trips should provide a link between the school and the community and assist the school staff in relating the ideas and theories of the classroom to practical applications. Field trips for instructional purposes are encouraged by the Arlington Public Schools with due consideration for budget limitations.
An extracurricular field trip is a planned visit outside the school taken by students, under the supervision of a teacher or other school official, in support of academic/athletic teams, performing groups, or other extracurricular activities.
An exceptional field trip is an instructional or extracurricular field trip which involves unusual circumstances.
The Director of Administrative Services is responsible for providing appropriate field trip opportunities and guidelines. The principal or supervisor is responsible for giving administrative direction to staff members who plan and supervise field trips and other uses of community resources.
