Historique de notre cabinet ORL (Centre Médico-Chirurgical de Touraine à Tours)

Auparavant le cabinet médical était localisé au 73 avenue de Grammont à TOURS et composé de trois ORL ; les Docteurs Michel GUINE, Christian HURET et Claude LOCICIRO, pratiquant depuis plus de 30 ans leur spécialité médico-chirurgicale avec excellence et reconnaissance de leurs patients. Dans le souhait de pérenniser leur travail, ils ont intégré le Docteur Vincent BOUETEL en 2008. Grâce à son dynamisme et sa faculté d’entreprise, le cabinet a été rénové et rajeuni par l’intégration successive des Docteurs Patrick LECERF, Karim HAMMOUDI et Bertrand FOUILLET, respectivement successeurs des Docteurs Claude LOCICIRO, Michel GUINE et Christian HURET.

Le projet du Centre Médico-Chirurgical de Touraine est né d’une réflexion médicale sur le devenir de la médecine de spécialité en ville, les contraintes de la pratique d’une spécialité chirurgicale nécessitant un plateau technique de proximité, le rapprochement des cliniques Saint-Gatien et l’Alliance sur un même site imposé par les tutelles et la volonté de rester attractif pour les jeunes générations. Notre cabinet a intégré depuis Octobre 2018 le Centre Médico-Chirurgical de Touraine situé à Saint-Cyr-sur-Loire à côté de la Nouvelle Clinique de Tours Plus, regroupant à partir de l’été 2019, Saint-Gatien et l’Alliance.

Le Docteur Clément BOBILLIER a intégré l’équipe début 2018 dans ce nouveau cabinet moderne, accueillant et adapté à toute personne. Au nombre de cinq, les médecins spécialistes ORL du cabinet, permettent une prise en charge rapide des patients par des délais de consultation raisonnables et une présence quotidienne même le samedi matin. Accompagnés par quatre assistantes, ils ont fait le choix de la proximité avec les patients et leurs médecins référents pour faciliter les échanges, la prise des rendez-vous et leur suivi.

Les examens au cabinet

Tests auditifs complets de l’enfant et de l’adulte

Les tests auditifs permettent de mettre en évidence une baisse de l’audition, d’en préciser l’intensité et d’en déterminer le type donc de localiser l’origine anatomique de la perte d’audition. Ils sont réalisés chez l’enfant en fonction de la participation de celui-ci le plus souvent à partir de 3 ans. Chaque bureau médical est équipé de matériel audiométrique pour réaliser les tests selon les normes d’isolation phonique recommandées.

  Vidéo-endoscopie des voies aériennes supérieures

L’examen est réalisé avec des fibroscopes souples par le nez, avec ou sans anesthésie locale en fonction de la tolérance de l’examen. Il permet de visualiser en haute définition l’ensemble des voies respiratoires jusqu’au larynx : fosses nasales, cloison nasale, cornets, orifice de ventilation des sinus, cavum (zone des végétations et départ de la trompe d’Eustache), pharynx et larynx.

Analyse de la voix

L’analyse de la voix peut être réalisée par stroboscopie et par analyse instrumentale de la voix. La stroboscopie du larynx est une technique de prise d’images du larynx, à l’aide d’une optique rigide placée dans la gorge, alternant des phases lumineuses et des phases obscures afin de visualiser les mouvements fins de la muqueuse des cordes vocales. L’analyse instrumentale de la voix est un outil de choix pour représenter et mesurer différents paramètres de la voix normale et pathologique.

Polygraphie ventilatoire nocturne

Cet examen est réalisé directement à votre domicile chez l’enfant et l’adulte afin de déterminer les troubles respiratoires du sommeil.

La polygraphie ventilatoire analyse différents paramètres durant votre sommeil (flux aérien, l’effort respiratoire, le rythme cardiaque et la saturation en oxygène, position du patient) afin de diagnostiquer un éventuel syndrome d’apnée du sommeil.

Les recommandations actuelles pour la prise en charge des troubles du sommeil dépendent du résultat de ce test. Il est parfois nécessaire de réaliser un examen polysomnographique, en hospitalisation au Laboratoire du Sommeil à la clinique, afin d’analyser d’autres paramètres pour permettre un diagnostic précis.

Exploration des troubles de l’équilibre

Les troubles de l’équilibre peuvent être liés à une pathologie vestibulaire (vertiges positionnels, maladie de Ménière, névrite vestibulaire…) qui nécessite des examens complémentaires spécifiques pour leur diagnostic. Nous disposons des outils diagnostics (vidéonystagmoscopie, VHIT, vibrateur, vidéonystagmographie avec épreuves caloriques à l'air). Nous pouvons réaliser une prise en charge rééducative au cabinet par réalisation de manoeuvres libératoires ou une rééducation vestibulaire par réalité virtuelle. La réalité virtuelle peut aussi être utilisée en cas de cinétose (mal des transports) ou de certaines phobies (agoraphobie, claustrophobie...).

Les traitements au cabinet

Certains traitements peuvent être réalisés au cabinet de consultation, ne nécessitant ni arrêt de travail, ni hospitalisation. Une anesthésie locale peut précéder le geste pour améliorer votre confort.

Le laboratoire de Sommeil de Touraine

  La prise en charge des troubles du sommeil, du ronflement et des apnées du sommeil nécessite un bilan complet et une prise en charge globale adaptée aux résultats de ce bilan. Le LST est constitué d’une équipe multidisciplinaire spécialisée dans les pathologies de sommeil permettant la réalisation de polysomnographie et de bilan complémentaire.
