Immigration Medical Exam

Immigration physicals play a key role in the I-693 process.

Passport Health Dallas-Fort Worth offers 11 USCIS designated clinics for Immigration Physicals. Service can be provided in Spanish as well. We offer competitive prices for the lab tests and vaccinations that may be required. Our civil surgeon completes the final exam and paperwork completion.

The I-693 form must be completed for anyone applying for a Visa, Green Card or United States Citizenship or any adjustment in status .

Generally, a blood draw and urine test is necessary. The specific tests will be based on the patient’s age and personal medical history. Following the first visit, the applicant will be seen in clinic for a second follow-up visit. The entire process may take up to 14 days.

Instructions for your physical:

You must bring all medical records including vaccinations and your Passport/VISA. Have them translated into English or bring a translator with you. **It is the responsibility of the applicant to have any record used for the Immigration Physical to be officially translated into English .

The following is a list of items you are required to bring to your examination:

  • Your personal photo identification, which is normally a Driver’s License or Passport
  • Hospital or clinic records including all vaccinations
  • Prior chest x-rays and any other diagnostic tests

At Passport Health DFW, we stock all required vaccinations which can be given on the first or second visit.

Once the I-693 is completed, the original is provided in a sealed envelope for delivery to USCIS and a second copy is provided for your personal records.

Payment is due at the time of service for all fees, lab work, x-rays, etc.. We accept cash and major credit cards.

Finding A Designated Civil Surgeon

When you apply for a green card (adjustment of status) in the United States, you are required to undergo an immigration medical examination. The examination must be done by a doctor who is designated by U.S. Citizenship and Immigration Services (USCIS). USCIS designates certain doctors (known as civil surgeons) to perform the immigration medical examination required for most green card applicants. Military physicians are authorized as military blanket designated civil surgeons to perform immigration medical examinations at a military treatment facility within the United States for U.S. veterans, members of the U.S. military and designated dependents.  Additionally, some applicants for adjustment of status are only required to undergo the vaccination portion of the immigration medical examination.   Refugee applicants may be able to visit their local health department to see a health department blanket designated civil surgeon.   These blanket designated civil surgeons will NOT be found on USCIS’ list of civil surgeons, but are authorized to conduct the entire examination, or portions of it.  See USCIS Policy Manual Part C - Civil Surgeon Designation and Revocation  for more information on blanket designated civil surgeons

Immigration medical examinations performed outside the United States and its territories must be done by a panel physician. Panel physicians are different from civil surgeons and are appointed by the  Department of State . Panel physicians provide immigration medical examinations required as part of visa processing at a U.S. Embassy or consulate abroad and cannot perform the examination for applicants applying for adjustment of status with USCIS.

What To Bring To The Examination

Bring the following to your medical examination:

Form I-693 , Report of Medical Examination and Vaccination Record

Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.

Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease, and  COVID-19 )

Health insurance card. You should ask if the civil surgeon accepts your medical plan for immigration medical examinations. Many health insurance plans do not cover all portions of this examination.

Payment (check with the civil surgeon’s office about acceptable forms of payment). It may be helpful to call a few civil surgeons to find out how much they charge for the immigration medical examination. Prices can vary by several hundred dollars.

During The Exam

The immigration medical examination entails a review of your medical history and a physical examination.

The civil surgeon will complete a comprehensive examination that will include a review of your medical history and a physical examination. The civil surgeon will also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea, depending on age, as required by the Centers for Disease Control and Prevention (CDC).  CDC sets the requirements for the immigration medical examination in the Technical Instructions for Civil Surgeons | CDC .

After the immigration medical examination, the civil surgeon will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. Make sure you get a copy of the completed Form I-693 for your personal records before the civil surgeon seals the envelope. USCIS will not accept the form if the envelope has been opened or altered.

How to Apply at a Passport Agency

1. find out if you're eligible.

To apply at this passport agency, you must:

  • Have international travel within 14 calendar days.
  • Show printed proof of international travel. See examples in the 'Collect all your application materials' section below on this page.
  • Pay the $60 expedite fee with your  application fees .
  • Note : If you need a foreign visa, your appointment must be within 28 calendar days of your travel.

