A visit with your health care provider can be overwhelming. There are a few things to consider before, during and after your visit so you make the most of the time you have with your health care provider.

Prepare a discussion checklist.

Your health care provider will ask you several questions. The amount of questions depends on the reason for your visit and whether this provider has seen you before. Your answers help your provider understand your health and your health concerns.

Be prepared to discuss:

• The main reason for your visit
• New concerns
• Past and present medical conditions
• Any past surgeries
• Prescription and nonprescription medications, vitamins and herbal remedies
• All allergies
• Immunization records
• Family medical history

Write down your main concerns.

Prepare a list of main concerns. Rank them in order of importance.

You may want to ask:

• How can I improve my health?
• Are there conditions or diseases I am at risk for?
• What are possible side effects of medications I am taking?
• Are there any available resources that will help me understand my medical conditions?

It also is important to tell your health care provider about anything in your personal life, such as an illness in the family, financial worries or job difficulties, that may affect your health.

Describe new symptoms or recent changes in symptoms.

Your health care provider may ask you detailed questions, such as:.

• How long have you had the problem?
• What does it feel like?
• How often do the symptoms occur?
• What brings on the symptoms?
• What reduces the symptoms?
• Is the problem getting worse?
• Have other family members had similar problems?

Having a family member or friend with you can be helpful. Having two people hear the discussion can help later when you want to remember what was talked about. In addition, you can read the provider's notes from your visit through the patient portal.

Expect to ask all of your questions and have them answered. Talk with your health care provider about resources that may help answer your questions. This may take more time than this visit allows. If you need more time for questions, you may need to schedule a follow-up visit or a phone call.

Recap what you heard after the visit.

It may be helpful to write down what you learned from your visit. Include information about how your health care provider is planning to help you and what actions you can take to improve your health. Request an after-visit summary that includes visit diagnoses, treatment plans, medication list and patient education, if one was not provided to you.

Health Insurers’ Lucrative, Little-Known Alliance: 5 Takeaways

A private-equity-backed firm has helped drive down payments to medical providers, drive up patients’ bills and earn billions for insurers.

Large health insurers are working with a little-known data company to boost their profits, often at the expense of patients and doctors, a New York Times investigation found. A private-equity-backed firm called MultiPlan has helped drive down payments to medical providers and drive up patients’ bills, while earning billions of dollars in fees for itself and insurers.

To investigate this largely hidden facet of the health care industry, The Times interviewed more than 100 patients, doctors, billing specialists, health plan advisers and former MultiPlan employees, and reviewed more than 50,000 pages of documents, including confidential records made public by two federal judges after petitions from The Times.

Here are five takeaways.

The smaller the payout to doctors, the bigger the fees for insurers and MultiPlan

When patients see medical providers outside their plans’ networks, UnitedHealthcare, Cigna, Aetna and other insurers often send the bills to MultiPlan to recommend a payment amount.

MultiPlan and the insurers have a powerful incentive to keep the payments low because their fees get bigger as the payments get smaller.

Here’s how it works.

The most common way Americans get health coverage is through an employer that pays for workers’ medical care itself and uses an insurance company to administer the plan. Providers in the plan’s network have agreed-upon rates, but out-of-network providers often must negotiate payments.

By using MultiPlan’s frugal recommendations, insurers say they are saving employers money. But insurers and MultiPlan also benefit because their fees are typically based on the size of the declared “savings” or “discount” — the difference between the original bill and the amount actually paid.

In some instances, insurers and MultiPlan have collected more for processing a claim than the provider received for treating the patient.

UnitedHealthcare, the largest U.S. insurer by revenue, has reaped about $1 billion in fees annually in recent years from out-of-network savings programs, including its work with MultiPlan, according to legal testimony.

Patients could be on the hook for the unpaid bills

Patients have seen their bills rise after their insurers began routing claims to MultiPlan, as providers charge them for the unpaid balance.

Some patients said they have scaled back or ceased long-term treatment as a result. The predicament can be especially punishing for people who depend on out-of-network specialists, including for mental health or substance abuse treatment.

Patients have limited recourse. If they want to sue, they usually must first complete an administrative appeals process, and even if the case goes forward, they stand to collect relatively modest amounts.

Self-funded plans are mostly exempt from state regulation, and the responsible federal agency says it has just one investigator for every 8,800 health plans.

Some medical providers face big pay cuts

MultiPlan and insurers say they are combating rampant overbilling by some doctors and hospitals, a chronic problem that research has linked to rising health care costs and regulators are examining. But low payments also squeeze small medical practices.

