Appointment & Clinic Policies

What you need to know as a patient of Central Family Medicine Teaching Centre

Booking Your Visit

When calling for your first appointment, please tell us the reason for your visit and have your Alberta Health Care number ready.

Family members may come with you, but if they need to be seen by the doctor, we ask that they make a separate appointment.

Checking In

Before your appointment, please check in with reception so we can update your information and inform the module staff that you have arrived. We will need:

  • Your current address
  • Your phone number
  • Emergency contact number
  • Two pieces of ID, including one photo ID

What if you cannot make it?

Call at least 24 hours in advance if you cannot make your appointment. This frees up your visit time for another patient.

If you’re going to be late for your appointment please let us know. In some cases there may be a delay or you may be asked to reschedule your appointment.

No Show Policy

If multiple appointments are missed without cancellation, you may be asked to seek medical attention at another facility. If you are a new patient and you miss your first appointment, another appointment will not be made with any physician in the clinic in the future.

Patient Outreach

If we have not seen a patient in greater than three years, our reception team will reach out to them to inquire if they are still a patient of ours or not.

Clinic Services

Prescription refills.

  • You may contact your drugstore for the status of refills that may be left on your prescription.
  • If there are no refills left, call the clinic early to arrange an appointment to review your condition and medications.
  • Your community pharmacist can extend most prescriptions for a short time to last until your next appointment. Exceptions are narcotics, triplicate order medications, antibiotics and steroids.
  • Please bring all your medications in the original containers to your visits at the clinic. This will assist greatly in your conversation with your physician regarding the amount of medication being ordered, and the number of refills before your next visit. Book the next visit before leaving the clinic, to ensure that the timing of your next visit will prevent running out of medication.

Referrals and Special Tests

You must see your family physician to get a referral to a specialist. Referrals will be processed by the referral staff. You will be called about the date and time when it is con­firmed. If you are not able to attend the appointment time or date given, please call the specialist’s of­fice to re-book.

Test Results

Please do not assume “no news is good news”. When tests are being ordered, partner with your physician in planning what follow-up is needed regarding results. This could result in another visit – if so, book that next visit before leaving the clinic. Your physician can advise how long results would usually take to arrive, and can suggest when the best timing for a follow-up visit would be.

Vaccinations

CTC offers the following vaccinations

  • Diphtheria, Tetanus, Acellular Pertussis (Tdap)
  • Pneumococcal Polysaccharide (Pneumo-P)

Uninsured Services

Certain services are not covered by Alberta Health and a fee will be charged for these services. The exact amount will depend on the service:

  • Driver’s Medical Exam
  • Insurance Forms
  • 3rd Party Request Exams
  • Sick Notes for work or school
  • Travel Advice

Location change for Calgary adults needing immunization

Starting March31, Calgary adults booked for vaccinations will only be able to get their shots at the Sheldon M. Chumir Health Centre.

Alberta Health Services says moving all the adult immunizations to one location means appointments will be available six days a week.  There will also be additional appointment times in the evenings and on weekend.

Immunizations are currently offered at AHS clinics in Brentwood, South Calgary Health Centre, and Sunridge.  But AHS points out adult shots are only offered one day per week at those locations.

Some of those locations offer free parking, but that’s not the case at the Sheldon M. Chumir.  It currently costs $2.25 per half hour but starting April 1 st , that will increase to $2.50. The rates are in effect 24 hours per day.

In a statement, AHS says “to support the public, alternative parking plans and supports are available”  AHS also points out there’s “free street parking within walking distance adding alternate transportation options, such as transit and carpooling, can also be considered to reduce parking fees.”

Calgarians are still facing long waits to book an appointment through 811.  On Thursday one patient ended up waiting an hour to get through.

In a statement, AHS says it “continues to work diligently to ensure Albertans have timely access to immunization services in the Calgary Zone”

It's also recruiting and training more staff to help answer more calls.

  • the dental services include: exams, X-rays, fluoride treatment, sealants, fillings, front tooth root canals, extractions, etc
  • priority to emergency based treatments

The following services are NOT available:

  • full dentures
  • orthodontics or braces
  • molar root canals
  • periodontal surgery

  • Proof of family income (e.g. “Notice of Assessment” for previous year’s tax return)

  • Emergency clients may fill the Temporary Assessment Form

Individuals with access to group dental plans (e.g. Blue Cross) or having similar coverage (e.g. Child Health Benefit or Dental Assistance for Seniors Program) are not eligible.

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Emergency clients (pain, infections, extractions) receive priority for dental appointments.

Patients who have received a full series of treatment from this program must wait three years after treatment is complete before re-qualifying for another full series of treatment. Emergencies can still be dealt with during this three-year waiting period.

An estimated wait time will be provided at the time appointment booking. Emergency usually accommodated same day or next business day.

Fees are approximately 20% of the Alberta Usual and Customary Fee Guide for Dental Services.

Calgary's first permanent, supervised consumption site is set to open downtown on Monday, offering "welcoming, warm, inclusive services" for people in the community.

Located at the Sheldon M. Chumir Health Centre, the facility will be a place for people to pick up harm reduction supplies, such as new needles, and inject, swallow, or snort their drugs in one of six designated booths under the supervision of a registered nurse trained in overdose response.

It's the latest action from the province in the battle against the opioid crisis, which claimed 482 lives through accidental overdoses in the first three quarters of 2017, according to the government's latest numbers.

"The goal is to reduce the deaths from the opioid crisis as well as mitigate some of the other health risks associated with drug use," said program co-ordinator Clare O'Gorman.

A temporary safe consumption site that had been operating out of a trailer at the Chumir centre since November will be decommissioned on Monday.

Associate health minister Brandy Payne said the province is considering reusing the trailer to provide these services in another part of the city.

What is available on site

People who enter the clinic off of 13th Avenue S.W. will not have to show any identification or their health care card in order to use one of the six booths in the consumption area.

"We really want to be welcoming, low barrier ... and a place where people don't feel that they're being judged for their substance use. We're here to support them in their journey, no matter what that looks like," O'Gorman said.

One major benefit of the permanent location is the extra space, which will allow for more in-house, wraparound services and support staff, she added.

A separate post-consumption room will be available to clients who wish to speak with a registered dietitian or addictions counsellor.

A social worker will also be on-site to help incoming clients connect with housing, food security and poverty resources in the community, or refer people to detox services.

"We have a few clients that are already on opioid replacement therapy through the [opioid dependency program] clinic here," said O'Gorman.

The new setup is similar to the temporary trailer that has been operating at the Sheldon M. Chumir Health Centre since November.

Over the past two months, the temporary site has seen more than 2,500 visits from more than 300 individual clients, O'Gorman said.

"What that tells me is that people are coming back, which says that we're offering welcoming, warm, inclusive services to our clients, and services they want to access," O'Gorman said.

In that time, the clinic has also prevented 55 overdoses.

Edmonton sites coming as early as spring, says government

Payne says four Edmonton locations have been approved by Health Canada, with the first to open later this spring.

Three will be open to the general public. The fourth, to be located at the Royal Alexandra Hospital, will be available only to in-patients, making it the first of its kind in North America, Payne said.

Another site has been approved for Lethbridge.

"As long as Albertans are dying of preventable overdoses, we're going to have more work to do, and we're going to keep working to address the opioid crisis," Payne said.