2. Collect all your application materials

3. Make an appointment

  • Go to our Online Passport Appointment System .
  • Enter information about your travel plans to see if you qualify for an appointment. You may make an appointment for up to 7 members of your household.
  • Enter your email address. We will email you a code which you must enter to continue making an appointment. Check your junk or spam folder if you do not see the code in your inbox.
  • Enter your mobile phone number. We will send you an SMS text message with another code you must enter to continue making an appointment.
  • Finish scheduling your appointment. Check your email for a message confirming the details of your appointment. The system will hold your appointment for 15 minutes. If you do not confirm your appointment within 15 minutes, start over.
  • If you need to cancel or change the appointment, click the link in the confirmation email to get information about your appointment. If you made an appointment over the phone, forward the email you received confirming your appointment to [email protected] with “Cancel my Appointment” in the subject line.

4. Arrive early to your appointment

  • We suggest you arrive 15 minutes early to go through security screening. You must show valid government ID to enter the building. Weapons, food, and drinks (except water in a sealable bottle) are not allowed.
  • No more than 15 minutes prior to your appointment time, take the elevator to the 11th floor, Suite 1120.
  • Check in at the Information Desk. Give them your appointment confirmation number to verify your appointment.
  • Be seated and wait until we call your number.
  • When we call your number, go to your assigned window.
  • A Department of State employee will review your application and supporting materials and discuss available delivery options.

There are parking lots on the corner of Commerce and S. Griffin St. in front of the Federal building; behind the Federal building at S. Griffin and Jackson St.; and at S. Griffin and Wood Street.

Sfo medical clinic.

The SFO Medical Clinic provides travel medicine, urgent care, immigration physicals and occupational health services. Our clinic offers the convenience of same day care and evening hours, and nearby free parking. Experienced physicians and registered nurses will treat you in our state-of-the-art facility with diagnostic X-ray services, physical therapy and pharmacy services. We are also available to conduct physical exams for sports teams, schools, camps, employment, DMV, and immigration. Our average wait time is 15 minutes. Drop-ins are welcome but appointments are preferred. We can see you today.

COVID-19 Booster

Starting November 9, 2023, COVID booster shots will be available to all workers and travelers, by appointment, at the SFO Medical Clinic . Contact the clinic to schedule an appointment: 650.821.5600

  • For passengers without U.S. health insurance, the booster shot is available for $190.
  • PPO insurance is accepted with direct billing. Please contact your individual healthcare provider about coverage.

Travel Medicine

Stay healthy on the road. Whether for business or pleasure, the Clinic offers the most complete travel and wilderness medical program in the Bay Area. Our certified travel vaccination center provides access to all the immunizations advised by the World Health Organization and the Centers for Disease Control (CDC). We offer immunizations for Hepatitis A, Hepatitis B, Influenza, Japanese Encephalitis, Meningococcal, Tetanus, Typhoid (1) , Yellow Fever and Rabies. An appointment is required; please call 650.821.5601 to schedule.

Urgent Care

The SFO Medical Clinic provides treatment for urgent medical needs such as sore throats, minor burns, urinary tract infections, fevers, rashes, ear infections, pink eye, sprains, and muscle strains. Please call 9-1-1 if you have more serious injuries or illnesses. 

Immigration Physicals

The SFO Medical Clinic has more than 40 years experience performing medical immigration exams. Our staff is up-to-date with all immigration law requirements. The facility is well stocked with adult immunizations and has on-site state-of-the-art digital equipment if an x-ray is needed. Convenient appointments at competitive prices are available. 

Occupational Health

The SFO Medical Clinic specializes in the treatment and management of work-related illness. The Clinic and its expert team of professionals are committed to promoting the health and wellness of your employees while offering a full continuum of injury prevention services including Ergonomic Assessments, Job Site Analysis and Educational Seminars. Priority will be given to random DOT screening. We strongly recommend that you preschedule any medical surveillance services to allow us to accommodate your employees.