Kelsey Toney, who provides behavioral therapy for children with autism in rural Virginia, saw her pay cut in half for two patients. She has not billed the parents of those children, but said she would not accept new patients with similar insurance.

Other providers said they have begun requiring patients to pay upfront because appealing for higher insurance payments can be time-consuming, infuriating and futile.

Former MultiPlan employees said they had an incentive to lock in unreasonably low amounts: Their bonuses were tied to the size of the reductions.

Employers are charged hefty fees

Insurance companies pitch MultiPlan as a way to keep costs down, but some employers have complained about large and unanticipated fees.

For a New Jersey trucking company called New England Motor Freight, UnitedHealthcare used MultiPlan to reduce a hospital bill from $152,594 to $7,879, then charged the company a $50,650 processing fee.

In the Phoenix area, trustees managing an electricians’ union health plan were surprised to learn that the fees charged by Cigna had risen from around $550,000 in 2016 to $2.6 million in 2019, according to a lawsuit the trustees later filed.

Employers trying to verify the accuracy of insurers’ charges have sometimes faced challenges getting access to their own employees’ data.

Private equity is playing both sides

For years, insurance companies have blamed private-equity-backed hospitals and physician groups for hiking bills and making health care more expensive. But MultiPlan is also backed by private equity.

MultiPlan’s annual revenues have climbed to about $1 billion thanks to its embrace of more aggressive approaches to reducing costs. Its premier offering is an algorithm-driven tool called Data iSight , which consistently recommends the lowest payments to doctors — typically resulting in the highest processing fees.

MultiPlan became publicly traded in 2020, and its largest shareholders include the private equity firm Hellman & Friedman and the Saudi Arabian government’s sovereign wealth fund, regulatory documents show.

New York City to wind down deal with embattled medical company tasked with housing migrants

ALBANY, N.Y. — New York City will move to end its contract with a medical services company hired to house and care for a recent influx of international migrants , following scrutiny over the firm’s costly deal with the city and the quality of its humanitarian services.

Mayor Eric Adams’ office on Tuesday said the city would not renew its contract with DocGo before it expires on May 5 and will instead search for a new housing provider for migrants.

DocGo, which previously worked with the city to provide COVID-19 testing, was awarded a $432 million no-bid emergency contract last year to help the city manage a massive new population of migrants.

The arrangement drew questions from city Comptroller Brad Lander, who in reviewing the contract determined there wasn’t enough detail to justify the cost and that it wasn’t clear how the company had the expertise to transport, house and feed thousands of migrants.

News reports from the New York Times and Albany Times Union also detailed alleged mistreatment of migrants under the company’s care, including issues with health care and food waste, among other things. Its chief executive officer resigned after he admitted to lying about his educational record.

DocGo currently provides care for 3,600 migrants in New York, half in the metro area and half upstate, officials said.

The company will continue to provide services for migrants who were relocated upstate until a new vendor is picked, with the city aiming to have a new contractor by the end of the year. In New York City, officials will temporarily use an existing contract with the company Garner Environmental Services to care for migrants.

“This will ultimately allow the city to save more money and will allow others, including non-profits and internationally-recognized resettlement providers, to apply to do this critical work, and ensures we are using city funds efficiently and effectively,” Camille Joseph Varlack, chief of staff for the mayor, said in a statement.

The city has other ongoing contracts with DocGo and will continue to work with the company in those deals.

In a statement, a DocGo spokesperson said the company is “immensely proud of the exceptional work that our team has accomplished and continues to perform in aiding the City’s response to this unprecedented crisis.”

New York has struggled to handle an influx of international migrants who have arrived in the city since 2022, with more than 187,000 people coming through its intake system seeking shelter.

City Hall – Today, the New York City Council voted to enhance safety measures for emergency medical services workers by requiring that the Fire Department of New York (FDNY) provide body armor, as well as de-escalation and self-defense training, to its Emergency Medical Services (EMS) employees.

Additionally, the Council approved land use applications estimated to produce more than 1,550 new housing units, over 1,400 of which are affordable, and Department of Housing Preservation and Development financing that preserves 655 rent-regulated units. The city legislature also voted to close a loophole that allowed some employers to utilize employment contracts that shorten the statute of limitations for employees to file legal claims and complaints for unlawful practices below timelines established by the City’s Human Rights Law. A resolution to annually recognize April 17 as Giovanni da Verrazzano Day was also passed.

“The Council is proud to pass safety measures that will better protect and support our frontline emergency medical services workers, who save lives and courageously serve our city,” said Speaker Adrienne Adams . “Our essential workers deserve to be safe while performing life-saving duties, and these new laws will help reduce the health and safety risks they experience.

“As part of the Council’s continued efforts to address the city’s housing crisis, our approval of more than 1,550 new housing units, over 1,400 of which are affordable, is an essential contribution to creating more homes,” continued Speaker Adams . “We will continue working with partners across all levels of government to deliver more protections for workers and more affordable housing for New Yorkers to remain and succeed in the city we all love.”

Improving Safety for Emergency Medical Services (EMS) Workers

Introduction 126-A , sponsored by Minority Leader Joseph Borelli , would require that the Fire Department of New York (FDNY) provide body armor to employees within the Bureau of Emergency Medical Services (EMS). This would largely codify existing practices and establish requirements that protective equipment meet ballistic and stab resistance standards of the National Institute of Justice.

Introduction 127-A , sponsored by Minority Leader Joseph Borelli , would require that FDNY provide de-escalation and self-defense training to EMS workers.

During Fiscal Year 2023, EMS responded to 30,306 reported segment one incidents, which are considered life-threatening medical emergencies. Basic Life Support incidents include a wide variety of non-life-threatening conditions. EMS workers have historically faced health and safety risks associated with delivering pre-hospital care. These risks include emotional stress and trauma, threatened and actual assaults , and relatively low financial compensation. Partially due to these employment-related experiences, EMS employee attrition rates have consistently remained high.  

“These brave men and women put themselves in harm’s way every day to save New Yorkers’ lives,” said Minority Leader Jospeh Borelli. “The least we can do is help protect them so they, too can get home safe to their families.”

Prohibiting Job Contracts That Shorten Legal Timeline to File Complaints for Unlawful Practices 

Introduction 69-A , sponsored by Council Member Lincoln Restler , would prohibit provisions in employment agreements that shorten the period in which claims and complaints of unlawful discriminatory practices, harassment, or violence may be filed and in which civil actions may be commenced. This bill is designed to close a loophole that has allowed some employers to shorten the statute of limitations in employee contracts, leaving many with limited windows to file claims that are below those established by the City’s Human Rights Law (NYCHRL). Under NYCHRL, employees have one year to file a complaint with the Commission on Human Rights for an unlawful discriminatory practice or act of discriminatory harassment or violence, three years to file a claim of gender-based harassment, and three years to commence a civil action. This bill would void any provision of any employment agreement that purports to shorten these periods.

“New York City workers must be able to rely on our landmark human rights laws to protect them from mistreatment in the workplace,” said Council Member Lincoln Restler . “Some of the biggest corporations in our City are forcing New Yorkers to sign away their right to fight back against harassment, discrimination, and violence due to an unfair loophole. I’m proud that the passage of our legislation will restore the true spirit of the City’s Human Rights Law and ensure that workers are protected in court and able to hold bad actors accountable. Thank you to Speaker Adams for her strong support of this legislation and to attorney and constituent Anne Clark for bringing this issue to our office’s attention.”

Recognizing April 17 as Giovanni da Verrazzano Day

Resolution 74 , sponsored by Council Member David Carr , would recognize April 17 annually as Giovanni da Verrazzano Day. Giovanni da Verrazzano was an Italian explorer, who sailed under the French flag and became the first European to sail into New York Harbor and into the mouth of the Hudson River on April 17, 1524 – 500 years ago next week.

396-40 Avenue X Rezoning – PG Realty Investments, LLC, seeks a zoning map amendment from R4/OP to R7A/C2-4/OP and a text amendment to map a Mandatory Inclusionary Housing Area (MIH) Options 1 or 2 (ZR Appendix F). These actions will facilitate a new 46,000 square foot, 7-story mixed-use building in Council Member Justin Brannan’s district. It will include approximately 45 housing units, with 12 affordable units, retail/medical office space and 45 accessory off-street parking. The Council will be modifying the MIH text amendment to remove Option 2, requiring MIH Option 1.

Willets Point Phase II – New York City Economic Development Corporation; Queens Development Group LLC; City Football Stadium Group LLC seeks an amendment to the text of the Zoning Resolution, special permits, and amendments to the City Map in Willets Point within the Queens district of Council Member Francisco Moya . These actions will facilitate the second phase of a long-term area-wide redevelopment strategy that will transform Willets Point into a new mixed-used neighborhood. This phase of the redevelopment will create approximately 1,400 units of permanently affordable housing, a 250-room hotel, a 25,000-seat soccer stadium to be the home of NYC Football Club, 2.8 acres of publicly accessible open space, and 80,000 square feet of local retail space. 

30-11 12th Street Rezoning – 30-11 12th Street Realty, LLC, seeks a Zoning Map Amendment to change existing R6B, R5B and R4-1 zoning districts to R6A and R6A/C2-3 zoning districts, and a Zoning Text Amendment to create a Mandatory Inclusionary Housing (MIH) area Options 1 and 2. These actions will facilitate a new 62,761 square foot, 8-story mixed-use building with approximately 86 housing units, 26 units will be permanently affordable, ground floor retail and 41 accessory parking spaces in Council Member Tiffany Cabán’s district. The Council is modifying the application to remove MIH Option 2, requiring Option 1 and adding the Deep Affordability Option.

23-01 Steinway Street Rezoning – Efraim Realty, LLC seeks Zoning Map Amendment to change existing R5D/C2-3 and R5D districts to R6A/C2-4 districts, and a Zoning Text Amendment to create a Mandatory Inclusionary Housing (MIH) area Options 1 and 2 in Council Member Tiffany Cabán’s district. These actions will facilitate a new 27,000 square foot, 6-story mixed-use building plus cellar with approximately 22 housing units, 6 affordable units, ground floor retail space and 6 parking spaces. The Council is modifying the application to remove MIH Option 2, requiring Option 1 and adding the Deep Affordability Option.

Apple TV+ isn’t usually the first option you think of when counting the best TV streaming services — but since its launch back in 2019, the platform has added hits like “Ted Lasso” and the Oscar-winning film “CODA”. It’s also become a brilliant place for modern sci-fi, in my opinion, with excellent shows like “Severance,” “Silo,” “Foundation” and more. As Power tells us, “If you want prestige TV at a reduced cost compared to Max, Apple TV Plus is absolutely worth investing in.”

As you’d expect from Apple, the interface is a brilliantly sleek affair and offers a streamlined experience for navigating your favorite shows. Similar to Amazon Prime, the Apple TV app offers plenty of licensed programming for purchase, even if it’s not covered by your subscription.

One big perk is Apple TV+’s integration with the broader Apple ecosystem. New Apple device purchases generally throw in a three-month subscription for you to test the streaming service out. If you’re a big Apple fan, you can opt for a $19.95 per month Apple One subscription that includes Apple Music, Apple TV+, Apple Arcade and 50GB of iCloud storage — truly a fantastic deal, in my opinion.

Prime Video

What we like about Prime Video: Offline downloads, original programming and the Prime ecosystem

Amazon Prime Video is one of the largest streaming services out there, whether you’re counting its massive library — with three times the number of movies as Netflix, at the time of writing — or its 200 million subscribers. 

Prime Video’s biggest benefit is the Prime ecosystem: when you subscribe, you don’t just get access to TV streaming, but also free delivery and exclusive discounts on the Amazon retail site, over 100 million songs through Amazon Music Prime, grocery delivery through Amazon Fresh, a Twitch Prime subscription and unlimited photo storage. 

But Prime Video alone seems worth the subscription cost, thanks to a huge content library with 15,000 films and almost 2,000 TV shows as of 2024 . By comparison, Netflix has half the number of movies, even if it does better on TV series. it’s an excellent choice for many genres especially if you’re partial to an evening film, in my opinion.

You should look out for streaming service deals or sales. Black Friday deals often offer bundles or full year subscriptions at a discount.

Power tells us that, aside from using free trial periods, “the best way to get a great subscription deal is by ‘subscription hopping’. Essentially, in the final week of any given month, look at each streamer’s upcoming slate for the month ahead. Powers explains that “the introduction of ad-supported tiers makes things more affordable for those on a budget. Even then, signing up to Netflix’s ad-based subscription means you have to sit through a certain number of ad breaks every hour, which doesn’t appeal to many.”

It’s well worth looking at feature functionality between streaming services – such as 4K / Ultra HD resolution, support for Dolby Atmos or Spatial Audio (for those with compatible speakers) or the ability to add multiple users to your account. Support for HDR (High Dynamic Range), which expands the contrast and color accuracy of the images onscreen, is also common across streaming services, though premium HDR formats are often locked behind certain plans – do check what your TV supports and try to pick a TV streaming service that caters to its specific formats.
